Drug-induced hair loss and hair growth. Incidence, management and avoidance.

A large number of drugs may interfere with the hair cycle and produce hair loss. Drugs may affect anagen follicles through 2 main different modalities: (i) by inducing an abrupt cessation of mitotic activity in rapidly dividing hair matrix cells (anagen effluvium) or (ii) by precipitating the follicles into premature rest (telogen effluvium). In anagen effluvium, hair loss usually occurs within days to weeks of drug administration, whereas in telogen effluvium, hair loss becomes evident 2 to 4 months after starting treatment. Anagen effluvium is a prominent adverse effect of antineoplastic age

Developmental toxicity of inhaled trans-1,2-dichloroethylene in the rat.

The developmental toxicity of trans-1,2-dichloroethylene (t-DCE), a component of certain Freon cleaning agents, was examined in pregnant rats. t-DCE was administered by inhalation 6 hr daily on Days 7-16 of gestation (the day copulation was confirmed was termed Day 1 of gestation) at exposure levels of 0, 2000, 6000, or 12,000 ppm. The offspring were then examined on Day 22 of gestation. Overt maternal toxicity was expressed as a significant reduction in weight gain at 12,000 ppm and in feed consumption at 6000 and 12,000 ppm. During the exposure period, lacrimation and stained periocular hair

Investigating drug induced alopecia.

The propensity of cytotoxic drugs to cause alopecia is well known, but nurses are perhaps less aware of the capacity of other drug groups to cause this unwanted side-effect. Increasing nurses' awareness of the actions of these drugs will enable the association between alopecia and toxicity to be recognised at an early stage, before the distressing effects of drug-induced alopecia develop.

[Teratogenic effects in a population based on a model of the bone system of the human embryo].

The dynamics of teratogenic effects was studied in population of town of Chernovtsy (Ukraine) on the model of the human embryo bone system. Visual and morphometric examinations of native intact bone preparations (including the ossification zones) of artificially aborted normal human embryos of 6-12 gestational weeks were made by the original method suggested by the author. The first stage of the study was performed in 1988 during acute total alopecia of children. The second stage was three and four years after the event. The obtained results have supported the evidence for a comparative rise i

Expression of the L-fucose moiety on infrainfundibular follicular keratinocytes of terminal follicles, its decreased expression on vellus and indeterminate follicles of androgenetic alopecia, and re-expression in drug-induced hair regrowth.

The distribution of various glycoprotein molecules on the surface of follicular keratinocytes was studied with a panel of lectins with specificity for various sugar moieties on biopsy specimens from both bald/balding scalp and normal occipital scalp, of 23 patients with androgenetic alopecia as well as on biopsies of normal forearm skin of four patients. The most significant differences between bald and normal scalp biopsy were noted with Ulex europaeus agglutinin I (UEA I). We noted an increased (91.8% +/- 3.1; mean +/- SE) expression of UEA I binding sites on the infra-infundibular follicula

Mitoxantrone, cytosine-arabinoside and 6-thioguanine (MAT) in the treatment of newly diagnosed acute non-lymphoblastic leukemia in adults.

Between January 1986 and September 1989, 28 patients over the age of 14 years were treated with a combination of mitoxantrone, cytosine-arabinoside (Ara-C) and 6-thioguanine (MAT) at the Kuwait Cancer Control Centre (KCCC). All patients were newly diagnosed cases of acute non-lymphoblastic leukemia (ANLL). Fifty-eight courses of treatment were given as induction and consolidation therapy. The main toxicity was bone marrow suppression. Other toxicities were mainly nausea and vomiting, hepatotoxicity, renal dysfunction and alopecia. In most patients these were mild and tolerable. Complete remiss

Alopecia areata. A review of therapy, efficacy, safety, and mechanism.

Although it is clear from the foregoing that some of the drugs and chemicals used to treat severe alopecia areata are efficacious to some degree, it is impossible to draw any meaningful comparisons among the data outlined in Tables 1 and 2. Virtually all of the studies were designed differently. Differences in chronicity and extent of disease as well as history of previous treatment resistance may significantly affect efficacy data even as two investigators compare the same drug. Drug-induced hair regrowth in alopecia areata may be very slow; a cosmetic response may take 1 to 2 years to achiev

Psychotropic medication and drug-related alopecia.

The literature on alopecia as a side effect of psychotropic medications is reviewed. Drug-induced alopecia usually presents as a diffuse, nonscarring alopecia that is reversible upon withdrawal of the drug. Certain psychotropic drugs--such as the beta-blockers, lithium, and anticonvulsants--are most likely to induce a drug-related alopecia. The evaluation and management of psychiatric patients with drug-induced alopecia are discussed.

