Partial Lipodystrophy Mimicking Cushing's Syndrome: Clinical and Metabolic Insights from a Multicenter Study.

CONTEXT: Partial lipodystrophies (PLD) can closely mimic the clinical presentation of Cushing's syndrome (CS). Simplified risk scores have been developed to estimate the clinical probability of CS. However, their ability to discriminate CS from overlapping conditions such as PLD remains unclear. Moreover, reduced leptin action in lipodystrophy may influence the hypothalamic-pituitary-adrenal (HPA) axis, although human data are limited. OBJECTIVES: To compare clinical and metabolic features of PLD and CS, evaluate the discriminatory ability of clinical risk scores for CS, and explore leptin-HP

Analysis of androgen modulation by natural products.

Natural products are widely promoted to increase or suppress androgen activity, but the strength and clinical relevance of the evidence vary markedly by preparation, population, endpoint, and study design. We conducted a structured critical review of human and mechanistic studies of androgen-modulating botanicals, with separate appraisal of in silico predictions, biochemical or cellular target effects, hormonal biomarkers, clinical outcomes, extract characteristics, and safety. Standardized extracts of Withania somnifera, Trigonella foenum-graecum, and Eurycoma longifolia have limited-to-moder

Elucidating the Molecular Mechanisms of Vitex negundo L. in Polycystic Ovary Syndrome via Metabolomics-Driven Network Pharmacology.

Polycystic ovary syndrome (PCOS) is a hormonal disorder marked by irregular menstrual cycles, elevated androgen levels, ovarian cysts, hirsutism, acne and other symptoms. While conventional medications such as Metformin and Spironolactone are commonly prescribed, they are often associated with undesirable side effects. As a result, there is growing interest in alternative treatments, particularly those involving medicinal plants. Vitex negundo L., a member of the Lamiaceae family, has demonstrated promising therapeutic effects against PCOS. However, its precise molecular mechanism of action re

The value of anti-Mullerian hormone for diagnosing polycystic ovary syndrome in adolescents: a case-control study based on an epidemiological survey.

BACKGROUND: Updated 2023 international evidence-based guidelines do not recommend anti-Müllerian hormone (AMH) for diagnosing adolescent polycystic ovary syndrome (PCOS, also newly termed polyendocrine metabolic ovarian syndrome [PMOS]) due to limited specificity. Physiological multifollicular ovaries are prevalent in teenagers, and existing studies show massive heterogeneity in participant age, BMI, AMH detection platforms and cut-off thresholds without unified adolescent-specific standards. This study aimed to analyze serum AMH correlations with core adolescent PCOS phenotypes and evaluate t

Age-stratified hormonal profiles in adolescent and young adult PCOS/PMOS: higher mid-adolescent AMH largely unexplained by BMI.

Polycystic ovary syndrome (PCOS) - for which a global multistakeholder consensus has recently proposed the name polyendocrine metabolic ovarian syndrome (PMOS) - shows phenotypic variation across adolescence that is poorly characterised in Turkish populations, and because puberty raises both body mass index (BMI) and reproductive hormones in parallel, age and BMI effects are potentially confounded. Within a single-centre retrospective cohort of clinician-assigned PCOS/PMOS (January 2024-February 2026), we described how hormonal, anthropometric and clinical profiles differ across age and whethe

Effects of estetrol/drospirenone vs drospirenone-only on thrombin generation in women with polycystic ovary syndrome: a randomized, double-blind, controlled trial.

BACKGROUND: Combined oral contraceptives are first-line treatment for polycystic ovary syndrome (PCOS); however, venous thromboembolism risk remains a concern. Estetrol/drospirenone (E4/DRSP) has a more favorable thrombin generation assay (TGA) profile than ethinyl estradiol-based combined oral contraceptives, but direct comparison with progestin-only pills is lacking. OBJECTIVES: This study aimed to compare the effects of E4/DRSP and DRSP-only on TGA parameters, hyperandrogenic symptoms, and vaginal bleeding patterns in women with PCOS. METHODS: In a randomized, double-blind, controlled tri

Body mass index outperforms the Rotterdam criteria in defining phenotypic heterogeneity in polycystic ovary syndrome: a multicenter study from the Italian PCOS registry (PCOSOUTCOME.net).

PURPOSE: Polycystic ovary syndrome (PCOS) is a common and highly heterogeneous endocrine-metabolic disorder. The Rotterdam criteria identify four diagnostic phenotypes; however, it remains unclear whether these phenotypes or body mass index (BMI) better predict metabolic risk. The PCOSOUTCOME.net registry collects data from multiple Italian centers and provides an opportunity to compare the effectiveness of Rotterdam phenotypes versus BMI in characterizing the clinical, metabolic, and psychological heterogeneity of women with PCOS. METHODS: We included 1,314 Caucasian women of reproductive ag

Proteolytic enzymes chymotrypsin and papain combined with Alexandrite laser hair removal versus laser hair removal alone for the treatment of facial hirsutism: a randomized comparative study.

Laser hair removal is the treatment of choice for reducing unwanted hair in hirsute patients. However, preventing hair regrowth remains a challenge. This study aimed to evaluate the combined effect of laser and enzymatic hair removal. Fifty-nine Greek women with facial hirsutism were divided into 2 groups. Group I was treated with combination of Alexandrite laser and iontophoresis of chymotrypsin and papain solutions. Group II was treated with laser alone. Outcomes included trichoscopic hair counts and patient-reported hair-free intervals (HFIs). Hair reduction from baseline was significantly

Primary Care Recognition of Rabson-Mendenhall Syndrome Despite Absence of Classical Diabetic Symptoms.

