Acne keloidalis nuchae and tufted hair folliculitis.

Acne keloidalis nuchae is a chronic, scarring folliculitis that affects mostly black patients and is located on the back of the neck of young adults. The course is progressive and leads to hypertrophic scarring, chronic abscesses and hair loss. We discuss the relationship between acne keloidalis and tufted hair folliculitis, pointing out the possibility that tufted hair folliculitis is not a specific disease but secondary to other progressive folliculitis like folliculitis decalvans, dissecting cellulitis or acne keloidalis.

[Tufted hair folliculitis].

A case of tufted hair folliculutis presenting as circumscribed, tender and inflamed areas in the occiput with residual tufted follicles in a 28-year old man is reported. Tufted hair folliculitis is a characteristic localized scarring bacterial folliculitis of the scalp due to Staphylococcus aureus. Histopathological studies reveal perifollicular inflammation around the upper portions of the follicles sparing the hair root level. Within areas of inflammation, several follicles converge toward a common follicular duct with a widely dilated opening. Currently, tufted hair folliculitis is consider

Family with "pure" hair-nail ectodermal dysplasia.

"Pure" ectodermal dysplasias are developmental disorders affecting only tissues of ectodermal origin. Two different pure ectodermal dysplasias involving only hair and nails have been described to date. Here we describe congenital nail dystrophy and hypotrichosis associated with folliculitis decalvans in a family suggesting autosomal-dominant transmission. This report documents peculiar clinical and ultrastructural hair findings that fit poorly into previously described conditions. Thus the reported patients could represent a new type of pure ectodermal dysplasia.

Cicatricial alopecia.

Scarring alopecias are of diverse etiology and pathogenesis. They may be histologically classified as primary or secondary, depending on involvement of reticular dermis. The most important primary scarring alopecias include pseudopelade, lichen planopilaris, and diffuse scarring of the vertex in African-Americans. The most important secondary scarring alopecias include folliculitis decalvans and late-stage lupus erythematosus.

Cimetidine improves the therapeutic/toxic ratio of dapsone in patients on chronic dapsone therapy.

We have previously shown that cimetidine, given concurrently for 2 weeks to patients on chronic dapsone therapy, reduced methaemoglobinaemia by inhibiting the formation of the toxic hydroxylamine metabolite of dapsone. The aim of the present study was to examine the effect of this combination on the benefit/toxic ratio of dapsone over a longer period. Eight patients (six dermatitis herpetiformis, one linear IgA disease, one folliculitis decalvans) on long-term dapsone 50-100 mg daily, took cimetidine 1.6 g daily concurrently for 3 months. At 3-weekly intervals, a clinical assessment was made,

Folliculitis decalvans and human T cell lymphotropic virus type I-associated myelopathy/tropical spastic paraparesis.

Human T-cell lymphotropic virus type I (HTLV-I) can be associated with either adult T-cell leukemia or HTLV-I-associated myelopathy/tropical spastic paraparesis (HAM/TSP), a chronic progressive immune-mediated myelopathy. Skin manifestations such as xerosis and erythema may be associated with HAM/TSP. Infective dermatitis due to Staphylococcus aureus or beta-hemolytic Streptococcus has recently been described as a marker for HTLV-I infection and as a probable risk factor for the development of adult T-cell leukemia and lymphoma in Jamaican children. We report a case of folliculitis decalvans,

[Quinquad's folliculitis decalvans and tufted hair].

We observed a case of folliculitis decalvans involving the beard and the scalp with a tufted hair folliculitis aspect in the occipital region. The presence of two clinical presentations in the same patient has not been reported in the literature. This would suggest that the two entities could be morphological aspects of the same pathological process which might cause either atrophy with loss or the annexes or tufted hair folliculitis depending upon the depts and destructive capacity of the inflammatory process.

[Erosive pustular dermatosis of the scalp after zoster ophthalmicus and trauma].

Erosive pustular dermatosis of the scalp (EPDS), first described in 1979, is a rare, chronic, pustular condition with scarring alopecia, and nonspecific histological findings. While the initial responded to steroids is good, it can be treated successfully by oral administration of zinc sulphate. Local trauma has recently been suggested to play a role in the pathogenesis of EPDS. The differential diagnosis of EPDS includes folliculitis decalvans, sterile eosinophilic pustulosis Ofuji, pustular psoriasis vulgaris, trichophytosis, Perifolliculitis capitis abscedens et suffodiens, pemphigus vulgar

[The Brocq pseudopelade--a disease picture or disease entity].

In 1885 Brocq described a type of scarring alopecia he called pseudopelade (PPB), whose character as a separate disease entity has been denied in recent decades. Several authors now believe PPB to be a polyetiological final stage (état pseudopeladique de Degos; EPP). In all, 142 patients with scarring alopecia were analyzed in an attempt to find whether PPB is a separate entity or not. Following examination, 26 cases with lichen ruber (LR), 18 cases with lupus erythematosus chronicus discoides (DLE), 2 cases with scleroderma, 1 case with folliculitis decalvans, and 1 case with ichthyosis vulga

Folliculitis decalvans--response to rifampin.

Folliculitis decalvans is a rare follicular inflammatory disease of the scalp. It is characterized by initial perifollicular inflammatory changes followed by peripheral extension and eventual circumscribed patches of cicatricial alopecia. The disease is known for its resistance to treatment, resulting in an unfavorable prognosis. The cause of the disease is unknown, although a bacterial etiology is postulated. We report a classic case that was temporized with various antibiotics and only subsequently resolved after ten weeks of therapy with rifampin. The patient has remained free of disease fo

Pseudopelade of Brocq.

41 cases with scarring alopecia seen from 1979 to 1983 were analyzed and differentiated. After exclusion of 7 cases with lichen planopilaris, of 5 cases with discoid lupus erythematosus, of 2 cases with scleroderma, and of 1 case with folliculitis decalvans, there remained 26 cases. The clinical histological and direct immunofluorescence (DIF) findings in these patients suggest that pseudopelade of Brocq might be a distinct disease unrelated to other known types of scarring alopecia. The histopathology is characteristic, and shows the following features: little or only moderate lymphocytic inf

Cicatrising Alopecias.

An attempt was made to study the distinguishing clinical features, histopathological findings especially the elastic tissue stain and examination of the plucked hairs in 30 cases of cicatrical alopecia. Clinically, pseudopelade had irregular areas of atrophy with tufts of normal hairs arising from the bald scarred areas. Folliculitis decalvans had a history or presence of inflammatory papules and pustules with atrophy extending to the temporal areas. DLE showed features of cutaneous DLE at least in some places. A smooth, slightly violaceous atrophic patch with spinous lesions in other areas su

[Folliculitis decalvans].

The author describes the main clinical and pathological aspects of Folliculitis decalvans, rare dermatosis, probably related with Staphylococcus sensibilization of the follicle. Although in some cases Staphylococcus are present in the lesions, the treatment is disappointing. Tetracycline oral and topically, sodium sulfacetamide and fusidic acid are mentioned by Rook. Alopecia is always definitive.
