Our goal was to analyze hematological ratios in patients with pediatric alopecia areata (AA) and assess their potential as markers of disease exacerbation. A retrospective cohort study of 31,869 patients (aged 0–18) with AA was conducted using blood samples collected within 30 days of diagnosis and 12–18 months prior. Univariate and multivariable analyses were used to compare the hematological ratios among healthy controls, patients with mild AA, and patients with severe AA. Data were gathered from Israel’s Clalit database and Soroka Hospital, Beer Sheva, and represented both Arab and Jewish p

Alopecia areata is a typical skin disease with unmet needs. So far, it has been understood that the main cause of the intractability of chronic cases is the decrease in inflammatory cell infiltration and falling into the telogen-like phase. However, in some cases, even in long-term chronic cases, inflammatory cell infiltration can be seen, so that we speculate that the long-term persistence of these perifollicular cells may be the reason for the lack of improvement as skin resident memory T (TRM) cells. To investigate the presence of TRM, nine treatment-resistant chronic AA patients and 5 acut

Alopecia areata (AA), an autoimmune condition characterized by non-scarring hair loss, affects 2% of the United States population. AA is a CD8 T-cell driven autoimmune disease in which the cytokines interferon-gamma and interleukin-15 play a central role in disease progression. These pathways depend on Janus kinase (JAK) signaling, making JAK inhibitors an ideal therapeutic option in treating AA. Deuruxolitinib, an oral JAK inhibitor, was approved by the US Food and Drug Administration in July 2024, for the treatment of severe AA in adults. Deuruxolitinib is the third oral JAK inhibitor approv

BACKGROUND: The Severity of Alopecia Tool (SALT) score is widely used. However, its inter- and intrarater reliability and smallest detectable difference (SDD) have not been rigorously evaluated.
OBJECTIVES: To determine the interrater and intrarater reliability of the SALT score and to quantify its SDD across different levels of disease severity. We assessed the diagnostic accuracy of the SALT score for identifying key clinical thresholds [for example, SALT score of ≥ 50% (SALT 50)].
METHODS: Ten patients were scored live by 3 experienced raters, and 53 patients were scored by 11 raters base

Alopecia Areata (AA) is a chronic disorder with significant psychological impact due to its unpredictability. While emotional distress (ED) is well-recognized in AA, the interplay between illness perception (IP) and obsessive-compulsive (OCD) symptomatology remains underexplored. This -sectional, observational study aimed to investigate the prevalence of OCD symptoms and ED in AA outpatients, analyzed the relationship between IP and OCD symptomatology. One-hundred-thirty-five AA outpatients, from a specialized Hospital in Rome, Italy, were recruited. Participants completed the DASS-21 for ED,

Background and Objectives: Oral Janus kinase (JAK) inhibitors have become an important therapeutic class in dermatology, with approved indications including atopic dermatitis and alopecia areata. Owing to their broad immunomodulatory effects and rapid onset of action, these agents are increasingly used off label for a variety of inflammatory skin disorders that are often refractory to standard therapies. The objective of this review was to provide a comprehensive overview of the published literature on the off-label dermatologic use of oral JAK inhibitors, summarizing clinical outcomes, safety

INTRODUCTION: Alopecia areata (AA) is a common autoimmune disorder characterized by nonscarring hair loss on the scalp and/or body, affecting individuals of all genders, ages, and races. AA can occur alongside various autoimmune, atopic, and psychiatric conditions, affecting patients' quality of life. Although AA is a relatively common condition, recent epidemiological data regarding this condition in Taiwan are lacking.
METHODS: This systematic literature review aimed to assess the epidemiology, risk factors, and comorbidities associated with AA in Taiwan by analyzing articles published from

OBJECTIVE: To provide a comparative overview of the post-marketing safety patterns of baricitinib and tofacitinib used for the treatment of adult alopecia areata (AA) by analyzing spontaneous reports from the FDA Adverse Event Reporting System (FAERS). Given the different regulatory approval statuses and real-world usage contexts of the two drugs, this study aims to identify potential safety signals rather than to estimate incidence or establish causal associations.
METHODS: FAERS data from 2004Q1 to 2024Q1 were retrieved and systematically cleaned using FDA-recommended deduplication procedur