BACKGROUND Rabson-Mendenhall syndrome (RMS) is an extremely rare autosomal recessive disorder caused by pathogenic variants in the insulin receptor gene, leading to severe insulin resistance and compensatory hyperinsulinemia. Classical features include acanthosis nigricans, non-obese or underweight body habitus, hirsutism, dental abnormalities, dysmorphic features, and variable growth abnormalities. Early recognition may be difficult when the initial presentation is dominated by non-specific symptoms rather than classical metabolic complaints. CASE REPORT A 10-year-old Saudi girl presented to

Uncovering the Metabolic Origins of Polycystic Ovary Syndrome.

Polycystic ovary syndrome (PCOS), now known as polyendocrine metabolic ovarian syndrome (PMOS), though the two terms remain in concurrent use during the ongoing transition, is a common endocrine disorder affecting women of reproductive age, characterized by disrupted hormonal balance and impaired metabolic health. Insulin resistance (IR) is a central pathological feature of PCOS, contributing to a broad spectrum of metabolic, reproductive, and psychological complications. Resulting hyperinsulinemia worsens hyperandrogenism, driving acne, hirsutism, menstrual irregularities, and infertility, wh

[A novel CYP19A1 variant causing aromatase deficiency in a family: case report and literature review].

Aromatase deficiency is a rare autosomal recessive disorder of estrogen biosynthesis caused by pathogenic variants in the CYP19A1 gene, located on chromosome 15q21.2. Maternal virilization during pregnancy serves as an important diagnostic clue. Affected females may present with virilized external genitalia at birth, and later in life may develop primary amenorrhea, ovarian cysts or ovarian dysplasia, delayed epiphyseal closure, and reduced bone mineral density. Herein, a family with aromatase deficiency admitted to the Department of Endocrinology and Metabolism at the Affiliated Hospital of Q

Clinical, Metabolic and Hormonal Overlap Between Nonclassic Congenital Adrenal Hyperplasia and Polycystic Ovary Syndrome: An Exploratory Study.

Objective: Polycystic ovary syndrome (PCOS) and nonclassic congenital adrenal hyperplasia (NCAH) overlap clinically, but direct comparative data specifically addressing non-National Institutes of Health (non-NIH) Rotterdam phenotypes of polycystic ovary syndrome are limited. We compared women with polycystic ovary syndrome, of whom 66.7% were classified as having non-NIH Rotterdam phenotypes, with women with nonclassic congenital adrenal hyperplasia and healthy controls to characterize the extent of phenotypic overlap and evaluate the discriminatory performance of individual androgen measures.

Cetuximab-induced Facial Hypertrichosis in a Woman Treated for Metastatic Colon Adenocarcinoma.

The use of anti-epidermal growth factor receptor antibodies, such as cetuximab, is a standard part of the management of metastatic colorectal and head and neck cancers, as they inhibit tumor proliferation. Their side effects include cutaneous complications such as skin rash, hypertrichosis, and male pattern alopecia. We report a case of a middle-aged woman who developed hypertrichosis following six cycles of cetuximab treatment with a hirsutism-related hormone profile.

Metabolic dysfunction in women with PCOS is more closely linked to BMI than to hyperandrogenism.

INTRODUCTION: Polycystic ovary syndrome (PCOS) is frequently conceptualized as a hyperandrogenic disorder, yet cardiometabolic risk may be driven primarily by obesity and insulin resistance. We characterized glucose-insulin dynamics in women with PCOS and examined whether metabolic dysfunction was more closely associated with obesity status than with androgen-related parameters. METHODS: In this retrospective cross-sectional study, 71 women with PCOS underwent a standardized 3-hour oral glucose tolerance test (OGTT, 0-180 min) and were stratified by obesity status (BMI <30 kg/m², n=37; BMI ≥3

Adjunctive Akkermansia muciniphila postbiotic supplementation modulates metabolic and endocrine profiles in obesity-associated polyendocrine metabolic ovarian syndrome: A randomized controlled trial.

BACKGROUND & AIMS: As a flagship next-generation probiotic, Akkermansia muciniphila (AKK) has been extensively validated for its prominent therapeutic advantages against metabolic diseases. However, clinical research on AKK remains scarce owing to its exclusion from the Official Catalog of Approved Food Microbial Strains in China. METHODS: In this study, 100 women with obesity-associated polyendocrine metabolic ovarian syndrome (PMOS) receiving standardized oral contraceptive therapy were randomized to receive AKK postbiotic supplementation or placebo, and the adjunctive effects of AKK postbi

Adult psychological outcomes among women with different adolescent PCOS presentations.

INTRODUCTION: Polycystic ovary syndrome (PCOS) is a prevalent endocrine disorder, often beginning in adolescence and associated with reproductive, metabolic, and psychological complications. The present study evaluated whether adolescent patients meeting the 2023 adolescent-specific PCOS criteria (Rotterdam phenotypes A and B) exhibit poorer mental health or lower quality of life (QoL) in adulthood compared with those with Rotterdam PCOS phenotypes C and D. METHODS: A retrospective analysis was conducted among women previously hospitalized in pediatric endocrinology wards and diagnosed with P
