Topical Finasteride 0.25% Solution for Female Pattern Hair Loss (FPHL) in Postmenopausal Women: A 52-Week Retrospective Observational Study.

BACKGROUND: Female pattern hair loss (FPHL), characterized by progressive follicular miniaturization, is the most common cause of hair thinning in women and increases in prevalence after menopause. While topical minoxidil is approved for FPHL, clinical response is variable. Oral finasteride is used off label in postmenopausal women but raises concerns regarding systemic adverse effects. Topical finasteride may represent an alternative with reduced systemic exposure. METHODS: A retrospective observational study was conducted in 26 postmenopausal women with FPHL who were treated with topical fi

Inflammation-fibrosis coupling in androgenetic alopecia: evidence and mechanistic hypotheses for progressive hair follicle miniaturization.

Androgenetic alopecia (AGA) is primarily explained by genetic susceptibility and androgen-dependent signaling in dermal papilla cells. This mini-review examines whether low-grade, regionally heterogeneous perifollicular inflammation interacts with extracellular matrix remodeling as a complementary component of AGA pathobiology, without implying an autoimmune mechanism. Human AGA scalp studies report upper-follicular and perifollicular CD4+ lymphocytes, mast cells, immune-enriched transcriptional states, connective tissue sheath abnormalities, and fibrosis in subsets of patients. Patient-derive

Nudix hydrolase 15 deficiency and azathioprine toxicity in a liver transplant recipient: A case report.

Thiopurines, such as azathioprine (AZA), are among the immunosuppressants used to prevent allograft rejection. Thiopurines are metabolized by thiopurine S-methyltransferase (TPMT) and nudix hydrolase 15 (NUDT15) enzymes, and their functions are highly influenced by genetic variation. We present a 6-year-old female with recurrent acute cellular rejection and steroid-induced glaucoma due to high-dose corticosteroid therapy. Consequently, AZA was incorporated into her calcineurin-inhibitor-based regimen to reduce steroid dose. Within 33 days of initiating AZA, alopecia and pancytopenia developed

Optimizing Concentrated Growth Factor Therapy: Sodium Citrate Reduces Pain in Hair Loss Treatment.

BACKGROUND: Androgenetic alopecia (AGA) is a prevalent condition causing significant psychosocial distress. While concentrated growth factor (CGF) therapy has emerged as a promising regenerative treatment, the choice of anticoagulant remains a critical but underexplored variable that may affect the growth factor release, and patient experience. OBJECTIVE: To compare the efficacy, procedural pain, and patient satisfaction of CGF prepared with sodium citrate, heparin sodium, or no anticoagulant in patients with AGA. METHODS: This phase 2, multicenter, single-blind, randomized controlled trial

GLP-1 receptor agonists and hair loss: adverse effect, epiphenomenon, or therapeutic paradox?

OBJECTIVE: Incretin-based therapies, including glucagon-like peptide-1 receptor agonists (GLP-1 RAs) and dual/triple agonists, have revolutionized the management of obesity and type 2 diabetes. However, hair loss has emerged as an adverse event in clinical practice. This review synthesizes the evidence on the prevalence, mechanistic pathways, and clinical course of hair loss associated with semaglutide, tirzepatide, and retatrutide. METHODS: A narrative review was conducted using the PubMed/MEDLINE database. Data were integrated from clinical trials, real-world observational studies, and phar

Composite microneedle-mediated co-delivery of magnesium hydride-magnolol for androgenetic alopecia.

Androgenetic alopecia (AGA) impairs quality of life, yet current pharmacotherapies are hampered by limited efficacy and adverse effects. Microneedle-assisted transdermal co-delivery of bioactive natural products and gaseous agents offers a promising alternative. Herein, we report composite polymer microneedles (MNs) for controlled co-delivery of magnesium hydride (MgH2, which generates cytoprotective H2 and Mg2+ in vivo) and magnolol (Mag, a polyphenol with anti-inflammatory and antioxidant activities). Drug-loaded particles were fabricated via mechanical ball milling and ethanol solvent metho

From Empiricism to Precision: Century of Paradigm Shifts in Medical Dermatology.

Over the past century, advances in scientific understanding and therapeutic development have fundamentally reshaped dermatology. Diseases once managed with empiric, toxic, or largely ineffective treatments can now be approached with therapies that target specific pathways, achieve sustained disease control, and, in some cases, alter prognosis. Herein, we reviewed landmark therapeutic advances across infectious, inflammatory, autoimmune, vascular, and oncologic dermatology, focusing on interventions that changed the standard of care or substantially improved patient outcomes. Transformative adv

Advances in androgenetic alopecia model research.

Androgenetic alopecia (AGA) is the most prevalent form of non-scarring hair loss, and its pathogenesis is quite complex, involving androgen metabolism, follicular microenvironment imbalance, oxidative stress, and inflammatory responses. Understanding these multifactorial mechanisms is critical for developing effective therapeutic strategies. To this end, various experimental models have been extensively employed. In this review, we summarized recent progress in AGA model research, covering in vitro cell models such as dermal papilla cells and keratinocytes, in vivo animal models-primarily mice

Minoxidil and Prostaglandin Analogs for Chemotherapy-Induced Alopecia: A Scoping Review.

BACKGROUND: Chemotherapy-induced alopecia (CIA) is common, distressing, and markedly impairs patients' quality of life. Although scalp cooling is the only approved preventive strategy, its effectiveness and accessibility remain limited. Therefore, alternative pharmacologic approaches, such as minoxidil and prostaglandin analogs, have been explored. METHODS: This scoping review aimed to explore the available evidence on minoxidil's efficacy and safety and the potential role of prostaglandin analogs in alopecia. PubMed, Cochrane Library, Medical Online, and Ichushi-Web were comprehensively sear

Limited-Dissection Ligament-Preserving Facelift for Early Facial Aging: Surgical Technique and Clinical Experience.

BACKGROUND: Early facial aging is characterized by midfacial descent, loss of mandibular definition, and attenuation of retaining ligaments. Although superficial musculoaponeurotic system (SMAS)-based and deep-plane facelifts remain the gold standard, many patients seek less invasive procedures with shorter recovery. OBJECTIVES: The aim of this study was to describe a limited-dissection ligament-preserving facelift technique and evaluate its safety and clinical outcomes in patients with early facial aging. METHODS: A retrospective case series of 100 patients treated between June 2021 and Jan

Exploratory Identification of Multidimensional Treatment-Response Profiles in Androgenetic Alopecia Treated with Combined Scalp Microneedling and Topical Minoxidil Using Unsupervised Clustering.

BACKGROUND: Combination therapy with scalp microneedling and topical minoxidil is more effective than minoxidil alone in androgenetic alopecia (AGA), yet treatment responses vary considerably among patients and the multidimensional structure of this heterogeneity remains poorly characterized. OBJECTIVE: To explore whether unsupervised clustering identifies distinct multidimensional treatment-response profiles during combination therapy and to describe their clinical and molecular characteristics as a hypothesis-generating basis for personalized management. METHODS: In this prospective cohort

Hair transplantation in scar reconstruction.

BACKGROUND: Scar alopecia following burns, trauma, and congenital conditions present a significant psychosocial burden. Conventional reconstructive approaches, including tissue expansion and scar excisions, can restore hair-bearing skin but are frequently limited by geometric constraints, residual scars, prolonged treatment timelines and an inability to recreate natural hairline transition zones. Hair transplantation is widely recognised within aesthetic practice, however its role as a reconstructive modality for post-traumatic scars may be underappreciated by clinicians less familiar with hai
