Understanding Alopecia Areata: An Integrative Review of Causes, Mechanisms, and Management Strategies.

INTRODUCTION: Alopecia areata is a non-scarring autoimmune disease that is usually characterized by patchy or diffuse hair loss. The collapse of the immunological privilege of hair follicles and activation of cytotoxic T-cell pathways, regulated by metabolic, psychological, and environmental variables, cause this disorder. Advances in the fields of immunology and regenerative medicine have identified new therapeutic targets and novel approaches. METHODS: Mechanistic studies, clinical trials, peer-reviewed articles, and epidemiological data have been combined into this review. A literature rev

The role of inflammatory cell death in type 1 dominant inflammatory skin diseases.

While there has been significant progress in the development of new therapies for psoriasis and atopic dermatitis in recent years, innovation in the field of type 1 dominant skin diseases is still limited. This category comprises diseases characterized by a cytotoxic immune reaction directed against resident skin cells. The histological correlate is interface dermatitis, defined by a subepidermal inflammatory infiltrate associated with epidermal keratinocyte apoptosis. Representative conditions include lichen planus, cutaneous lupus erythematosus, erythema multiforme, alopecia areata, and viti

Upadacitinib is effective in treating psoriasis combined with lichen planus: a case report.

Psoriasis frequently coexists with other immune-mediated conditions such as arthritis, alopecia areata, vitiligo, and hidradenitis suppurativa, many of which remain challenging to manage. To date, there have been rarely reported cases of treating psoriasis combined with lichen planus. We report a case of coexisting psoriasis and lichen planus in a female patient. Despite initial efficacy with acitretin and topical mometasone, the patient self-discontinued treatment over safety concerns, prompting a relapse. Upadacitinib was subsequently initiated and yielded marked improvement within 3 months.

Leukotrichia: A comprehensive review of follicular depigmentation.

Leukotrichia refers to whitening or depigmentation of hair resulting from reduced or absent melanin in the hair shaft. Although often considered a cosmetic finding, it may reflect underlying alterations in follicular melanocyte biology and therefore have diagnostic, prognostic, and therapeutic relevance. The clinical spectrum includes congenital and genetic disorders, including piebaldism, Waardenburg syndrome, tuberous sclerosis complex, and pigment dilution syndromes; autoimmune and inflammatory conditions, including vitiligo and follicular vitiligo, alopecia areata, and Vogt-Koyanagi-Harada

Deucravacitinib in dermatology: current indication and existing off-label data.

Tyrosine kinase 2 (TYK2) inhibition has recently emerged as a novel therapeutic strategy in dermatology, targeting key cytokine pathways involved in immune-mediated inflammatory diseases. Deucravacitinib, an oral, selective TYK2 inhibitor that binds the regulatory pseudokinase domain, modulates signalling mediated by IL-12, IL-23 and type I interferons whilst sparing the broader JAK family, thereby providing targeted immunomodulation with a potentially improved safety profile compared with conventional JAK inhibitors. Pivotal randomized clinical trials have demonstrated robust efficacy and sus

Comorbidities of atopic dermatitis: Emerging evidence and clinical considerations.

Atopic dermatitis (AD) is the most common chronic inflammatory skin disease and has traditionally been viewed as a disorder confined to the skin. However, emerging epidemiologic, genetic, and mechanistic evidence increasingly supports the concept that AD represents a systemic disease associated with a broad spectrum of immune-related comorbidities. This review synthesizes current literature on the comorbidities associated with AD, including dermatologic, respiratory, gastrointestinal, rheumatologic, neurologic, ophthalmologic, and endocrine disorders. Well-established associations include othe

Efficacy of Dupilumab in Treating Trachyonychia and Other Nail Dystrophies: A Systematic Review of Case Reports.

Trachyonychia and related nail dystrophies are often associated with inflammatory skin diseases such as atopic dermatitis, psoriasis, and alopecia areata. Management remains difficult, as standard therapies frequently yield limited benefit. Dupilumab, an IL-4Rα antagonist, is effective for Th2-driven conditions, but its role in nail dystrophies has not been systematically assessed. This review evaluates the existing evidence on dupilumab for trachyonychia and related nail disorders. A systematic search of PubMed, Wiley, Web of Science, MEDLINE, Google Scholar, and EBSCO was performed from ince

Epithelial Barrier Diseases Among Adult Patients With Seborrheic Dermatitis.

