Prepared or at-risk? Evaluating live viral vaccine coverage and vaccine immunogenicity before pediatric solid organ transplantation.

Pediatric solid organ transplant recipients are at high risk for complications from vaccine-preventable diseases, yet pre-transplant vaccination coverage is often inconsistent. We conducted a retrospective cohort study of 421 pediatric transplant recipients between 2018 and 2024 to evaluate vaccination status and serologic immunity for varicella and measles at the time of initial evaluation, listing, and transplantation. At the time of transplant, 19.2% of patients for varicella and 21.6% for measles were under-vaccinated, with infants having the highest rates of incomplete vaccination. A port

Guideline for the Diagnosis and Management of Heritable IFNAR1 Deficiency in Oceania.

Autosomal recessive interferon alpha and beta receptor subunit 1 (IFNAR1) deficiency is a rare and heritable inborn error of immunity (IEI) predisposing individuals to severe and life-threatening viral infections. It is more common in people of Western Polynesian ancestry, with estimates of around one in six thousand live births affected, due to being homozygous for or having two copies of the regionally relevant pathogenic IFNAR1 variant c.1156G>T, p.Glu386*. IFNAR1 deficiency confers an increased risk of severe and life-threatening infections caused by naturally circulating viruses including

[Acute Measles Encephalitis].

Measles is acute and highly infectious. Febrile rash may be complicated by severe pneumonia or encephalitis. Acute measles encephalitis is a rare condition that develops within a few days of the appearance of a rash and presents with high fever, altered consciousness, seizures, and focal neurological signs. It frequently results in death, and survivors often experience residual neurological sequelae. No specific antiviral therapy is available. Management is primarily supportive. In Japan, the incidence is low owing to high vaccination coverage; however, maintaining a two-dose vaccination sched

When measles is not benign: meningoencephalitis and status epilepticus in an unvaccinated adolescent (case report).

Measles is a highly contagious viral exanthematous disease, caused by a Morbillivirus, that continues to cause outbreaks in under-immunized populations. While neurologic complications are rare, the progression to meningoencephalitis with convulsive status epilepticus is exceptional, particularly in immunocompetent adolescents. We report a 17-year-old unvaccinated male who presented with a febrile, descending asymptomatic maculopapular rash, lacking Koplik spots, followed by rapid onset of severe headache, photophobia, vomiting, and convulsive status epilepticus. Day-7 IgM was negative, and PCR

Opportunistic CNS Viral Infections in Immunocompromised Patients.

Immunocompromised patients face increased morbidity and mortality from central nervous system (CNS) opportunistic viral infections, with risk level driven by specific immune defects. While our diagnostic capacity expands, immunocompromised patients may present atypically or with mild symptoms, delaying or complicating diagnosis. Treatment options are often limited, although novel agents show promise. This review discusses why immunocompromised patients carry higher immunologic risk and how certain viral classes display neurotropism to invade the CNS. We outline the utility and limitations of c

[Rapid progression of neurological symptoms in an adult].

Measles is a highly contagious viral disease whose incidence has declined significantly since the introduction of vaccination in the mid-1970s. However, local outbreaks continue to occur due to insufficient vaccination rates. Adults are at higher risk of complications. The present case of a patient with acute post-infectious measles encephalitis despite a history of presumed measles vaccination underscores the critical importance of vaccination as a preventive measure. In everyday general practice, there is an opportunity to identify individuals who have not been vaccinated against measles and

Mumps Virus: Replication, Immune Response, and the Changing Landscape of Vaccine Effectiveness.

Mumps virus (MuV) is a single-stranded, negative-sense RNA virus of the Family Paramyxoviridae. MuV is a highly contagious human pathogen that causes primarily mild symptoms, including hallmark swelling of the parotid glands. Severe cases can occur, leading to neurological complications, including deafness, meningitis, and encephalitis. The mumps vaccine, now included in combination with measles and rubella vaccines (MMR), was first made available in the 1960s. After its introduction, mumps incidence dropped dramatically to less than 500 cases annually in the US. However, even with long-standi

In recovered COVID-19 patients, the state of mucosal immunity remains understudied. Cytological, functional, and metabolic characteristics of neutrophils and the interleukin status will help to correctly assess the need for immunorehabilitation measures. The study objective is to assess the state of mucosal immunity after COVID-19. A comprehensive study of mucosal immunity included the assessment of nasal mucosal neutrophils with the monitoring of destructive and apoptotic changes as well as examination of the functional and metabolic activity of neutrophils entering the nasal secretions. Adult-Onset Subacute Sclerosing Panencephalitis Presenting With Subacute Cognitive Deficits.

