KRT6A Variant Underlies Pachyonychia Congenita: Insights Into Protein Aggregation and PPAR Signaling.

Variants in the keratin 6A (KRT6A) gene are a major cause of pachyonychia congenita (PC), a rare autosomal dominant disorder characterized by nail hypertrophy and other ectodermal abnormalities. This study aimed to identify the causative mutation in a PC family and investigate the underlying pathogenic mechanism. We performed exome sequencing on this PC pedigree and validated candidate variations using Sanger sequencing. In silico predictions and in vitro experiments showed that the heterozygous missense variant c.512A>G in KRT6A was pathogenic, inducing protein aggregation and disrupting fila

Dermoscopic features of nail unit melanoma: a scoping review.

Nail unit melanoma (NUM) is a diagnostically challenging cutaneous malignancy that often presents with subtle or inconspicuous clinical features, contributing to delayed recognition and poorer outcomes. Dermoscopy has emerged as a valuable tool for improving early detection, yet the existing literature is fragmented and variably reported. This scoping review synthesized 26 studies encompassing 416 patients to characterize demographic patterns and dermoscopic features of NUM. Across included studies, most affected adults are in the fifth to sixth decades of life, with lesion duration at diagnos

A case of subungual squamous cell carcinoma treated with local resection and adjunctive ALA-PDT.

The early symptoms of subungual squamous cell carcinoma are often nonspecific, leading to frequent misdiagnosis as conditions such as paronychia or subungual warts. Treatment options for this malignancy remain relatively limited in clinical practice. This study presents the case of a 74-year-old male with subungual squamous cell carcinoma who achieved significant improvement following photodynamic therapy. The patient had experienced pain and exudation of the big toe for over six months. Initially diagnosed with paronychia, he was treated with oral clarithromycin tablets (0.5 g daily) for half

Biofilm and Dermatophytoma.

Dermatophytoma is a type of onychomycosis with distinctive clinical features. Clinically, it typically appears as a linear spike, triangular, or round white- or yellow-colored mass on the nail plate. Dermatophytes are thought to adhere to the nail surface and secrete extracellular polysaccharides (EPS), proteins, DNA, and other components, forming a biofilm matrix. EPS typically encases this microbial aggregation and is synthesized by the microbial constituents of the biofilm. Herein, we review dermatophytoma and the relationship between nail infection and biofilm. We also discuss green nails

High-throughput screening of small molecule Wnt/β-catenin activators for hair and nail growth.

Hair and nails, though structurally different, both rely on Wnt/β-catenin signaling for regeneration. To identify small molecules that enhance this pathway, we screened 5,170 FDA-approved and natural compounds using HEK293 cells with a TCF/LEF luciferase reporter. Nine compounds significantly activated Wnt signaling, including antivirals (imidocarb, proflavine, aminoacridine), anticancer agents (entinostat, enzastaurin, abemaciclib), and GSK-3β inhibitors (BIO, CP21R7). Aminoacridine and proflavine markedly upregulated RSPO3 and RSPO4 while suppressing WIF1. In mice, aminoacridine-especially c

Non-coding genome in nail-patella syndrome: Genetic diagnosis as a guide for personalized follow-up.

Limb malformations are paradigmatic of altered gene regulation in human disease. Nail-Patella Syndrome (NPS) is a rare condition characterized mainly by skeletal defects, glomerulonephritis and glaucoma, with variable expressivity. NPS is caused by the haploinsufficiency or loss-of-function of LMX1B, which encodes a transcription factor involved in limb dorsalization, in the renal glomerular filtration barrier and the anterior segment of the eye. The dorsal expression of LMX1B in the developing limbs is under the control of LMX1B autoregulatory modules (LARMs), which are non-coding cis-regulat

Comparative evaluation of conventional and nanopore approaches in onychomycosis diagnosis.

BACKGROUND: Onychomycosis is a frequent and chronic nail infection caused by dermatophytes, yeast, and non-dermatophyte molds (NDM). Detection of the causative agent is relevant to overcoming recurrent infections and improving treatment. Combined microscopy and culture are the diagnostic gold standard; however, more sensitive tests are needed to speed detection. This work aimed to assess fungal nail diagnosis using conventional and molecular techniques. We explored the potential of nanopore sequencing for fungal detection. METHODS: A total of 104 nail samples from 51 patients with onychomycos

Nail Patella Syndrome Diagnosed in a Proband With Renal Failure Through Skeletal and Nail Deformities in Her Offspring: A Case Report of a Novel De Novo Genetic Defect in the LMX1B Gene.

Nail-patella syndrome (NPS) is an autosomal dominant genetic condition characterised by dysplastic nails, skeletal deformities and potential kidney involvement. It is caused by mutations in the LMX1B gene. In this case, we report a proband and her son, both affected by NPS. The proband presented with kidney failure and exhibited classical NPS deformities that had gone undiagnosed previously. As her son also had similar skeletal deformities, genetic testing for NPS was done, which revealed a novel missense genetic defect in the LMX1B gene. Interestingly, neither the parents nor siblings of the

Verweija noviomagensis gen. sp. nov.-A novel member of the Onygenales incertae sedis isolated from a dystrophic nail.

The order Onygenales includes keratinophilic fungi, such as dermatophytes, that can cause onychomycosis. Although dermatophytes are the primary cause of these infections, some non-dermatophytic, keratinophilic onygenalean fungi have been reported as causing nail infections. Other such fungi are frequently isolated as surface contaminants of nails sent for culture, but are not etiologic agents of onychomycosis. Here we introduce a novel onygenalean fungus isolated in the Netherlands from a nail that was suspected to have a fungal infection. As only a single sample was available, etiologic invol

Pediatric and Adolescent Nail Disorders.

Nail disorders are a frequent reason for dermatological consultation in children and adolescents. While most are benign and self-limited, some may signal underlying systemic diseases or genetic syndromes. Understanding the anatomical and physiological characteristics of the pediatric nail unit is essential for accurate diagnosis and appropriate management. We aimed at conducting a structured review of the most common pediatric nail alterations, including physiological, congenital, infectious, inflammatory, pigmentary, and tumoral conditions.

Deposition of Heavy Metals in Patients with Deep Venous Thrombosis and Healthy Individuals: A Case-Control Study with Laser-Induced Breakdown Spectroscopic Analysis of Nail Edges.

Background: Deep vein thrombosis (DVT) is one of the most common cardiovascular diseases and is especially prevalent in areas with environmental pollution. Bioaccumulation of toxic heavy metals may lead to deterioration of homeostasis with cellular change, endothelial dysfunction, DNA impairment and cellular signaling. The reason for this is usually the accumulation of thrombogenic toxins in the body as a result of long-term exposure or a lack of regulatory gene expression. In this study, we aimed to measure the minerals that potentially accumulate in the nail. The measurement method was laser

Detection of dermatophytes, Candida spp., and molds by the EuroArray Dermatomycosis® microarray versus fungal culture in patients with suspected dermatomycosis.

Highly sensitive molecular assays have become standard in most microbiology laboratories for the detection of a large variety of pathogens. For dermatophytes, however, fungal culture is often still pursued as the routine method although it requires weeks before results are available. We were interested in whether dermatology patients might benefit from the performance of a commercially available microarray that detects 23 dermatophytes, three yeasts, and three molds. We consecutively examined 49 skin specimens and 52 nail samples from 99 outpatients by both microarray and culture for the detect
