Efficacy and safety of oral minocycline hydrochloride 40 mg extended-release capsules (DFD-29) versus doxycycline in rosacea: a systematic review and meta-analysis.

BACKGROUND: Rosacea is a chronic skin disorder characterized by facial erythema, telangiectasias, papules and pustules. Guidelines recommend subantimicrobial doses of drugs such as doxycycline 40 mg. In patients with persistent symptoms, evaluating the efficacy and safety of subantimicrobial doses of minocycline may expand treatment options. OBJECTIVES: To evaluate minocycline hydrochloride 40 mg extended-release capsules (formerly known as and hereafter referred to as DFD-29) as a low-dose tetracycline alternative to treat rosacea. METHODS: Our analysis included data from three randomized c

UVB Upregulates Inflammatory Cytokines in Rosacea Cell Model by Promoting the Expression of TRPVs and TLR2.

BACKGROUND: The expression of transient receptor potential vanilloid (TRPV) and Toll-like receptor 2 (TLR2) in the lesion sites of patients with rosacea is increased. Ultraviolet B (UVB) radiation can induce inflammation of keratinocytes in patients with rosacea, but the relationships between UVB and TRPVs/TLR2 in rosacea remain unclear. METHODS: RT-qPCR and Western Blot were applied to determine the expression levels of TRPV1, TRPV2, TRPV3, TRPV4 and TLR2 in the rosacea keratinocyte model after ultraviolet irradiation (n=3). After treating the rosacea cell model with TRPV1, TRPV4 and TLR2 in

Acneiform drug eruptions-update on pathophysiology and culprit drugs.

Acneiform adverse drug reactions (ADRs) appear within weeks to months after the initiation of a new medication. As opposed to acne vulgaris, they typically present monomorphic inflammatory lesions, may involve atypical, non-seborrheic body sites and may arise outside the usual age range of any acne subtype. A wide range of drugs can trigger the eruption of acneiform ADRs with targeted therapies used in oncology (e.g., epidermal growth factor receptor inhibitors) carrying the highest risk. More recently, an increasing incidence of acneiform ADRs has been observed with the advent of januskinase

Safety and effectiveness of topical dapsone in acne treatment: A review.

Oral dapsone is effective in treating several dermatologic conditions, including acne vulgaris. However, the rate of adverse effects of oral dapsone is a concern when prescribing it. Topical dapsone may mitigate these adverse effects and enhance the safety and tolerability of dapsone. Here, we review the published reports and studies to examine the efficacy and safety of topical dapsone in treating acne. A literature review was conducted using the PubMed database. The review considered patient and disease factors, such as the type of acne, acne location, patient gender, age, skin type, and rac

An Attempt to Establish the Consensus Regarding the Diagnosis, Classification, and Treatment of Rosacea in Japan Using a Modified Delphi Method: The Japan Rosacea Consensus.

In Japan, rosacea has attracted increasing interest. However, because rosacea had been thought to be relatively rare in Japan, the perception of this disease varies among dermatologists. To address these challenges, this study developed a consensus regarding the diagnosis, classification, and treatment of rosacea using a modified Delphi method based on the expertise of Japanese specialists in rosacea. Ten Japanese dermatologists were included in the expert panel based on their expertise in rosacea treatment and contributions to the field of rosacea. For each item that mentioned rosacea, the sp

Case Report: Lupus miliaris disseminatus faciei successfully treated with thalidomide combined with photodynamic therapy: two cases and literature review.

Lupus miliaris disseminatus faciei (LMDF) is a rare granulomatous rosacea characterized by multiple papular eruptions primarily affecting the central face, resulting in both physical discomfort and notable cosmetic concerns. Despite various proposed therapies, the optimal treatment for LMDF remains controversial. Photodynamic therapy (PDT) employs a photosensitizer, light source, and oxygen to generate reactive oxygen species (ROS) that selectively target and destroy abnormal tissue, and it has also been shown to reduce and prevent scar formation. However, data on PDT use in LMDF is limited. H

The brain-gut-skin axis in inflammatory and disfiguring skin diseases: mechanistic insights, clinical correlations, and therapeutic strategies.