Alopecia areata (AA) and androgenetic alopecia (AGA) both present with hair loss but require different therapies, and reliable biomarkers to guide treatment remain lacking. We integrated bulk and single-cell RNA-seq to compare JAK-STAT signaling in AA versus AGA. In AA, 257 immune-enriched differentially expressed genes (DEGs) were identified; WGCNA and consensus machine learning (LASSO, SVM-RFE, random forest) yielded six candidate hub genes, and external validation narrowed these to four key genes-granzyme A (GZMA), interleukin-2 receptor β (IL2RB) and γ (IL2RG) chains, and eomesodermin (E

BACKGROUND: Alopecia areata (AA) is a common autoimmune disease which causes non-scarring patches of hair loss and has a 2% lifetime risk. Its chronic, recurrent nature makes treatment difficult. Potential roles of the JAK/STAT pathway, long non-coding RNAs (lncRNAs), & microRNAs (miRNAs) are highlighted by recent discoveries on the pathophysiology of alopecia areata (AA).
METHOD: This case-control study aimed to assess the role of lncRNA H19, miR-29a, and the JAK2/STAT3 pathway in alopecia areata (AA). We enrolled 29 patients and 30 healthy controls, collected venous blood from all participa

Through a multidisciplinary quality improvement initiative, the Pre-Immunosuppression (Pre-IS) Clinic was created at a tertiary referral institution to ensure appropriate vaccination and infectious disease screening for patients on immunosuppressive medications. Consensus guidelines on immunisation and infectious disease screening for immunosuppressed patients were created through a multidisciplinary committee. The guidelines included three sections: (1) screening recommendations for chronic/latent infections prior to immunosuppression, (2) immunisation recommendations for immunosuppressed pat

BACKGROUND: This study quantifies the global burden of alopecia areata (AA) in young adults (15-49 years) from 1990 to 2021 using data from the Global Burden of Disease 2021 study.
OBJECTIVE: To analyze trends in AA incidence and disability-adjusted life years (DALYs) across demographic and geographic strata.
METHODS: We evaluated age-standardized incidence rates (ASIRs) and age-standardized DALYs rates, stratified by sex, age, and Socio-demographic Index (SDI). Estimated annual percentage changes (EAPCs) were calculated to assess trends.
RESULTS: Global ASIR (EAPC, -0.20) and DALY rates (E

ETHNOPHARMACOLOGICAL RELEVANCE: Platycladus orientalis (L.) Franco is traditionally used to promote hair growth. Cedrol (CE) is the main active ingredient in Platycladus orientalis (PO), promoting hair growth. However, the anti-alopecia areata (AA) effect and mechanism of action of CE in the imiquimod-induced AA model have not been reported.
AIM OF THE STUDY: The macrophage polarization imbalance is a key pathological driving factor for AA. This study aims to elucidate the potential mechanism by which CE ameliorates AA by regulating macrophage polarization homeostasis.
MATERIALS AND METHODS:

We report the case of a 50-year-old woman with severe palmoplantar psoriasis who developed alopecia areata (AA) 2 months after initiating treatment with tildrakizumab (100 mg subcutaneously). The patient had no personal or familial history of autoimmune disorders, and the diagnosis of AA was confirmed by clinical and trichoscopic evaluation. Tildrakizumab was discontinued, and the patient was treated with topical corticosteroids and low-dose oral methylprednisolone, achieving complete hair regrowth within 6 months. Psoriasis subsequently recurred, and treatment was successfully switched to bim

BACKGROUND: Janus kinase inhibitors (JAKis) have transformed the treatment of alopecia areata (AA), but not all patients respond to initial therapy. The utility of switching between JAKis in treatment-refractory AA remains underexplored.
OBJECTIVE: To evaluate the efficacy and safety of switching between JAKis in patients with severe AA.
METHODS: In this multicenter retrospective study, we analyzed 108 patients with severe AA who switched between oral JAKis after a minimum treatment duration of 6 months. Treatment response was assessed using the Severity of Alopecia Tool (SALT). Multivariabl

BACKGROUND: Alopecia areata (AA) is a common autoimmune disease characterized by non-scarring hair loss. The disease occurs in both sexes, with no racial or age-related predilection. Many patients experience spontaneous improvement and regrowth of hair, while some patients are resistant to common therapies and others progress to more severe forms of alopecia. The primary treatment for patchy and limited alopecia of the scalp and face is intralesional triamcinolone injection. Previous studies have also reported the efficacy of cryotherapy (liquid nitrogen), which promotes hair regrowth through
