Clinical progress note: Rubella.

Rates of rubella infection and congenital rubella syndrome decreased significantly since the introduction of the rubella vaccine in 1969. Endemic rubella was declared eliminated in the United States in 2004, and since 2012, all rubella cases in the United States have been associated with infections acquired abroad. With vaccine rates falling worldwide and outbreaks of vaccine preventable diseases increasing, it is important for clinicians to be prepared to recognize and manage diseases they may have never seen before, including rubella. This article reviews the clinical manifestations, complic

Prevalence of antibodies against rubella virus, cytomegalovirus, hepatitis b, and Toxoplasma gondii in women of reproductive age prior to conception in Iran.

Toxoplasma gondii (TOX), Rubella virus (RUV), Cytomegalovirus (CMV) and herpes simplex virus (HSV) are major TORCH pathogens that can cause serious congenital infections and adverse pregnancy outcomes if primary infection occurs during pregnancy. This cross-sectional study assessed the serological status of TORCH infections among 388 women of reproductive age prior to conception referred to a clinical laboratory in Tabriz, Iran, between April 2024 and April 2025. The objective was to determine the prevalence of IgG and IgM antibodies against Toxoplasma gondii, Rubella virus, CMV, and the prese

Performance of two chemiluminescent assays in determining rubella IgG in sera with low levels of specific IgG.

INTRODUCTION: Rubella is a generally mild viral infection, but it carries the risk of potentially severe complications, especially during pregnancy. Immunity to rubella is determined using commercial chemiluminescent immunoassays, calibrated with international standards. Despite this, discrepancies have been observed in the results obtained by different methods. METHODS: This study evaluates the performance of two chemiluminescent immunoassays (CLIAs) (Alinity i® Rubella IgG and Atellica IM® Rubella IgG) in determining rubella immunity in patient sera with low levels of anti-rubella virus (RV

A rare cause of neonatal cholestasis: congenital portosystemic shunt.

Neonatal cholestasis is a pathological condition characterized by impaired bile flow in infants, typically presenting within the first 3 months of life. While common etiologies include biliary atresia, metabolic disorders, and infections, rare causes such as congenital portosystemic shunts must also be considered in the differential diagnosis. We present the case of a term infant with a congenital portosystemic shunt who presented with cholestasis and hyperammonemia. Bilirubin and liver function enzymes gradually decreased after initiation of ursodeoxycholic acid therapy. In newborns presentin

HTLV-1, a neglected disease among breastfeeding women from endemic areas: a national survey of knowledge and screening practices in France.

BACKGROUND: Screening of some infectious diseases is consistently recommended during pregnancy in France. Although Human T lymphotropic virus type 1 (HTLV-1) infection is a contraindication for breastfeeding, its screening is not routinely recommended as it is uncommon in a native mainland French population. However, HTLV-1 screening should be recommended among women from endemic countries. This study aims to evaluate HTLV-1 knowledge and screening practices of French healthcare professionals among pregnant women. METHODS: A French national professional practice evaluation study was conducted

Infections during pregnancy: An ongoing threat.

Congenital infections are an ongoing relevant cause of congenital and perinatal abnormalities since effective vaccination is not available for many diseases. Moreover, climate change, mutational evolution of pathogens, or their vectors associated with intense human traveling predispose to outbreaks and modification of the geographic distribution of traditionally limited infectious diseases. Here, we review the so-called TORCH infections (toxoplasmosis, others, rubella, cytomegalovirus, herpes) with a special emphasis on the arboviral infections (zika, dengue, chikungunya, oropouche). Cataracts: A Review.

IMPORTANCE: Age-related cataract, defined as progressive opacification or clouding of the eye's natural lens, is a leading cause of visual disability and blindness. Cataract surgery is one of the most commonly performed procedures in high-income countries. More than 3.5 million cataract operations are performed annually in the US. OBSERVATIONS: Older age is the primary risk factor for cataracts, with approximately two-thirds of the population older than 80 years affected. As the population ages, the number of people with cataracts in the US is expected to increase to 50 million by 2050. Addit

TAP-I Deficiency Presenting With Chronic Granulomatous Rubella Virus-Driven Cutaneous Ulceration: A Case Report and Scoping Literature Review.

