Emergency department management of measles, mumps, rubella, and varicella in pediatric patients.

Public health programs have helped decrease the prevalence and sequelae of measles, mumps, rubella, and varicella; however, misinformation regarding vaccinations and recent disruptions to vaccination programs have caused outbreaks to occur. Emergency clinicians must be able to readily identify the characteristic presentations of measles, mumps, rubella, and varicella, with the goals of optimizing supportive care, limiting the spread of disease, and giving appropriate precautionary advice for complications to patients and their families. This issue provides a review of the clinical features, di

Measles Matters: A Clinical Overview and Update.

Measles is a highly transmissible viral disease that is preventable through vaccination. Despite global initiatives, outbreaks persist, demonstrating the need for high immunization coverage. Spread occurs via respiratory droplets and airborne particles, reinforcing the need for herd immunity. Symptoms include fever, cough, and a distinctive rash. Complications may include pneumonia, encephalitis, and in rare cases, acute disseminated encephalomyelitis and subacute sclerosing panencephalitis. Diagnosis relies on reverse transcription-polymerase chain reaction and serology tests. Two doses of th

[Subacute sclerosing panencephalitis].

Subacute sclerosing panencephalitis (SSPE) is a rare, fatal complication of measles, manifesting years after infection due to persistent measles virus in the central nervous system. An 11-year-old Syrian boy with prior measles and measles - mumps - rubella (MMR) vaccination developed progressive encephalopathy and a vegetative state. MRI showed diffuse white matter, thalamic, and pontine changes; SSPE was confirmed post-mortem. This case emphasises the need for high MMR vaccine coverage and the importance of clinical awareness of rare infectious diseases in patients from crisis-affected countr

Modeling Reemergence of Vaccine-Eliminated Infectious Diseases Under Declining Vaccination in the US.

IMPORTANCE: Widespread childhood vaccination has eliminated many infectious diseases in the US. However, vaccination rates are declining, and there are ongoing policy debates to reduce the childhood vaccine schedule, which may risk reemergence of previously eliminated infectious diseases. OBJECTIVE: To estimate the number of cases and complications in the US under scenarios of declining childhood vaccination for measles, rubella, poliomyelitis, and diphtheria. DESIGN, SETTING, AND PARTICIPANTS: A simulation model was used to assess the importation and dynamic spread of vaccine-preventable in

Vaccine hesitancy and bone health: Musculoskeletal sequelae of vaccine-preventable diseases.

Vaccine hesitancy threatens global health by diminishing herd immunity and permitting the return of vaccine-preventable diseases (VPDs) including poliomyelitis, measles and coronavirus disease 2019 (COVID-19). These infections carry short-term and long-term musculoskeletal consequences, such as paralysis, limb deformities, septic or reactive arthritis, osteomyelitis and chronic pain, that place an ever increasing burden on orthopaedic services. This narrative review synthesizes clinical and epidemiological evidence linking sub-optimal immunization to orthopaedic morbidity caused by major VPDs.

Disparities in Toxoplasmosis, "Other" Infections (Syphilis, HIV, Hepatitis Viruses, Varicella-Zoster Virus, and Parvovirus B19), Rubella, Cytomegalovirus, and Herpes Simplex Virus Seroprevalence and Childbirth Experiences between Migrant and Nonmigrant Pregnant Women in Italy: A Single-Center Study.

The toxoplasmosis, "other" infections (syphilis, HIV, hepatitis viruses, varicella-zoster virus, and parvovirus B19), rubella, cytomegalovirus, and herpes simplex virus (TORCH) complex poses significant risks for maternal and neonatal outcomes. Additionally, the childbirth experience, particularly for migrant women, is shaped by sociocultural and linguistic factors. This study evaluated the prevalence of TORCH infections and compares childbirth experiences between migrant and nonmigrant pregnant women in Italy. This observational study was conducted from January 1 to October 30, 2024, and it i

Optimizing Pregnancy Outcomes: The Role of Gynecologists in Preconceptional Care in Italy.

The preconceptional period is crucial for women and couples who desire children as it influences the progress and outcomes of a healthy pregnancy. Preconceptional care is now an accepted paradigm by most healthcare professionals. However, its implementation in healthcare programs is still in the early stages. Preconceptional care starts with communication and a collection of personal and family medical history. It then encompasses a variety of recommendations for lifestyle interventions, as well as preventive laboratory tests. This review will summarize current recommended procedures, from nut

STORCH Brazil: multicenter cohort study protocol to investigate neurodevelopmental paths and functioning in infants exposed to STORCH in Brazil.

