Revisiting onychomycosis: Etiological agents and antifungal susceptibility profile.

Onychomycosis has a prevalence varying from 2% to 8% at present. The incidence is rising day-by-day worldwide. There are only a few studies that have looked into resistance pattern of onychomycosis isolates, and so the present study was planned to study the epidemiology and mycological profile, including antifungal susceptibility testing, for patients presenting with onychomycosis. The present prospective study (January 2019 to June 2020) was conducted on a total of 92 clinically suspected patients of onychomycosis as per standard mycological techniques. Antifungal susceptibility testing (AFST

Determinants of mortality following intertrochanteric fractures, comorbidities versus reduction quality: retrospective cohort study.

BACKGROUND: Intertrochanteric femoral fractures are common in the elderly population and are associated with substantial mortality. While fracture reduction quality is known to influence mechanical outcomes, its relative impact on mortality compared with patient-related comorbidities remains unclear. The objective of this study was to evaluate the impact of fracture reduction quality on survival in patients with intertrochanteric hip fractures, relative to the effect of comorbid diseases. METHODS: This retrospective cohort study included 297 elderly patients who underwent surgical treatment w

Post-Finasteride Syndrome: Survey of Dermatologists From the Spanish Hair and Nail Disorders Group.

BACKGROUND AND OBJECTIVE: Post-finasteride syndrome (PFS) refers to a set of persistent symptoms reported by patients after discontinuation of 5-alpha-reductase inhibitors (5-ARIs). This study aimed to explore the perception of PFS among dermatologists belonging to the Spanish Hair and Nail Disorders Group. MATERIALS AND METHODS: We conducted a qualitative literature review to develop a survey. Fifty-four dermatologists participated in a questionnaire based on a Likert scale. Consensus was defined using specific statistical criteria, including a median within a predefined range and an interqu

Six-month open-label clinical study of a novel topical solution for mild distal lateral subungual onychomycosis.

Systemic antifungals such as terbinafine and itraconazole are often limited by drug interactions and side effects in the treatment of onychomycosis. We tested a new topical solution to address the clinical need for safer and more effective topical therapies. The solution (containing urea, lactic acid, ethoxydiglycol, decylene glycol, and polyquaternium-7) was applied twice daily for six months in a single-arm, open-label trial, enrolling 50 adult subjects with clinically diagnosed distal lateral subungual onychomycosis. Outcomes included a blinded investigator assessment, mycological evaluatio

[Diagnosis and management of dermatophyte onychomycosis].

This article provides an updated management algorithm for dermatophyte onychomycosis (OM), aimed at standardizing diagnostic and therapeutic decision making. OM is a common condition in dermatology and primary care settings. Nail treatment should only be initiated after mycological confirmation, as approximately half of nail dystrophies are attributable to fungal infection. Unlike inflammatory nail disorders, dermatophyte OM is quite easily curable when managed appropriately. Accurate diagnosis, appropriate treatment selection, patient adherence, and consistent clinical follow-up are key to su

The protocol for a patient-driven online prospective European observational cohort aiming to determine risk factors for the development of psoriatic arthritis among people living with psoriasis: the HIPPOCRATES Prospective Observational Study.

BACKGROUND: Up to one-third of people living with psoriasis develop psoriatic arthritis (PsA), and the majority have active psoriasis prior to the development of arthritis. Clinical risk factors, such as nail involvement, in conjunction with novel blood biomarkers, could improve PsA risk monitoring and early diagnosis. OBJECTIVES: The aim of the HIPPOCRATES Prospective Observational Study (HPOS-www.hpos.study) is to follow a cohort living with psoriasis and identify risk factors for the development of PsA. DESIGN: HPOS is a patient-driven online prospective European observational cohort. ME

Corrective and Restorative Dermatology in Cancer Survivors: An Urgent Unmet Need!

The increasing efficacy of oncological therapies has led to a rapidly growing population of cancer survivors, shifting clinical attention from acute treatment-related toxicities to long-term and late sequelae. Among these, dermatological sequelae represent some of the most visible and persistent consequences of cancer treatments and may significantly impair quality of life. Persistent alopecia, nail disorders, pigmentary alterations, chronic radiation-induced skin changes, scarring, vascular abnormalities, mucosal alterations, and secondary skin cancers may develop or persist long after treatm

Advances in Nail Evaluation: US and MRI.

High-quality imaging with CT, US, and MRI enhances the evaluation of nail disorders, but these modalities have strengths and weaknesses. Radiography is a useful adjunct in the evaluation and diagnosis of nail diseases such as subungual exostosis and tumors such as glomus tumors and squamous cell carcinoma. The authors review normal nail anatomy with imaging correlations and illustrate a wide spectrum of nail pathologic disorders, including inflammatory, infectious, traumatic, and neoplastic conditions. Furthermore, the authors discuss dedicated imaging protocols and practical tips to optimize

Definition of the terms used to describe psoriatic disease datasets: a HIPPOCRATES initiative.