IMPORTANCE: The epithelial barrier theory (EBT) proposes that epithelial barrier disruption is implicated in the development of skin, respiratory, gastrointestinal, and ocular diseases (epithelial barrier diseases, or EBDs). There is a need to better understand the relationship between seborrheic dermatitis and EBDs, and we hypothesize that seborrheic dermatitis, characterized by epithelial barrier dysfunction, is associated with increased frequency of other EBDs. OBJECTIVE: To explore the association between seborrheic dermatitis and EBDs. DESIGN, SETTING, AND PARTICIPANTS: This retrospecti

Common Skin Diseases and Metabolic Syndrome: A Proinflammatory Chemokine Perspective.

Skin diseases frequently coexist with other disorders, such as metabolic syndrome, diabetes mellitus, depression, psoriatic arthritis, and cardiovascular disease. Altered levels of distinct chemokines, like CCL5/RANTES, CXCL12/SDF-1a, CCL7/MCP-3, CCL2/MCP-1, CXCL1/GROa, and the eotaxin family, contribute to the development and/or exacerbation of inflammation, which is a common feature of numerous skin diseases as well as metabolic syndrome. The pathological and molecular connections between chronic inflammatory skin diseases and metabolic syndrome are increasingly recognized as being driven by

Ease of Use of the Lebrikizumab Prefilled Pen for Individuals with Atopic Dermatitis.

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disease that can require long-term systemic therapy in individuals with moderate-to-severe disease. Lebrikizumab is a monoclonal antibody that targets interleukin 13, an important cytokine involved in AD pathogenesis. A single-use, prefilled pen has been developed to enable self-injection, and its usability is the focus of this study. Because ease of use and user confidence are considered important factors in self‑injection experiences, evaluating the usability of injection devices is an important component of patient-centered c

Atopic dermatitis.

Atopic dermatitis (AD) is the most common inflammatory skin disease and carries the highest disability-adjusted life-years burden, ranking 15th among all non-fatal diseases globally. It is characterized by intensely itchy skin and is associated with multiple comorbidities, such as food allergy, asthma, allergic rhinitis and eosinophilic oesophagitis, which are mainly driven by type 2 immune responses. Other comorbidities include mental health disorders, disordered bone health, and cutaneous and extracutaneous infections. AD is also associated with other immune-mediated inflammatory diseases, i

The Association Between Alopecia Areata and Iron Deficiency Anemia: A Large-Scale Population-Based Case-Control Study.

Background: Alopecia areata (AA) is a chronic autoimmune disorder characterized by non-scarring hair loss and commonly coexists with other autoimmune diseases, suggesting systemic immune dysregulation. An association between AA and iron deficiency anemia (IDA) has been proposed, though prior studies have yielded conflicting results. Objective: Our aim was to examine the association between AA and IDA in a large, population-based cohort. Methods: We conducted a retrospective case-control study using data from a health maintenance organization (HMO) serving approximately 2.35 million individuals

Risk of Safety Events in Vitiligo Patients: A Retrospective Real-World Data Study in the US.

Vitiligo is a chronic autoimmune depigmenting disease characterized by loss of pigment in the skin, hair, or both. As treatment options evolve, particularly with the emergence of oral Janus kinase inhibitors and dual Janus kinase 3/tyrosine kinase expressed in hepatocellular carcinoma family kinase inhibitor, it is essential to assess population-level safety events in patients with vitiligo. This retrospective cohort study examined patients aged ≥ 12 years with vitiligo in the United States, comparing them with age-, sex-, and race-matched controls to evaluate patient demographics, baseline co

Clinical Spectrum of Cutaneous, Ocular, and Hair Manifestations in Patients With Inborn Errors of Immunity: Insights From a Single Center in Turkey.

BACKGROUND: Inborn errors of immunity (IEI), previously referred to as primary immunodeficiencies, are a heterogeneous group of genetic disorders affecting immune development and function. While once considered rare, IEIs are increasingly recognized, particularly in regions with high consanguinity rates. Cutaneous manifestations, as well as ocular and hair abnormalities, may provide early and clinically relevant diagnostic clues. This study aimed to assess the prevalence, types, and diagnostic value of cutaneous, ocular and hair manifestations in patients with IEI. METHODS: A total of 386 pat

Alopecia Areata in a Patient Undergoing Treatment for Secondary Syphilis: A Diagnostic Challenge.

A 17-year-old male with a recent history of incarceration and a diagnosis of secondary syphilis presented for follow-up three months after receiving appropriate antibiotic treatment. His venereal disease research laboratory (VDRL) titers showed a favorable decline (from 1:16 to 1:4); however, he reported new-onset patchy scalp alopecia. Clinically, the lesions raised suspicion for syphilitic alopecia (SA). Nevertheless, trichoscopy revealed hallmark findings of alopecia areata (AA), including exclamation mark hairs and black dots, while lacking specific signs of SA, such as an erythematous bac
