JAK Inhibitors for Alopecia Areata: Approved Therapies, Efficacy, and Unanswered Questions.

Alopecia areata (AA) is a common autoimmune disorder characterized by non-scarring hair loss. While traditionally viewed as a T-cell-mediated disease, recent advances have elucidated the central role of the Janus kinase/signal transducer and activator of transcription (JAK/STAT) signaling pathway in its pathogenesis, establishing a self-sustaining inflammatory loop. The identification of this pathway has transformed AA from a condition with limited therapeutic options to one with targeted biological interventions. Over the past five years, multiple JAK inhibitors have received regulatory appro

Baricitinib Efficacy Through 4 Years in Responders and Safety up to 5 Years in Adults with Severe Alopecia Areata: Final Results from BRAVE-AA1 and BRAVE-AA2.

INTRODUCTION: Long-term efficacy and safety data are imperative for therapies used to treat severe alopecia areata (AA) because the disease is chronic, loss of response can be devastating to patients, and the safety profile needs to support long-term therapy. Baricitinib, a selective JAK inhibitor, has been approved for severe AA in adults and is being studied in pediatric patients. This study evaluates the final long-term efficacy and safety of baricitinib in adults with severe AA. METHODS: Efficacy was assessed for patients randomized to baricitinib 4 mg or 2 mg who achieved response (Sever

Monitoring the Response to JAK Inhibitor Treatment in Alopecia Areata Using Reflectance Confocal Microscopy and Videotrichoscopy: A 60-Week Observational Study.

INTRODUCTION: Alopecia areata (AA) is an autoimmune disease affecting approximately 2.1% of the population. JAK inhibitors represent a therapeutic breakthrough for severe AA, yet objective tools for monitoring treatment response remain limited. The objective of this study was to correlate trichoscopic and reflectance confocal microscopy (RCM) features with clinical changes in patients with severe AA treated with JAK inhibitors (upadacitinib and baricitinib) over 60 weeks. METHODS: Twenty patients with severe AA were enrolled. Patients were treated with upadacitinib 15 mg/day or baricitinib 4

Add-on Anifrolumab for Residual Mucocutaneous and Systemic Manifestations During Cyclophosphamide Therapy in Severe Neuropsychiatric Lupus: A Case Report.

A 29-year-old woman presented with fever, malar rash, and alopecia. Laboratory evaluation demonstrated a high-titre antinuclear antibody of 1:5120, hypocomplementaemia, and positivity for anti-double-stranded DNA and anti-Smith antibodies, leading to a diagnosis of systemic lupus erythematosus. During the clinical course, she developed progressive peripheral neuropathy with inability to walk. Intravenous methylprednisolone pulse therapy and intravenous immunoglobulin were administered, but neurological improvement was limited, and intravenous cyclophosphamide therapy was initiated. Identifying and addressing obstacles to optimal alopecia areata and atopic dermatitis care in the UK.

Alopecia areata (AA) and atopic dermatitis (AD) are both chronic inflammatory conditions, associated with physical and significant psychosocial burden, with patients facing considerable barriers to care in the UK. In this perspective, we explore gaps in care in the UK healthcare system for patients with AA and AD, gaining insights from healthcare professionals, patients and patient advocates to identify opportunities for improvement within primary care and secondary care.

Hair follicle signalling networks: From molecular crosstalk to precision regenerative strategies.

Hair follicles (HFs) are dynamic mini-organs that undergo cyclical phases of growth, regression, and rest, regulated by interconnected signalling pathways including Wnt/β-catenin, Hedgehog (HH), TGF-β/BMP, Notch, PI3K/AKT, androgen receptor (AR), and prostaglandin signalling. Although these pathways are often studied individually, increasing evidence indicates that their reciprocal interactions coordinate HF stem cell activation, lineage specification, dermal papilla (DP) function, and hair-cycle progression. Disruption of this signalling network can impair follicular homeostasis and contribut

Case Reports of Alopecia Areata Associated With Apical Periodontitis.

Apical periodontitis is an inflammatory response of periradicular tissues caused by microbial infection within the root canal system. Alopecia areata (AA) is a T-cell mediated, autoimmune condition affecting hair follicles causing nonscarring hair loss, clinically characterised by small patches of baldness on the scalp and/or around the body. Although there have been links to genetic, environmental and hormonal triggers, this condition has rarely been associated with oral infection. This report presents three cases of AA with no obvious aetiology in fit and healthy patients, whereby the alopec

Efficacy and safety of Cinainu in paediatric alopecia areata: an international, double-blind, randomized, placebo-controlled, phase II/III trial.

BACKGROUND: Alopecia areata (AA) is a debilitating autoimmune-mediated disorder marked by nonscarring hair loss. It significantly impairs patients' quality of life (QoL). Inflammation and immune dysregulation, especially involving chemokines like interleukin-8 and Janus kinase (JAK)-dependent signalling pathways, underlie AA pathogenesis. As oral JAK inhibitors are approved for severe cases only, unmet needs persist, particularly for paediatric patients and those with moderate AA. Cinainu, a topical solution containing four botanical extracts, may address these gaps due to its anti-inflammator

Amygdalin Regulates FOXO3a Signaling to Inhibit Oxidative Stress and Improve Alopecia Areata in Mice.

This paper aims to explore the mechanism of amygdalin's (AMG) therapeutic effect on alopecia areata (AA). Hematoxylin-eosin staining was used to probe AMG's hepatorenal toxicity. After establishing the AA model of C3H/HeJ mice and H2O2-damaged human primary epidermal melanocytes (HPEMs), and following AMG intervention, the reactive oxygen species (ROS) content and the levels of forkhead box O3A (FOXO3a) protein and gene were monitored. Biochemical kits or ‌enzyme-linked immunosorbent assay‌ were used to detect the contents of superoxide dismutase (SOD), catalase (CAT), glutathione peroxidase 4

A two-hit mechanism triggers autoimmune hair loss.

Autoimmune diseases arise from a breakdown of immune tolerance, yet the mechanisms that initiate tissue-specific autoimmunity remain poorly understood1-4. Here we identify a two-hit mechanism in which immune dysregulation and physiological neural activation converge to trigger autoimmune alopecia. We show that transient depletion of regulatory T (Treg) cells during the anagen phase of the hair cycle, but not during telogen, induces CD8+ T-cell-mediated destruction of immune-privileged hair follicle stem cells (HFSCs), resulting in hair loss. This anagen-restricted susceptibility depends on sym

Dermatologic adverse events associated with Sacituzumab Govitecan: an international retrospective study from the EADV Task Force "Dermatology for Cancer Patients" and pharmacovigilance analysis.

PURPOSE: Sacituzumab govitecan (SG) is an antibody-drug conjugate approved across multiple metastatic breast cancer settings. Although hematologic and gastrointestinal toxicities dominate its safety profile, dermatologic adverse events (dAEs) remain poorly characterized, despite the physiological expression of Trop-2 in epidermal keratinocytes. We aimed to comprehensively characterize the spectrum, severity, time to onset, and oncologic impact of SG-associated dAEs across real-world and pharmacovigilance data sources. METHODS: An international retrospective cohort study was conducted within t
