Mental disorders are the most frequent cause of self induced injuries (self-injury). Self-mutilation is a form of auto-aggression, which is a manifestation of psychic discomfort. Skin changes, defined as self-induced skin diseases, constitute the secondary problem, and require patients to consult dermatologists. Self-induced skin diseases include Münchhausen syndrome, acne excoriée, trichotillomania, nail biting (onychophagia), dermatitis artefacta, neurotic self-mutilation, self-mutilation when associated with hallucinations and delusions. Compulsive skin picking, 'acne excoriee', neurotic (psychogenic) excoriation, dermatotillomania, occurring in 2% dermatology patients mostly in women, is a result of excessive scratching, picking, gouging or squeezing of the skin using teeth, tweezers, nail files, pins and knives, etc. The lesions are usually found on face and also on upper limbs and upper back, areas patients can easily reach. They may occur in absence or in response to skin pathology or sensation of itching. A young female patient attended OPD with the complaints of multiple excoriated lesions over the face, arms and forearm

PURPOSE OF REVIEW: Factitial skin diseases are characterized by unusual patterns of skin lesions that do not conform to any known dermatologic condition and that are consciously or subconsciously fabricated by the patient. This review summarizes the current literature regarding the diagnosis and management of factitial dermatoses in children.
RECENT FINDINGS: Neurotic excoriations, acne excoriee and trichotillomania are the most common factitial skin diseases seen in children. Dermatitis artefacta is also seen in children, but is less common. In many cases, the development of factitial skin d

BACKGROUND: The intimate relationship of the skin and the mind illustrates the importance of a holistic approach in caring for patients with skin problems. Habits related to psychological problems may lead to skin problems such as lichen simplex chronicus and acne excoriée. Emotional stress may affect skin diseases such as atopic dermatitis, perioral dermatitis, pompholyx, and plaque psoriasis.
OBJECTIVE: This article describes the relationship between the mind and skin disease.
DISCUSSION: Skin disease can exert a profound effect on the psychosocial dimension of a patient's life. Its effect

BACKGROUND: Self-inflicted skin ulcers and scars are often observed in patients with compulsive skin picking. The term "neurotic excoriation" has been used to describe this condition and may or may not coexist with other true skin pathologies, such as acne. The condition poses a diagnostic and treatment challenge because patients often also have an undiagnosed underlying psychologic disorder.
CASE REPORTS: Two patients with numerous linear and stellate facial ulcers and hypertrophic and erythematous scars were diagnosed with acne excoriée in the setting of an obsessive-compulsive disorder lin

Hypnotic suggestion successfully alleviated the behavioral picking aspect of acne excoriée des juenes filles in a pregnant woman who had been picking at the acne lesions on her face for 15 years. Acne excoriée is a subset of psychogenic or neurotic excoriation. Conventional topical antibiotic treatment was used to treat the acne. Compared with other treatments for uncomplicated acne excoriée, hypnosis is relatively brief and cost-effective and is non-toxic in pregnancy.

Biofeedback can improve cutaneous problems that have an autonomic nervous system component. Examples include biofeedback of galvanic skin resistance (GSR) for hyperhidrosis and biofeedback of skin temperature for Raynaud's disease. Hypnosis may enhance the effects obtained by biofeedback. Cognitive-behavioral methods may resolve dysfunctional thought patterns (cognitive) or actions (behavioral) that damage the skin or interfere with dermatologic therapy. Responsive diseases include acne excoriée, atopic dermatitis, factitious cheilitis, hyperhidrosis, lichen simplex chronicus, needle phobia, n

BACKGROUND: The use of psychological therapies in dermatology is being increasingly recognized because stress and negative emotions are major factors in a wide range of dermatologic disorders. Eye movement desensitization and reprocessing (EMDR) is a relatively new psychological intervention which was first described to be effective in post-traumatic stress disorder, a condition associated with extremely stressful life situations.
OBJECTIVE: We evaluated the efficacy of EMDR in primary dermatologic disorders where psychological stress plays an important part in the pathogenesis of the disorde