Autosomal recessive mutations in TAP1, TAP2, TAPBP, or B2M, are associated with major histocompatibility complex (MHC) class I deficiency. Individuals may present with granulomatous skin ulceration, but the underlying antigenic triggers remain largely unknown. We identified TAP1 deficiency in a 32-year-old female referred with a 7-year history of localized skin ulceration. Histologic immunofluorescence revealed that rubella virus (RuV) infection was a likely driver of the associated inflammation, and modest clinical improvement was observed following topical calcineurin inhibition. Unveiling the Hidden Burden of TORCH Infections in HIV Patients: A Residual Serum Seroepidemiological Study in a Korean Metropolitan City.

BACKGROUND: Although human immunodeficiency virus (HIV) is a legally notifiable disease in Korea, surveillance data are reported in aggregate without regional breakdowns. This limits the ability of local governments to design targeted prevention and care strategies. The coronavirus disease 2019 pandemic underscored the importance of region-specific epidemiological data for timely public health responses. TORCH infections-comprising Toxoplasma gondii, Treponema pallidum, rubella virus, cytomegalovirus (CMV), and herpes simplex virus (HSV)-can cause severe complications in immunocompromised indi

The Effects of Vaccination Status and Age on Clinical Characteristics and Severity of Measles Cases in the United States in the Postelimination Era, 2001-2022.

BACKGROUND: Despite high vaccine effectiveness, wild-type measles can occur in previously vaccinated persons. We compared the clinical presentation and disease severity of measles by vaccination status and age in the postelimination era in the United States. METHODS: We included U.S. measles cases reported from 2001 to 2022. Breakthrough measles was defined as cases with ≥1 documented dose of measles-containing vaccine, classic measles as the presence of rash, fever, and ≥1 symptoms (cough, coryza, or conjunctivitis), and severe disease as the presence of pneumonia, encephalitis, hospitalizat

Spatial analysis of measles cases and vaccination coverage in the Somali region, eastern Ethiopia.

BACKGROUND: Measles is a major public health concern that causes morbidity and mortality among children. In 2019, measles incidence reached its highest level in 23 years, with low measles containing vaccine dose one coverage playing a vital role. It can be prevented by two doses of the measles vaccine, either alone or in combination with measles-rubella (MR), which is a low-cost strategy for lowering morbidity and mortality among children. OBJECTIVES: To conduct spatial analysis of measles cases and vaccination coverage in the Somali region, Eastern Ethiopia. METHODS: This retrospective stud

Serological status of measles, mumps, and rubella antibodies in HIV-positive women of childbearing age at a referral hospital in Iran.

BACKGROUND: The human immunodeficiency virus (HIV) increases susceptibility to measles, mumps, and rubella (MMR) infections due to decreased cluster of differentiation 4 + T-cell levels and rapid waning of protective antibodies following vaccination, which imposes a significant impact on HIV-positive women of reproductive age, for whom MMR vaccination is a crucial preventive measure. This study aimed to shed light on the immunity status of women of childbearing age with HIV infection post-MMR-vaccination during their childhood and the necessity of further vaccination in these individuals. What's Old Is New Again: Measles.

Measles is resurging in the United States, fueled by decreasing vaccination rates and rising vaccine hesitancy. Clinicians are confronted with a highly infectious vaccine-preventable disease that is unfamiliar to them because, as a result of routine childhood vaccination with measles, mumps, and rubella vaccine, measles was declared nonendemic in the United States in 2000 and many pediatricians have never seen a case. In this article, we provide an update on measles presentation, clinical course, and outcomes by reviewing the literature and sharing the expertise of senior pediatric infectious

Measles: An Ongoing Threat.

Measles is an acute, serious, highly contagious viral infection. Despite the existence of an effective and safe vaccine for more than half a century, measles outbreaks continue in the United States and cases are increasing worldwide. Further, there is a decreasing trend in national measles, mumps, and rubella vaccination coverage among kindergarteners. This article is intended to provide practitioners caring for children with an overview of the history of measles, its current national and international epidemiology, clinical picture, and important complications, as well as methods for preventi
