Pilar cysts, also known as trichilemmal cysts, are benign keratin-filled lesions that arise from the outer root sheath of the hair follicle. This article presents the rare occurrence of a trichilemmal cyst in the orbit of a 69-year-old female patient, who presented with a headache of acute onset and relative unilateral proptosis of the left eye. CT scan showed a cystic, well-demarcated, round hypodense lesion, with spotted calcification. Surgery was performed for the extraction and analysis of the lesion, which showed typical histological findings of a trichilemmal tumor. Pilomatricoma and trichilemmal cyst are two benign tumors of hair follicles that can affect children. We performed a retrospective study to histologically differentiate the two tumors to determine if this has clinical utility.
