Skin care in ethnic populations.

Use of over-the-counter cosmetics, approaches to hygiene, and many basic dermatologic principles differ between individuals with Caucasian skin and ethnic skin. Still, comparatively few publications highlight these variations or discuss appropriate management. Among many ethnic patients, issues related to skin hydration, restoration of even pigmentation, hair removal, and acne care remain problematic yet not fully addressed. As well, there are some dermatologic conditions that may be rare in Caucasian skin but are much more common in the ethnic patient. Here, we discuss various aspects of skin

Cost-effectiveness evaluation of a quadrivalent human papillomavirus vaccine in Belgium.

BACKGROUND: Nearly 1 million new episodes of herpes zoster (HZ) occur annually in the US, yet little is known about the medical resource utilization (RU) and costs associated with HZ and its complications. OBJECTIVES: To describe the medical RU and cost burden of HZ in the first 90 days and the first year after diagnosis from the health insurer perspective and to stratify this burden for patients diagnosed with post-herpetic neuralgia (PHN) and those who are immunocompromised. In addition, this study explores costs from the societal perspective as a result of work loss in the first year after

Hypertrophic pseudofolliculitis in white renal transplant recipients.

Renal transplant recipients (RTRs) are susceptible to many cutaneous disorders, including drug-induced skin changes. Hypertrichosis, sebaceous hyperplasia and gingival hyperplasia are well-recognized effects of ciclosporin, one of the immunosuppressants commonly used in RTRs. Pseudofolliculitis barbae usually affects hair-bearing areas of skin in men with darkly pigmented skin who shave on a regular basis. We describe five cases of hyperplastic pseudofolliculitis occurring in white RTRs taking ciclosporin as part of their immunosuppressive regimen. All five had involvement of the chin, the cla

Hair follicle-specific keratins and their diseases.

The human keratin family comprises 54 members, 28 type I and 26 type II. Out of the 28 type I keratins, 17 are epithelial and 11 are hair keratins. Similarly, the 26 type II members comprise 20 epithelial and 6 hair keratins. As, however, 9 out of the 37 epithelial keratins are specifically expressed in the hair follicle, the total number of hair follicle-specific keratins (26) almost equals that of those expressed in the various forms of epithelia (28). Up to now, more than half of the latter have been found to be involved in inherited diseases, with mutated type I and type II members being r

Epiluminescence dermatoscopy enhanced patient compliance and achieved treatment success in pseudofolliculitis barbae.

SUMMARY A patient presented with recalcitrant pseudofolliculitis barbae and hypertrophic scarring. The use of epiluminescence dermatoscopy rendered clear visualization of U-shaped ingrowing hairs corresponding to the sites of individual papules. Such real-time demonstration led to an attitude change and good compliance with medical advice against over shaving, resulting in a successful treatment outcome.

Modified superlong pulse 810 nm diode laser in the treatment of pseudofolliculitis barbae in skin types V and VI.

BACKGROUND: Laser hair removal is a well-established therapy for pseudofolliculitis barbae, but there have been few studies on the use of a very-long-pulse 810 nm diode laser for this condition. OBJECTIVE: To determine the efficacy and tolerability of a modified 810 nm superlong-pulse diode laser (Palomar Medical, Burlington, MA, USA) in the treatment of pseudofolliculitis barbae (PFB) in subjects with type V and VI skin. METHODS: Thirteen patients were treated three times at 2-week intervals on one side of their neck with varying fluences. They were evaluated for (1) reduction of shaving bu

Direct comparison of EMLA versus lidocaine for pain control in Nd:YAG 1,064 nm laser hair removal.

BACKGROUND: Laser hair removal is an effective therapy for the treatment of hirsutism, hypertrichosis, and pseudofolliculitis barbae. Although side effects are uncommon, pain is described by most patients undergoing long-pulsed neodymium:yttrium-aluminum-garnet (Nd:YAG) 1,064 nm laser therapy. OBJECTIVES: To compare the efficacy of topical eutectic mixture of local anesthetics (EMLA) versus topical lidocaine (LMX) in pain control for Nd:YAG 1,064 nm laser hair removal. METHODS: Sixty-four patients were enrolled in a double-blind randomized study over a 6-month period. Each patient had half o

Difficult wounds: an update.