Dermatotillomania (also called psychogenic excoriation, neurotic excoriation, acne excoriée, skin picking and pathological or compulsive skin picking) is characterized by excessive scratching or picking of normal skin or skin with minor surface irregularities. Until recently, there have been few therapeutic options for patients with this disorder, which causes substantial distress and functional impairment in many patients. Recent pharmacological studies have found that some selective serotonin reuptake inhibitors may be effective in the treatment of this disorder. Furthermore, phenomenologica

BACKGROUND: The self-inflicted dermatoses such as acne excorieé and neurotic excoriations are often chronic, recurring, and resistant to standard dermatologic therapies.
OBJECTIVE: We present a 28-year-old woman with longstanding acne excorieé, whose acne started at age 14 years and was followed by acne excorieé at age 16 years. The patient reported that her acne and self-excoriative behavior were exacerbated by psychological stress. The previously treatment-resistant acne excorieé responded favorably to treatment with the atypical antipsychotic agent olanzapine.
METHODS: The patient was sta

Psychogenic excoriation (also called neurotic excoriation, acne excoriée, pathological or compulsive skin picking, and dermatotillomania) is characterised by excessive scratching or picking of normal skin or skin with minor surface irregularities. It is estimated to occur in 2% of dermatology clinic patients and is associated with functional impairment, medical complications (e.g. infection) or substantial distress. Psychogenic excoriation is not yet recognised in the DSM. We propose preliminary operational criteria for its diagnosis that take into account the heterogeneity of behaviour associ

This paper provides an updated review of the use of antidepressant drugs in dermatology. Some of the psychiatric disorders that are usually comorbid with dermatological disorders and respond to antidepressants include major depressive disorder, obsessive compulsive disorder, body dysmorphic disorder, social phobia and post-traumatic stress disorder usually secondary to trauma and abuse during early life. Cutaneous symptoms may be the feature of a primary psychiatric disorder, e.g. cutaneous body image problems, dermatitis artefacta, neurotic excoriations and trichotillomania, or psychiatric sy

BACKGROUND: Hypnosis is an alternative or complementary therapy that has been used since ancient times to treat medical and dermatologic problems.
OBJECTIVE: To describe the various uses for hypnosis as an alternative or complementary therapy in dermatologic practice.
METHODS: A MEDLINE search was conducted from January 1966 through December 1998 on key words related to hypnosis and skin disorders.
RESULTS: A wide spectrum of dermatologic disorders may be improved or cured using hypnosis as an alternative or complementary therapy, including acne excoriée, alopecia areata, atopic dermatitis,

BACKGROUND: Studies of the psychosomatic correlates of self-excoriative behavior in acne have involved mainly women with acne excorieé. Little is written about the psychosomatic factors that affect men with acne. Excessive self-excoriation of acne lesions is an important clinical factor because it can prolong the course of the disease and exacerbate the deeper inflammatory process with an increase in the severity of scarring.
METHODS: Thirteen men (mean +/- SE: age: 22.2 +/- 1.4 years) with mild to moderate facial acne, whose self-excoriative behavior was not severe enough to result in acne e

Psychiatric and psychometric issues were assessed in 12 acné excoriée patients. Two patients exhibited a DSM-III-R dysthymia and 2 patients a DSM-III-R personality disorder, which were considered as associated comorbidities. Most patients reported concern for being unattractive and a compulsive urge to manipulate their skin. However, 8 out 12 patients did not fulfill criteria for any DSM-III-R disorder. In particular, we could not find evidence for the assumption of underlying obsessive-compulsive disorders or body image disorders in acné excoriée.

Childhood and adolescent obsessive-compulsive disorders are sometimes seen in dermatology practice. When present, OCD is most frequently manifested in younger patients as trichotillomania, onychotillomania, onychophagia, and acne excoriée. In addition to these disorders, obsessive hand washing, AIDS phobia, and other psychosomatic dermatoses characterized by obsessive-compulsive tendencies are seen in all age groups. It is important to define the nature of any underlying psychiatric disorder before beginning drug treatment of these patients. With the availability of new medications, most notab

Acne excoriée is a self-inflicted skin condition in which the sufferer has an urge to pick real or imagined acnieform lesions and which results in a worsening and spreading of the acne. The condition differs from most artefactual dermatoses as the patient usually spontaneously admits the self-inflicted nature of the condition. Although this condition is considered to be a neurotic manifestation of the patient, traditional psychiatric treatments have proved unsuccessful.