BACKGROUND: The acronym STORCH encompasses gestational infections that can lead to congenital syndromes or adverse neurological outcomes in children. In Brazil and worldwide, there has been an alarming increase in confirmed cases of STORCH in recent years. However, no study has examined the impact of STORCH on infants' neurodevelopmental outcomes in a large, multi-center cohort, recruiting a substantial number of participants, with analysis across a broad set of variables and ages and based on the International Classification of Functioning, Disability and Health (ICF) model. OBJECTIVE: To ex

Speech and Language Development in Children with Congenital Infections: Findings from a Specialized Hearing Health Service.

INTRODUCTION: Congenital infections remain a significant public health concern worldwide, contributing to developmental delays that may compromise speech, language, and cognitive development. These infections can lead to sensorineural hearing loss (SNHL), further affecting communication and neurodevelopment. This study aimed to assess speech, language, and developmental milestones in children with congenital infections, with and without SNHL, at a specialized auditory diagnosis and rehabilitation center. METHODS: This cross-sectional observational study included 55 children with laboratory-co

Hammersmith Infant Neurological Examination at 3 Months in Infants at Risk for Congenital Infections: A Cohort Study.

INTRODUCTION: STORCH refers to a group of congenital infections (syphilis, toxoplasmosis, rubella, cytomegalovirus and herpes) that can impact the central nervous system. As clinical signs may not appear until several months or years after birth, the early detection of risk in STORCH-exposed infants has been challenging, and the use of sensitive tools in this population is understudied. OBJECTIVE: To compare STORCH-exposed infants with non-exposed controls using the Hammersmith Infant Neurological Examination (HINE) at 3 months of age. METHODS: This is an observational cohort study. A total

TORCH: Current state of knowledge as of 2025.

The acronym TORCH designates a group of pathogens that can lead to serious pregnancy complications, such as miscarriage, fetal growth restriction, and congenital infections. This review summarizes current insights into these infections, offering practical guidance primarily for obstetricians, infectious disease specialists, and general practitioners involved in prenatal care. The TORCH complex includes Toxoplasma gondii, Rubella virus, Cytomegalovirus (CMV), and Herpes simplex virus (HSV). The definition may be extended to encompass additional (other) pathogens such as Hepatitis B and C viruse

Individuals with latent tuberculosis in a high TB endemic country show mild COVID-19.

INTRODUCTION: Infection with Mycobacterium tuberculosis (MTB) may result in active tuberculosis (TB), bacterial clearance, or asymptomatic latent infection (LTBi). During the COVID-19 pandemic, interactions between MTB and SARS-CoV-2 infections were coincident in high TB burden countries such as Pakistan. The impact of LTBi on COVID-19 is not well understood. Here we investigated the association of LTBi with COVID-19 and its severity by determining cellular activation to MTB, IgG antibody responses and expression of genes associated with host immunity. METHODS: Age and sex matched Healthy Con

Anaplastic Thyroid Carcinoma: The Great Masquerader.

Anaplastic thyroid carcinoma (ATC) has been regarded as the rarest form of thyroid malignancy, which entails around 2% of thyroid malignancies. It traditionally presents with rapidly enlarging anterior neck swelling with obstructive symptoms. Due to its rich blood supply and lymphatic drainage, the thyroid gland is resistant to infection. Parallel to that, the presentation of thyroid abscess frequently points towards an underlying upper respiratory tract infection. We present a case involving an elderly female presenting with a thyroid abscess, which later turned out to be ATC following a hist

UNLABELLED: The consumption of pharmaceuticals has rapidly increased on a global scale due to the serious increase in Covid-19, influenza and respiratuar sinsityal virus, which is called "triple epidemic" in the world. The use of non-prescription analgesic and anti-inflammatory drugs (AAIDs), especially paracetamol, is higher compared to pre-pandemic. This increased the AAIDs load discharged to the aqueous media through sewerage treatment plant (STP). Therefore, simple and effective treatment options for removing AAIDs from STP effluents are needed. The aim of the study was to remove AAIDs (pa
