INTRODUCTION: JAK inhibitors (JAKi) represent a novel therapeutic approach for treating the immune-mediated dermatologic diseases alopecia areata (AA) and atopic dermatitis (AD). JAKi have shown a favorable safety profile in controlled clinical trials; however, real-world safety data in routine dermatologic practice remain limited. The objective of this study is to evaluate the safety of JAKis (abrocitinib, baricitinib, upadacitinib, and ritlecitinib) in patients with AA or AD in a real-world setting.
METHODS: We conducted a single-center retrospective observational study based on electronic

BACKGROUND: Diffuse unpatterned alopecia (DUPA) is an uncommon and incompletely characterized variant of androgenetic alopecia (AGA) marked by diffuse follicular miniaturization across the entire scalp, including the occipital region, which distinguishes it from classic patterned AGA and often precludes hair transplantation.
SUMMARY: In this review of the available literature on DUPA, we discuss its clinical presentation, diagnostic features, competing pathophysiologic hypotheses, and management considerations. Current evidence suggests DUPA typically presents with diffuse thinning across all

IMPORTANCE: Alopecia areata (AA) is a chronic, systemic immune-mediated disease characterized by nonscarring hair loss. Many patients do not experience improvements with available treatment options and only ritlecitinib is approved for adolescent patients; therefore, there remains a clear unmet need for additional and improved systemic therapies.
OBJECTIVE: To evaluate the efficacy and safety of upadacitinib-an oral selective Janus kinase inhibitor-in adult and adolescent patients with severe AA.
DESIGN, SETTING, AND PARTICIPANTS: This global clinical program included 2 parallel phase 3 repl

BACKGROUND: Alopecia areata (AA) is an autoimmune disease that can affect the quality of life of affected individuals. Publicly available online conversations and search behavior provide valuable insights into public awareness, understanding, and information related to AA. An infodemiology approach, therefore, offers an opportunity to characterize these perspectives across Gulf countries. This study evaluated the understanding and awareness of AA in Bahrain, Kuwait, Oman, Qatar, and the United Arab Emirates based on AA-related mentions and internet searches.
OBJECTIVE: Despite its prevalence,

Alopecia areata (AA) is a chronic immune-mediated disease characterized by non-scarring hair loss and a highly heterogeneous clinical course. Historically managed with nonspecific immunosuppressive therapies, AA has undergone a major therapeutic transformation driven by advances in the understanding of its immunopathogenesis, particularly the central role of interferon-γ, interleukin-15, and other cytokines that signal through the Janus kinase-signal transducer and activator of transcription (JAK-STAT) pathway as well as the development of JAK inhibitors. Targeted JAK inhibitors have emerged a

Alopecia areata (AA) is a relapsing autoimmune disease characterized by non-scarring loss of hair following the collapse of immune privilege of the anagen hair follicles. This review summarizes recent molecular findings linking follicular stress to an atypical cytotoxic response, with special emphasis on the role of CD8 + NKG2D+ T cells, stress-induced ligands (MICA/ULBPs), and a Th1 cytokine circuit triggered by IFN-γ and IL-15, which communicates via the JAK/STAT pathway. We present new evidence that oxidative stress and mitochondrial dysfunction can enhance immune activation and may follow

The physiological effects of glucagon-like peptide-1 receptor agonists (GLP-1RAs) and dual glucose-dependent insulinotropic polypeptide (GIP)/GLP-1RAs extend beyond glucose metabolism and body weight regulation. Accumulating clinical evidence has highlighted an association between these incretin-based therapies and hair loss; however, the underlying molecular mechanisms are not fully understood. White adipose tissue (WAT) is increasingly recognized as a critical regulator of hair follicle homeostasis that may undergo substantial drug-associated functional alterations. Thus, GLP-1RAs and GIP/GL

Background and objective Alopecia encompasses a heterogeneous group of scarring and non-scarring disorders that often have overlapping clinical features, complicating diagnosis when scalp biopsy is unavailable. The real-world diagnostic yield of trichoscopy in rural Indian populations remains incompletely characterised. The objective of this study is to evaluate the diagnostic yield of trichoscopy, quantify its capacity to alter or refine provisional clinical diagnoses, and assess its role in reducing invasive investigations. Methods A hospital-based cross-sectional study enrolled 200 consecut

