Proliferating trichilemmal cyst: an ultrastructural study.

A 73-year-old woman had a walnut-sized tumor in the occipital region for 10 years. The growth consisted of multiple small and large cystic epithelial masses. The cyst wall exhibited trichilemmal keratinization. Numerous squamous eddies as well as many glycogen-rich cells were present in the cyst wall. Some small round keratohyalin granules were observed in the cells below the horny layer. The desmosomes and masses of lipid droplets were seen in the horny layer. The findings described above are similar to those observed in the follicular isthmus and the follicular infundibulum. Other interestin

Proliferating pilar (trichilemmal) cyst. Report of two cases, one with carcinomatous transformation and one with distant metastases.

We examined two cases of proliferating pilar cysts of the inguinal region. One of the cases showed a transformation to epidermoid carcinoma and, despite surgical resection, the patient died by means of distant generalized metastases. The diagnosis of proliferating pilar cyst should be made in pilar cysts with epithelial proliferation that shows well-defined borders and abundant keratinization despite cytologic atypia and mitotic activity. The diagnosis of carcinoma in proliferating pilar cysts should be reserved for lesions showing poorly defined borders and clear-cut infiltrative properties w

[Importance of the proliferating trichilemmal cyst].

The biological behavior of proliferating trichilemmal cysts is commented on, based on two cases. Two female patients, 71 and 70 years of age, developed solitary monstrous tumours of the scalp in the course of repeatedly recurring trichilemmal cysts. The biological behaviour of both was marked by infiltration of the galea aponeurotica, and in the latter case, invasion of the calvaria and the venous sinuses of the brain with a lethal outcome. Histologically, the differentiation of the cyst walls ranged from pseudoepitheliomatous hyperplasia to areas largely lacking differentiation, indicating a

Development of a malignant proliferating trichilemmal cyst in a patient with multiple trichilemmal cysts.

A case of multiple trichilemmal cysts is presented. After trauma, a large tumor developed on one of the cysts. The histologic features of this tumor were similar to those of proliferating trichilemmal cyst, but some of the tumor cells showed definite nuclear atypicality. The malignant potential of this tumor was confirmed by later development of metastases to the regional lymph nodes.

Proliferating trichilemmal cyst.

Fourteen cases (9 female, 5 male) of proliferating trichilemmal cysts are reported. The patients age-range was 40-80, 12 being over 60. The cysts wee present for 2-50 years and enlarged in the months or years before the observation. Seven were ulcerated. In 4 with multiple lesions both proliferating and ordinary trichilemmal cysts were present. The histological features were typical in all but one patient (evolution towards squamous cell carcinoma?) with some variation from lesion to lesion and from area to area in the same tumour. The microscopic features suggest that the proliferating trichi

Hybrid cyst: a combined epidermoid and trichilemmal cyst.

Seven cases of hybrid cyst, a combined epidermoid and trichilemmal cyst, are reported. Hybrid cysts occurred in five men and two women, and involved the scalp, cheek, back, arm, elbow, and forearm. The clinical diagnosis was usually cyst or sebaceous cyst. Microscopically, the upper portion of the cyst showed epidermoid keratinization in continuity with the surface epidermis, and was indistinguishable from an epidermoid cyst. Showing sharp transition from it, the inferior portion of the lesion displayed trichilemmal keratinization and was indistinguishable from a trichilemmal cyst. Hybrid cyst

[Proliferating trichilemmal cyst. Apropos of a case].

Malignant change in some keratinous cysts is a possible but rare condition. It seems acceptable that many publications concerning squamous cell carcinomas arising from previous cysts, actually correspond to the entity which Wilson-Jones called "proliferating epidermoid cyst" and they must be considered as pseudomalignant lesions, despite the presence of some features that may lead to a diagnosis of carcinoma. Although it is rare, which is nowadays called "proliferating Trichilemmal cyst", should be widely acknowledged since its misdiagnosis as squamous cell carcinoma leads to surgical treatmen

Proliferating trichilemmal cyst: a simulant of squamous cell carcinoma.

Comparison of 50 proliferating trichilemmal cysts with 50 "ordinary" trichilemmal cysts indicated that both types almost invariably occurred on the scalps of women, were diagnosed clinically as cysts, followed a benign course, and featured trichilemmal keratinization. A spectrum was observed from trichilemmal cyst with minimal hyperplasia, to full-blown proliferating trichilemmal cyst. Occasionally, patients had ordinary trichilemmal cysts on their scalps associated with a proliferating trichilemmal cyst. In addition to trichilemmal keratinization, which is characteristic of the follicular ist

Trichilemmal horn: cutaneous horn showing trichilemmal keratinization.

A unique and distinctive clinicopathological entity occurred in nineteen patients who ranged in age from 16 to 72 (median 50) years. Clinically, these were solitary cutaneous horns. Nine were on the limbs, four on the back, two on the face, three on the scalp, and in one the site was not known; the median duration was 2 years. Histologically, there was a benign picture with a protrusion of massive horn and trichilemmal keratinization at the base. This tumour, which I have named trichilemmal horn, must be differentiated from other lesions that show trichilemmal keratinization (trichilemmal cyst

Trichilemmal tumor undergoing specific keratinization: "keratinizing trichilemmoma".

Numerous tumors with follicle-like direction of differentiation have been described. At least three of them connected to the overlying epidermis are related to the outer root sheath or trichilemma: the tumor of follicular infundibulum (Mehregan & Butler 1961) and the inverted follicular keratosis (Helwig 1955) or follicular poroma (Duperrat & Mascaro 1963) have been related to the infundibulum, the tricholemmoma (Headington & French 1961) to the lower outer root sheath. On the occasion of a case first diagnosed as proliferating trichilemmal cyst of the scalp, we think we have individualized a

[Pigmented cysts. Pigmented epidermal cysts and pigmented trichilemmal cyst].

The clinical and histopathological picture of the pigmented epidermal cyst and the pigmented trichilemmcyst is illustrated by three case-reports. The pigment is melanin, which is located in the epithelium of the cyst-wall, the corneocytes of the lumen and in macrophages in the dermis. The clinical appearance of melanin pigmented cysts is blue due to the Tyndall-phenomenon.

Visualization of nonvascular cranial tumor in cerebral flow study: case report.

Unusual results were obtained from a cerebral blood flow study and a brain scan performed on a patient with an uncommon, nonvascular scalp tumor eventually identified as a proliferating trichilemmal cyst with acanthosis. The tumor accumulated the radionuclide during the early phase of the cerebral flow study, and the increased radioactivity was persistent, being prominently observable in a scan made 3 min and 2 hr after the radionuclide injection. This unusual, positive finding in a cerebral blood flow study of a nonvascular tumor was thought to be a consequence of the higher counting efficien
