The acronym "TORCH" was originally introduced in 1974 by Dr Nahmias to collectively describe the most common pathogens responsible for congenital and perinatal infections, including Toxoplasma gondii, others, rubella virus, cytomegalovirus (CMV), and herpes simplex virus. With advances in diagnostic capabilities and expanded epidemiologic surveillance, the list of recognized congenital pathogens has grown to include syphilis, varicella-zoster virus, Zika virus, HIV, parvovirus B19, and lymphocytic choriomeningitis virus. While the TORCH acronym continues to be widely used in clinical and educa

BACKGROUND: Dropped head syndrome (DHS) is a rare, disabling chin-on-chest deformity caused by severe cervical extensor weakness. The etiologic spectrum is dominated by peripheral myopathies, motor neuron disease, and myasthenia gravis; central nervous system causes are seldom considered, particularly in older adults where coexisting cervical spondylosis appears to offer a unifying structural explanation. To our knowledge, DHS as the sole presenting manifestation of brainstem encephalitis has not been previously reported.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Passive maternal-fetal transfer of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) antibodies has been demonstrated, whilst the degree of transfer depending on the trimester of infection is lacking. Due to neonates' immature immune systems, this knowledge could be of interest when investigating the degree of early-life protection against SARS-CoV-2. For perinatal infections such as Rubella and Toxoplasmosis, the timing of infection related to gestational age is crucial for the severity of maternal-fetal outcomes; hence, the trimester of SARS-CoV-2 infection could potentially be cr

Rubella is a virus that can cause severe complications, such as congenital rubella syndrome, in pregnant women and newborns. Most clinical laboratories currently use qualitative methods to detect IgM antibodies; however, these have limitations in determining disease severity and progression. Thus, a quantitative IgM assay is required to enhance clinical diagnosis and treatment. Therefore, we employed the principle of the capture method for detecting rubella virus (RV) IgM. RV IgM was captured using anti-human IgM antibodies immobilized on magnetic beads, followed by adding Tm3+-labeled RV dete

Viral infections can significantly affect the physiopathology of reproductive organs, leading to fertility problems, reducing the success rates of assisted reproductive technologies, and negatively impacting pregnancy. This review aims to summarize the current evidence on viral pathogens that are either suspected or confirmed to play a role in reproductive medicine and their effects on early pregnancy. For instance, viral hepatitis and human immunodeficiency virus can decrease sperm quality. Human papilloma virus infection in men appears to cause infertility, while herpesviruses pose a greater

The rise in neurodevelopmental disorders linked to maternal viral infections, particularly during the first and second trimesters of pregnancy, is concerning. Rubella, a contagious viral disease, primarily affects children and young adults, presenting as a rash and mild fever. It can also cause symptoms such as a swollen spleen, blueberry muffin skin spots, small head circumference, meningoencephalitis, developmental delays, and jaundice. When contracted in the first trimester, rubella can lead to severe birth defects or fetal death, with the risk declining after 20 weeks. Congenital rubella s

REASON TO REPORT: Aicardi-Goutières syndrome is a rare congenital multisystem disease of genetic origin, and its manifestations resemble those of an intrauterine infection; therefore, TORCH infection (toxoplasmosis, others, rubella, cytomegalovirus, herpes) is its main differential diagnosis. The classic brain imaging features-leukoencephalopathy, striatal necrosis, intracranial calcifications and atrophy-are well known. Recently, another radiological pattern characterized by large-vessel stenosis, including moyamoya syndrome, and ischaemic lesions, has been reported, mainly with SAMHD1 mutati

Diagnosis of cytomegalovirus (CMV) primary infection (PI) during pregnancy relies on serology (CMV-IgG, IgM, and IgG avidity). However, as for toxoplasmosis, subsequent serology testing 3-5 weeks later is often performed to confirm the diagnosis. In this study, we aimed to show that testing CMV-IgG with different assays may lead to misinterpretation of CMV-IgG kinetics and to determine the sensitivity and specificity of CMV-IgG stability and significant increase to exclude or confirm recent CMV PI. We conducted a retrospective study on (i) a CMV-IgG external quality control program (2015-2022)

Inborn errors of immunity (IEIs) are caused by deleterious variants in immune-related genes. ASXL1 is an epigenetic modifier not previously linked to an IEI. Clonal hematopoiesis and hematologic neoplasms often feature somatic ASXL1 variants, and Bohring-Opitz syndrome, a neurodevelopmental disorder, is caused by heterozygous truncating ASXL1 variants. We present an IEI caused by biallelic germline missense variants in ASXL1. The patient had a history of hematologic abnormalities and viral-associated complications, including chronic macrocytosis, persistent vaccine-strain rubella granulomas, a

BACKGROUND: Myositis, characterized by inflammation and weakness of skeletal muscles, is a multifactorial condition caused by various infectious, autoimmune, and neuromuscular disorders. Among these, infective myositis is a rare but significant clinical entity, predominantly caused by viral pathogens. Viruses such as influenza, enteroviruses, rubella, and human immunodeficiency virus are common culprits, with influenza-associated myositis being a particularly well-documented phenomenon. Despite its self-limiting nature in many cases, influenza-associated myositis warrants clinical attention ow

Transplacental infections such as cytomegalovirus, Zika virus, toxoplasmosis, and rubella, especially if acquired in the perinatal period, are well-known causes of congenital anomalies. However, their phenotypic overlap with certain genetic syndromes can pose significant diagnostic challenges during fetal autopsy and prenatal counseling. With an objective to review and highlight the clinical and pathological similarities between congenital infections and genetic syndromes, and emphasizing diagnostic pitfalls in fetal autopsy and the importance of integrated approaches, a comprehensive literatu