OBJECTIVE(S): GLP-1 receptor agonists (GLP-1s) are widely prescribed for type 2 diabetes (T2DM) and obesity, but associated hair loss outcomes remain poorly characterized. We evaluated whether GLP-1s were associated with different subtypes of hair loss in adults with T2DM or obesity.
METHODS: This retrospective TriNetX cohort study included adults with established care from 2018 to 2025. GLP-1 users were grouped by indication and propensity score matched to nonusers by demographics, comorbidities, and hair loss-associated medications. Outcomes were new diagnoses of nonscarring alopecia, telog

Hair loss (alopecia) is a common presentation in both primary care and dermatology. It often signals underlying inflammation or abnormalities in hair cycling or hair shaft structure, requiring a thorough and systematic evaluation. Clinical guidelines emphasize pattern recognition, trigger identification, and targeted interventions. This article equips physician associates (PAs) with an evidence-based approach to assessing patient history, conducting physical examinations, selecting appropriate diagnostics, and implementing treatment strategies for alopecia. Alopecia areata (AA) is a highly prevalent human autoimmune disease. It is now regarded by most scientists as a defect of immune privilege (IP). The impaired IP in the hair follicles is an essential driver of AA, which is developed by the imbalanced local CD8+ T cell, NK cell, tissue-resident Treg cells and infiltration of many factors such as IFN-γ and TGF-β. In the past, IL-2 and IL-15 are generally recognized for their roles in promoting pro-inflammatory immune responses. Moreover, cytokine-based therapeutic strategies utilizing IL-2 or IL-15 have the capacity to expand effector T cell and

INTRODUCTION: Alopecia areata (AA) is a chronic immune-mediated disorder associated with substantial psychosocial burden. Janus kinase (JAK) inhibitors have transformed the treatment of moderate-to-severe AA, but concerns regarding their long-term safety remain, particularly because class warnings are largely derived from higher-risk rheumatologic populations.
AREAS COVERED: This review evaluates the safety of JAK inhibitors used for AA in adults, with emphasis on baricitinib, ritlecitinib, and deuruxolitinib, alongside relevant evidence for other JAK inhibitors. PubMed/MEDLINE, Embase, and S

Alopecia areata (AA) is a chronic, immune-mediated disease characterized by nonscarring hair loss and a substantial clinical and psychosocial burden, yet it is still widely perceived as a purely cosmetic concern. In this viewpoint, we draw on the available Malaysian studies, selected international evidence, and our collective clinical experience to propose that AA warrants greater clinical, research, and policy attention in Malaysia. Malaysia-specific evidence remains limited and is derived largely from retrospective, single-center studies; nonetheless, the available data point to variable dis

Alopecia areata (AA) is associated with substantial psychosocial morbidity. Population-based studies have identified increased suicide-related risks, although findings are heterogeneous, which raises an ethical question regarding dermatologists' obligation to screen for psychiatric morbidity. Routine screening for depression and psychologic distress may reasonably form part of AA care. In contrast, formal suicide risk assessment should follow identification of suicidal ideation or other concerning clinical findings rather than be universally administered. This proportionate approach balances b

Alopecia universalis (AU) is a severe autoimmune disorder characterized by complete loss of scalp and body hair. The introduction of Janus kinase (JAK) inhibitors has expanded therapeutic options for patients with severe disease; however, treatment responses remain variable, particularly among individuals with extensive and longstanding hair loss. Reporting cases with atypical or limited responses to therapy remains important for understanding the spectrum of clinical outcomes observed in practice. We describe a 30-year-old man with a 23-year history of AU who demonstrated minimal response to

Alopecia areata (AA) is a common autoimmune condition that affects approximately 2% of the global population. Although it most commonly presents with the abrupt onset of well-circumscribed, non-scarring alopecic patches on the scalp, the nails and other hair-bearing areas are also frequently affected. Clinical evaluation should document disease extent and employ trichoscopy to identify characteristic signs of the condition (e.g., black-dot hairs, exclamation point hairs). The Severity of Alopecia Tool (SALT) is standard for quantifying severity, and newer instruments like the Alopecia Areata S