[Ichthyosis and alopecia after maprotiline: corneolysis caused by temporary disorder of keratinization].

A 37-year-old woman developed ichthyosiform desquamation of the skin and a severe diffuse alopoecia 3 weeks after taking the antidepressant maprotilin. No signs of inflammation were present. Histology revealed acanthosis with preserved stratum granulosum, follicular hyperkeratosis and dystrophic changes of the hair follicle. Electron microscopy revealed rarefication of tonofilaments and necrobiotic changes of epidermal keratinocytes with vacuolar degeneration of the cytoplasm and disorganization of the organelles. Pathogenetically this disease represents a drug-induced transitory disorder to k

Cyclophosphamide toxicity. Characterising and avoiding the problem.

Cyclophosphamide, an orally active alkylating agent, is widely used to treat a variety of malignant and nonmalignant disorders. Although it has some tumour selectivity, it also possesses a wide spectrum of toxicities. The requirement of metabolic activation before cyclophosphamide exerts either its therapeutic or toxic effects is well established, but has not led to effective counter-measures. Clinically, damage to the bladder (haemorrhagic cystitis), immunosuppression (when not desired) and alopecia are the most significant toxicities associated with cyclophosphamide. Cardiotoxicity is also a

HMG-CoA reductase inhibitor-induced myopathy in the rat: cyclosporine A interaction and mechanism studies.

Recent clinical evidence indicates a potential for skeletal muscle toxicity after therapy with HMG-CoA reductase inhibitors (HMGRIs) in man. Although the incidence of drug-induced skeletal muscle toxicity is very low (0.1-0.2%) with monotherapy, it may increase following concomitant drug therapy with the immunosuppressant, cyclosporine A (CsA), and possibly with certain other hypolipidemic agents. In the Sprague-Dawley rat, very high, pharmacologically comparable dosages (150-1200 mg/kg/day) of structurally similar HMGRIs (lovastatin, simvastatin, pravastatin and L-647, 318) produced dose-rela

[Preventive and therapeutic measures in cytostatic-associated toxicity].

Strategies (both prophylactic and therapeutic) to reduce side effects of chemotherapy are reviewed. In particular, issues concerning proper patient information are discussed which should help to increase patient compliance. Chemotherapy, especially regimens with a curative intent, must normally be administered under the guidance of an oncologist. However, close cooperation with the general practitioner is essential in order to ensure high quality counselling and treatment of the outpatient.

Changes in hair color.

Hair color changes result not only from alterations of melanin production but also from changes in the hair structure itself, altering its optical properties. A variety of genetic, metabolic, nutritional, and acquired disorders result in hair color changes. When the underlying defect can be corrected, hair color usually returns to normal. The flag sign can occur as a result of nutritional insults or due to medications. Most drug-induced changes in hair color result in lighter hair color, although PABA and some chemotherapy regimens have darkened hair. Green hair due to exogenous copper may be

Phase I--preliminary phase II trial of iproplatin, a cisplatin analogue.

Iproplatin was administered intravenously over 30 min daily for 5 consecutive days every 3 weeks to 80 evaluable patients with a variety of refractory solid tumor malignancies. Thrombocytopenia was the dose-limiting toxicity. Reversible drug-induced renal dysfunction was observed in 3 patients. One patient sustained mild ototoxicity but neurotoxicity was not encountered. Transient neutropenia, anemia, nausea, vomiting, diarrhea, elevations of liver enzymes, alopecia, and skin rash also occurred. The spectrum and severity of toxicity of iproplatin were found to differ from those of cisplatin. T

Doxorubicin-induced hair loss in the Angora rabbit: a study of treatments to protect against the hair loss.

An animal model for anticancer drug-induced hair loss has been developed using the Angora rabbit given i.v. doxorubicin, 2 mg/kg, twice weekly for 3 weeks. There was a 167% increase in the weight of hair collected by grooming between weeks 2 and 5, and a 72% inhibition of new hair growth at week 6 compared with non-treated animals. The hairs that grew in the doxorubicin treated rabbits did so at the same rate as in non-treated rabbits and appeared normal by light microscopy. Topical application of dimethylsulfoxide (DMSO), of 10% alpha-tocopherol in DMSO, of 0.5% naphthazoline hydrochloride in

The biology of hair.

Hair is a product of small pits in the skin known as hair follicles. The most important feature of hair follicles is that their activity is intermittent; each active phase or anagen is succeeded by a transitional phase (catagen) and a resting phase (telogen), during which the fully formed "club hair" is retained for a period and then shed. The growth of facial, body, axillary, and pubic hair depends on androgens. Facial hair and body hair require high levels of testosterone and its conversion to 5-alpha-dihydrotestosterone. Pubic and axillary hair follicles require much lower levels of hormone