Emerging evidence suggests that the brain-gut-skin axis (BGSA) plays a critical role in the pathogenesis of inflammatory and disfiguring skin diseases. Conditions such as acne, atopic dermatitis, psoriasis, rosacea, vitiligo, and alopecia areata, once regarded as localized disorders driven mainly by cutaneous immune dysfunction, are now recognized as systemic conditions associated with neuroendocrine stress responses, gut microbial dysbiosis, and chronic low-grade inflammation. Mechanistic studies elucidate the intricate interorgan communication mediated by microbial metabolites (e.g., short-c

Combination Therapy with Oral Doxycycline and Intradermal Botulinum Toxin Type A for Moderate-to-Severe Rosacea: A Randomized Controlled Trial.

BACKGROUND: Rosacea is a chronic inflammatory skin disorder, particularly challenging in moderate-to-severe cases, characterized by symptoms such as facial papules, pustules, erythema, flushing, and telangiectasia, which severely impact quality of life. Although oral doxycycline is a primary treatment, its efficacy against erythema is limited, necessitating the exploration of combination therapies. Intradermal botulinum toxin type A (BTX) has shown promise in managing facial erythema and flushing, but its combined use with doxycycline has not been evaluated. OBJECTIVES: This study aims to eva

Tear Film Interferometry, Meibography, and Optical Coherence Tomography Angiography for Rosacea.

BACKGROUND/OBJECTIVES: Rosacea is a chronic inflammatory dermatosis that may involve the eye, causing surface and adnexal damage that can precede cutaneous signs. Detecting subclinical ocular changes is clinically important because early ocular surface dysfunction may be missed on routine examination yet progress to corneal complications, allowing earlier preventive management when identified. We prospectively evaluated subclinical ocular alterations in cutaneous rosacea using a combined, fully non-invasive high-tech imaging workflow-tear film interferometry, infrared meibography, and explorat

TRPV4-Mast Cell Interactions in Neurogenic Inflammation and Chronic Diseases: A Narrative Review.

Transient receptor potential vanilloid 4 (TRPV4) is a polymodal cation channel that is widely expressed in sensory neurons, immune cells, and structural tissues, where it integrates mechanical, osmotic, and chemical stimuli to regulate both physiological responses and disease-associated signaling. Mast cells (MCs), key immune effector cells capable of rapid mediator release through degranulation, also express TRPV4. Increasing evidence supports TRPV4-MC signaling as an important neuroimmune interface, linking mechanical and inflammatory stimuli to tissue hypersensitivity and pain. In this revi

Borneol-caprylic acid deep eutectic solvent (DES) based microemulsion gel for improved topical delivery of ivermectin.

Ivermectin (IVM), a broad-spectrum and hydrophobic antiparasitic drug, is used topically for the treatment of inflammatory skin conditions such as rosacea. The hydrophobicity of IVM and its limited solubility in lipid vehicles necessitate the development of new topical delivery approaches. Here, we evaluate the potential of deep eutectic solvent (DES) to improve the solubility of IVM, followed by the development of a DES-in-water microemulsion gel to improve the topical delivery of IVM. We demonstrate that borneol, a hydrophobic bicyclic terpene alcohol with penetration-enhancing capabilities,

Environmental and Lifestyle Triggers of Rosacea.

Rosacea is a chronic inflammatory skin condition characterized by facial erythema, flushing, telangiectasias, papules, and pustules. Its pathophysiology involves a multifactorial interplay of genetic, immune, microbial, and neurovascular factors contributing to inflammation and vascular dysregulation. Environmental and lifestyle triggers are important contributors to disease exacerbation. In this review, we highlight 4 major modifiable triggers-UV radiation exposure, temperature fluctuation, skin care practices, and diet-and their roles in rosacea pathogenesis and management. Lifestyle modific