We describe the case of a 41-year-old man diagnosed with adult-onset subacute sclerosing panencephalitis (SSPE). The patient presented with subacute progressive cognitive deficits and a neuropsychological profile indicating predominant frontoparietal dysfunction. MRI showed only mild parietal-predominant cerebral atrophy. The patient later developed periodic myoclonic jerks with time-locked periodic slow wave complexes on electroencephalography. Evidence of intrathecal synthesis of anti-measles IgG was identified in the cerebrospinal fluid. The patient satisfied modified Dyken diagnostic crite

Subacute sclerosing panencephalitis in a child six years after measles infection.

Measles is a highly contagious viral illness that remains endemic in many parts of the world, posing significant risks of severe and long-term complications. We report the case of an 8-year-old boy hospitalized for epilepsy with myoclonic spasms and cognitive decline. Clinical and paraclinical investigations, including electroencephalography, imaging tests, and cerebrospinal fluid (CSF) analysis, suggested a neurodegenerative process. The patient had contracted measles at the age of two and had not received full vaccination. Elevated measles-specific immunoglobulins and intrathecal focus of sy

Preliminary evidence of cardiac dysfunction in subacute sclerosing panencephalitis: a systematic review and meta-analysis.

PURPOSE: Subacute sclerosing panencephalitis (SSPE) is a catastrophic neurological disorder that typically presents in infancy and early adolescence. Cardiac arrhythmia in SSPE is a significant but often under-recognized complication of the disease. The persistent measles virus infection that characterizes SSPE can have widespread effects on the central nervous system, including areas that regulate autonomic function, which in turn can impact cardiac rhythm. In this article, we aim to comprehensively review articles describing cardiac dysfunction associated with SSPE. METHODS: We conducted a

Clinico-virological investigation of measles complications in immunocompromised pediatric patients.

Measles is a highly contagious yet vaccine-preventable disease that is re-emerging in settings with suboptimal immunization, putting immunocompromised children at particular risk. Although measles complications are well documented in children, the data on measles-related complications in immunocompromised hosts remain scarce. We conducted a clinico-virological and bioinformatics investigation of five immunocompromised pediatric measles cases: four with measles-associated encephalitis consistent with measles inclusion body encephalitis (MIBE) and one with severe pneumonia. Cases were confirmed

Hemiballistic hemi-myoclonus in Subacute Sclerosing Pan encephalitis - A novel observation.

We describe three patients with subacute sclerosing panencephalitis (SSPE) presenting with hemiballistic slow myoclonus, an unusual and rarely reported motor phenotype. Periodic slow myoclonus is a hallmark clinical feature of SSPE and is considered pathognomonic, yet its expression is typically axial or generalized. Hemiballistic presentation of slow myoclonus is distinctly uncommon and has received little attention in the literature. These cases expand the recognized motor repertoire of SSPE and highlight a variant expression of periodic cortical discharges involving asymmetric ballistic rec

Earlier Onset of Subacute Sclerosing Panencephalitis in Bangladeshi Children: Insights from a Case Series.

Subacute sclerosing panencephalitis (SSPE) is a fatal neurodegenerative complication of measles, characterized by a progressive decline in neurological function. This case series described 42 children diagnosed with SSPE and received treatment at a tertiary neurology hospital in Dhaka, Bangladesh, from the year 2019 to 2021. The median age at onset of was 4 years (range: 2.5-11 years), with a mean latent period of 2.68 ± 0.97 (SD) years. While 30 cases (71.4%) were immunized, 23 of these children developed SSPE despite receiving immunization after 9 months of age. Additionally, a small proport

A rare homozygous mutation in TYROBP resulting in early-onset dementia with bone cysts.

Nasu-Hakola disease (NHD) is a rare autosomal recessive disorder characterized by progressive dementia and multiple bone cysts. The known pathogenic genes include TYRO protein tyrosine kinase binding protein (TYROBP) on chromosome 19q13.1 and triggering receptor expressed on myeloid cells 2 (TREM2) on chromosome 6p21.1. Until now, no case related to TYROBP gene mutation has been reported in the Chinese population. We report a 39-year-old Chinese Han patient who presented with early-onset dementia and cystic bone lesions. Brain magnetic resonance imaging showed extensive cerebral atrophy with v

Identification of cannabichromevarin as a potent stabilizer of the measles virus prefusion F protein: structural insights from long-timescale molecular dynamics.

Measles virus (MeV) remains a serious public health concern, necessitating the development of effective antivirals targeting the viral fusion (F) glycoprotein. This study employed a robust computational pipeline, including molecular docking, 1000 ns all-atom molecular dynamics (MD) simulations, and free energy landscape (FEL) analysis, to evaluate minor cannabinoids as novel inhibitors of the MeV F protein. Initial virtual screening identified Cannabichromenic acid (CBCA), Cannabichromevarin (CBCV), and Cannabiripsol (CBR) as high-affinity leads, with docking scores of - 8.5, - 8.2, and - 8.1