Psoriatic arthritis (PsA) is a complex, multisystem disease with no diagnostic criteria and discordant approaches to assessing disease activity and response to treatment. PsA is part of the Psoriatic Disease spectrum, encompassing cutaneous psoriasis and nail changes, PsA and in some cases, uveitis and inflammatory bowel disease. The HIPPOCRATES consortium is addressing unmet needs in PsA and has now established a harmonised data platform, the Secure HIPPOCRATES Data Management Platform (SHDMP), which will be transformative in providing researchers with the ability to interrogate at scale psor

Increased Serum Cold-Inducible RNA-Binding Protein Levels in Psoriasis.

BACKGROUND: Psoriasis is a chronic inflammatory skin disorder typified by well-demarcated erythematous plaques with scales. While considered an immune-driven condition, its underlying molecular triggers remain insufficiently defined. Cold-inducible RNA-binding protein (CIRP), a stress-response protein, has recently been recognized as a damage-associated molecular pattern that can stimulate immune responses. OBJECTIVE: This study aimed to explore the potential association between circulating CIRP levels and the clinical as well as histological characteristics of psoriasis. METHODS: Serum CIRP

Clinical, functional, and quality-of-life outcomes after arthroplasty versus osteosynthesis for metastatic lesions of the proximal femur: a 12-month observational cohort study.

BACKGROUND: Metastatic lesions of the proximal femur frequently result in severe pain, impaired mobility, and pathological fractures. Surgical intervention aims to restore skeletal stability, allow early mobilization, and improve quality of life. This study compared clinical, functional, and quality-of-life outcomes following endoprosthetic reconstruction (arthroplasty) versus intramedullary osteosynthesis in patients with metastatic proximal femoral disease. METHODS: This single-center observational cohort study included 54 patients treated with either arthroplasty (n = 27) or intramedullary

Keratin 16 inhibits type I interferon responses in differentiating keratinocytes of stressed and diseased skin.

The stress-induced keratin 16 intermediate filament gene, KRT16, and K16 protein are spatially restricted to the suprabasal layers of the epidermis and widely used as biomarkers in psoriasis, hidradenitis suppurativa, atopic dermatitis, and other inflammatory disorders. Pathogenic variants in KRT16 cause pachyonychia congenita (PC), a rare genetic condition in which tissue homeostasis is disrupted in palmoplantar epidermis, nail, oral mucosa, and other KRT16-expressing epithelial appendages. How K16 may regulate the molecular mechanisms underlying these conditions is poorly understood. Here, w

Nail: News in Onychopathology.

Onychopathology (nail unit pathology) is a niche area in dermatopathology. There is a lack of familiarity with nail unit pathology and basic anatomy in general among pathologists and dermatopathologists, because of the relative rarity of such specimens in practice, and the lack of exposure and teaching regarding these lesions in training. It is however a vibrant area of dermatopathology, with knowledge evolving rapidly. In this article, we explore new information in the analysis of melanocytic and epithelial lesions of the nail unit, as well as nail inflammatory disease.

Optimization of nail therapies: the role of microstructure in the absorption and efficacy of topical treatments.

The most frequent nail diseases are onychomycosis and nail psoriasis. These pathologies must be treated pharmacologically over extended periods, with topical therapy being the most desirable option to avoid the side effects linked to systemic administration. However, the low permeation of drugs applied topically to the nail plate limits the effectiveness of these treatments. The nail is a highly keratinized structure attached to the skin with essential protective and mechanical functions, but its composition, nature, and compact, dense structure limit the permeability of topically applied subs

Acanthotic subtype of onychocytic matricoma - A rare case with unusual features.

Onychocytic matricoma (OCM) is a benign tumor of the onychocyte. It clinically presents as localized longitudinal pachyonychia and melanonychia. It has many clinical and histopathological differential diagnoses. A 54-year-old male presented with a 2-year history of melanonychia on the left thumb. Local examination revealed a 2 mm skin colored papule over the proximal nail fold. The proximal nail plate showed longitudinal melanonychia with subungual hyperkeratosis. Histopathological examination of the nail bed revealed proliferation of basaloid epithelial cells. There were many organized sphere

Case report: dual pathology of LMX1B-associated nephropathy and iga nephropathy in a middle-aged woman.

BACKGROUND: Proteinuria is a common manifestation of glomerular disease. Advances in genetic testing have improved recognition of hereditary nephropathies such as LMX1B-associated nephropathy. Coexistence with immune-mediated glomerulonephritis is exceptionally rare. CASE REPORT: We describe a 58-year-old woman with a history of hypertension, type 2 diabetes mellitus, hyperlipidemia, and prior pulmonary embolism who presented with persistent proteinuria. Urinary protein-to-creatinine ratios ranged from 1679 to 3944 mg/g, while renal function remained preserved (estimated GFR 69-103 mL/min/1.7
