[Pachydermoperiostosis].

Pachydermoperiostosis (Friedreich-Erb-Arnold syndrome, Touraine-Solente-Golé syndrome) is a rare disease with an autosomal dominant mode of inheritance; it occurs almost exclusively in men. A report of typical pachydermoperiostosis in a 31-year-old man is presented. Associated features were clubbed digits of the hands and feet with watch-crystal nails, thickening of the skin and soft tissues, hyperhidrosis of hands and feet, hyperplasia of the sebaceous glands with seborrhoea, gynaecomastia, and ophthalmic abnormalities that had been present since puberty. Radiological examination revealed per

Nail abnormalities in nondermatologic patients: prevalence and possible role as diagnostic aids.

BACKGROUND: Nail changes have traditionally been used as diagnostic aids. Their usefulness in general and predictive value in particular, however, is not known. OBJECTIVE: Our purpose was to survey nail changes and to clarify the potential of nail abnormalities to diagnose nondermatologic diseases. METHODS: The fingernails of 567 patients admitted to general medical and surgical wards were examined. The point prevalence rates were calculated on the basis of a binomial distribution. The results were compared with the diagnosis with logistic regression analysis correcting for age and sex. RES

[Vörner keratosis palmoplantaris diffusa. Clinical, formal genetic and molecular biology studies of 22 families].

In 1901, Vörner described a diffuse keratoderma of palms and soles with autosomal dominant inheritance. Histopathologically, this disease has the typical features of epidermolytic hyperkeratosis. Clinical examination does not allow differentiation between keratoderma of the Vörner type and the keratoderma described by Thost in 1880 and Unna in 1883. Reexamination of the family originally seen by Thost revealed histopathological signs of epidermolytic hyperkeratosis, confirming that keratoderma of the Vörner type is present in this family. The clinical features and variability of this palmoplan

[Cutaneous manifestations of liver cirrhosis].

It is well recognized that liver cirrhosis may cause many characteristic skin changes. This paper deals with skin manifestations of liver cirrhosis, according to the pathological process. Portal hypertension may cause development of collateral circulation, including, visible varicose veins on the abdominal wall ("caput Medusae"). Hormonal disturbances, as the result of inability of the liver, are responsible for several vascular changes, such as, red palm, vascular spider and "paper money" skin, for gynecomastia and for loss of axillary and pubic hair. Nail changes, such as, clubbing, white na

[Three cases of arteriovenous malformation of the lung].

Three cases of arteriovenous malformation (AVM) of the lung are presented. The first patient was 36-year-old female and the second was 25-year-old male, and both with asymptomatic bilateral multiple small AVMs. We used coil spring occlusion in therapeutic transcatheter embolization of the AVMs. A pulmonary arteriogram confirmed the completely occluded AVMs. The another patient was 21-year-old female whose lips and nails were cyanotic. She had clubbing fingers and vascular murmur was audible on her chest wall. The laboratory examination revealed polycythemia and hypoxemia. A pulmonary angiogram

[Idiopathic clubbing of the fingers. Pathogenetic mechanisms and differential etiologic diagnosis].

We report on a patient with marked clubbing of the fingers and toes with watch-glass deformity of the nails, diagnosed as idiopathic clubbing. New findings on the pathogenesis of clubbing provide evidence for the important role of cytokines, especially platelet-derived growth factor and tumor necrosis factor-alpha. The differential diagnosis includes, besides rare primary forms, clubbing in malignant neoplasias and chronic inflammatory diseases of the heart, the lung, the upper gastrointestinal tract and the liver. Clubbing can precede other symptoms of neoplasias by mouths and could be depend

The aetiology of clubbing and hypertrophic osteoarthropathy.

The evidence is reviewed for the hypothesis that clubbing and hypertrophic osteoarthropathy are due to the peripheral impaction of megakaryocytes and platelet clumps in the fingers and toes, to which this particulate matter has passed in an axial stream. The normal pulmonary vascular bed retains these large particles, which fragment before entering the systemic circulation. A right-to-left shunt allows them to bypass the pulmonary vascular bed. A preliminary histological report of platelet clumps seen at necropsy in nail bed capillaries of clubbed fingers supports the hypothesis. Platelets con

The medical and surgical treatment of finger clubbing and hypertrophic osteoarthropathy. A blind study with colchicine and a surgical approach to finger clubbing reduction.

In fourteen patients affected with pachydermoperiostosis (PDP), or primary hypertrophic osteoarthropathy, the efficacy of colchicine (0.5 mg day for one month) versus placebo on the main clinical features of the disease (finger clubbing, arthritis and pachydermia) was evaluated. In addition, in one patient the usefulness of surgical reduction of clubbed fingertips was investigated. Colchicine did not demonstrate any appreciable effect on finger clubbing (expressed in degrees) or pachydermia, while an effect on arthralgia (as evaluated by the Ritchie Index and Pain Scale) was observed. The surg

Osteoid osteoma of the distal phalanx of the finger: a diagnostic challenge.