The purpose of this collective review is to describe revolutionary advances in the treatment of Gardner's syndrome (GS), pseudofolliculitis barbae, nasal septal perforation, factitious wounds, and hidradenitis suppurativa (HS). Gardner's syndrome or familial polyposis has various manifestations that appear to be controlled by a single genetic locus. Apart from the large bowel adenomas, which are always present, a common extracolonic symptom of Gardner's syndrome is the occurrence of epidermal cysts. These cysts can be seen before the intestinal polyps are evident. Because epidermal cysts in pa

[Clinical variants of acne].

Acne is a very common dermatosis with characteristic clinical features. It is a polymorphic disease. The clinical expression ranges from non-inflammatory closed and open comedones to inflammatory papules, pustules, and nodules. Most patients have a mixture of non-inflammatory and inflammatory lesions, although some have predominantly one or the other. Acne varies in severity from a very distressing, socially disabling disorder to a state that has been regarded as physiological by some authors. The most severe forms of acne are acne fulminans and acne inversa. Although acne may occur in all age

Enhancing the care and treatment of skin of color, part 1: The broad scope of pigmentary disorders.

Scientific research and technologies related to skin pigmentation and dyschromias, which are often key skin concerns for patients of color, have led to recent developments in skin care and treatment. Differences and similarities between skin of color and white skin and current issues in the treatment of ethnic skin are reviewed. Recent research findings, such as the elucidation of the protease-activated receptor 2 (PAR-2) pathway and its role in pigmentation, and areas for further investigation, such as the pathogenesis of pseudofolliculitis barbae (PFB), also are discussed. Awareness of this

An unusual Ala12Thr polymorphism in the 1A alpha-helical segment of the companion layer-specific keratin K6hf: evidence for a risk factor in the etiology of the common hair disorder pseudofolliculitis barbae.

Pseudofolliculitis barbae (PFB) is a common hair disorder characterized by a pustular foreign body inflammatory reaction that is induced by ingrown hairs of the facial and submental (barbea) regions after regular shaving. It occurs predominantly in black males, while it is rather rare and usually far less severe in Caucasian males. Black individuals have a higher propensity of developing PFB due to their genetic predisposition for curly hair which inherently possesses a much higher risk of growing back into the skin than straight or wavy hair. The PFB process is, however, not gender dependent

The medical and surgical therapy of pseudofolliculitis barbae.

Pseudofolliculitis barbae (PFB) is a common, chronic, inflammatory skin disorder seen mainly in individuals with curly hair. This condition is seen most frequently in black men who shave their beards but may also be seen in women of all races who wax or shave the axillary and pubic skin. The etiology of PFB is multifactorial, and heretofore a cure has been considered impossible for those desiring a clean-shaven face. The following article serves to discuss the current medical and surgical therapies available for this condition. Medical treatments for this condition include various combinations

Laser-assisted hair removal for darker skin types.

Before the advent of longer wavelengths, longer pulse durations and more efficient cooling devices, laser-assisted hair removal was best suited for phototypes I-III with dark terminal hairs. Now, laser-assisted hair removal can be performed safely and efficaciously on darker skin types. The long-pulsed diode and Nd:YAG-wavelength-based laser systems are best suited to safely and effectively treat patients with darker skin types. By using conservative fluences, longer pulse durations and multiple treatments, safe and effective laser-assisted hair removal is possible for darker skin types. Patie

Chemical peeling in ethnic/dark skin.

Chemical peeling for skin of color arose in ancient Egypt, Mesopotamia, and other ancient cultures in and around Africa. Our current fund of medical knowledge regarding chemical peeling is a result of centuries of experience and research. The list of agents for chemical peeling is extensive. In ethnic skin, our efforts are focused on superficial and medium-depth peeling agents and techniques. Indications for chemical peeling in darker skin include acne vulgaris, postinflammatory hyperpigmentation, melasma, scarring, photodamage, and pseudofolliculitis barbae. Careful selection of patients for

Twice-daily applications of benzoyl peroxide 5%/clindamycin 1% gel versus vehicle in the treatment of pseudofolliculitis barbae.

Pseudofolliculitis barbae (PFB) is an inflammatory disorder characterized by the formation of papules, pustules, and hyperpigmentation as a result of ingrown hairs. It is estimated that PFB affects 45% to 83% of black men who shave regularly. In this multicenter, double-blind pilot study, men with 16 to 100 combined papules and pustules on the face and neck were randomized to receive twice-daily benzoyl peroxide 5%/ clindamycin 1% (BP/C) gel (BenzaClin)(n = 47) or vehicle (n = 41) for 10 weeks. Sixty-eight (77.3%) of the participants were black. All patients were required to shave at least twi