Osteoid osteomas of the distal phalanx of fingers are uncommon. An extensive review of the literature indicates that the diagnosis of osteoid osteoma of the distal phalanx is often delayed for several months to years (average 34.3 months). Pain is the most common finding (92 percent), followed by swelling and clubbing (75 percent). Relief of pain by aspirin was indicated in 10 patients (42 percent). Multiple operations were done in 7 patients prior to the appropriate diagnosis and treatment. A typical case report is presented to illustrate the difficulties in diagnosis and treatment of this be

[Clubbed fingers, hourglass nails].

Clubbing was the main clinical symptom in a 27 year old male patient. There was no clubbing in other family members. Secondary clubbing was excluded by clinical findings, laboratory, x-ray of the chest, electrocardiography, pulmonary function test, blood gas analysis, colonoscopy, x-ray of hands and feet and capillary microscopy. Final diagnosis was idiopathic clubbing.

The harlequin nail. A marker for smoking cessation.

Changes in the human nail frequently serve as an indicator of local and systemic disease. Alterations in the morphology, structure and growth characteristics of the nail accompany chronic cigarette smoking; yellow pigmentation of the nail plate--referred to as the "nicotine sign"--is common. The clubbed yellow nail may indicate the presence of lung cancer. In contrast to the ominous nature of the clubbed yellow nail, we describe a sign that is more propitious: the sudden cessation of smoking due to an intercurrent disease, often a cerebrovascular accident (CVA), leads to the development of a d

Digital clubbing: a numerical assessment of the deformity.

We describe a simple method that defines with numbers the drumstick deformity of the digits. We measured 2 circumferences on each of the 10 fingers at the nail bed (NB) and at the distal interphalangeal joint (DIP), the sum of the 10 ratios NB:DIP was termed Digital Index. Intraobserver variation of the Digital Index was small, with a Pearson's correlation coefficient of R = 0.979; interobserver variation was also not significant (R = 0.999). Making use of this Digital Index, we studied 22 patients with digital clubbing associated with cyanotic congenital heart disease and 66 healthy controls.

A study of capillary morphology in the digits of patients with acquired clubbing.

We sought to determine the capillary morphology in patients with clubbing to compare it with that in subjects without clubbing. Subjects were placed in the clubbed or nonclubbed groups on the basis of caliper measurements of their index fingers. In each subject, photomicrographs of eight fingers were done focusing on the dorsal skin just proximal to the base of the nail. Each photomicrograph was randomly graded with respect to the presence and extent of plexus formation, the presence of arborized loops, and the presence of splayed loops. Comparison of the plexus score, presence of arborized lo

The pathology of clubbing: vascular changes in the nail bed.

In this study post-mortem angiograms were prepared from both normal and clubbed hands and attempts were made to estimate the size of the vascular bed in the terminal parts of each finger. Representative cases were also examined histologically. There were no differences between the groups. The increase in blood flow in clubbing is the result of vasodilatation rather than hyperplasia of vessels in the nail bed.

Clubbing and koilonychia.

Although some cases of spoon nails and clubbing are manifestations of internal disease, a significant number are completely idiopathic and of no consequence to the patient. Many patients have been forced to pay high insurance premiums or have been denied employment because of insignificant nail changes. When a patient has spoon nails or clubbing, a thorough but reasonable search for a primary etiology should be made. It should be recalled that when the complete syndrome of hypertrophic osteoarthropathy is present and joint pain is prominent, the possibility of a bronchogenic carcinoma is great

Skin, hair and nail changes in a case of citrullinemia with late manifestation.

We report the case of a 56-year-old woman with a history of episodes of vomiting and coma. Hyperammoniemia , hypouremia , hypercitrullinemia and the decreased arginino -succinic synthetase activity on skin fibroblast cultures permitted us to diagnose citrullinemia. We think that the skin changes, i.e. atrophy of the epidermis, thin and short collagen bundles, dystrophic elastic fibers, increased fundamental matrix, were due to the undernutrition state and were not specific. The dosage of the amino acids of the total skin showed a decreased hydroxylation of lysine in the insoluble fraction, and

[Pachydermoperiostosis (Touraine-Solente-Golé syndrome). Clinical and radiological differential diagnosis (author's transl)].

This report describes a patient with pachydermoperiostosis (Touraine-Solente-Golé-syndrome). This disorder is characterized by clubbing of the terminal phalanges with watchglass-like nails, enlargement of hands and feet, pachydermia and periostotic skeletal changes. The signs of pachydermia are cutis verticis et frontis gyrata, ptosis and increased skin-thickness of the palms of the hand and soles of the feet. Frequently, a hyperhidrosis of the hands and feet can be found. The differential diagnosis of clinical symptoms and changes in the bones is discussed in detail.
