Epidemiology, treatment patterns, and survival outcomes of porocarcinoma: A population-based analysis.

Porocarcinoma is a rare malignant adnexal tumor arising from the eccrine sweat gland acrosyringium. The epidemiology, treatment patterns, and prognostic factors for this malignancy remain incompletely characterized due to its extreme rarity. This study aimed to provide a comprehensive population-based analysis of porocarcinoma in the United States. We retrospectively analyzed all porocarcinoma cases (ICD-O-3 code 8409/3) from the Surveillance, Epidemiology, and End Results database diagnosed between 2000 and 2022. Patient demographics, tumor characteristics, treatment modalities, and survival

Obesity, skin disorders, and the microbiota: Unraveling a complex web.

Obesity is increasingly recognized as a systemic pro-inflammatory condition that influences not only metabolic and cardiovascular health but also the development and exacerbation of cutaneous inflammatory diseases. This review examines the interplay between obesity, microbial dysbiosis, and two archetypal inflammatory skin disorders-hidradenitis suppurativa (HS) and psoriasis. We highlight how obesity-induced changes in immune signaling, gut permeability, and microbiota composition-both in the gut and the skin-contribute to cutaneous inflammation. Special emphasis is placed on shared pathways

Targeting Neutrophil Function as Therapy for Hidradenitis Suppurativa.

Hidradenitis suppurativa (HS) is a chronic, recurrent inflammatory skin disease characterized by painful nodules, abscesses, and epithelialized tunnels, predominantly affecting flexural regions. With a global prevalence of approximately 1%, HS has a significant negative impact on quality of life. Multi-omics and histopathology studies have revealed a complex interplay between innate and adaptive immunity in HS, with neutrophils emerging as important drivers of inflammation. While therapies targeting TNF-α and IL-17 isoforms offer a degree of benefit, significant unmet need remains. Neutrophil

Dermal Fibroblasts, Not Keratinocytes, Dominate IL-17A/TNF-Driven Inflammation.

Chronic inflammatory skin diseases such as psoriasis and hidradenitis suppurativa are driven by cytokines, including IL-17A and TNF. Although biologics targeting these cytokines have transformed therapy, the transcriptional contributions of individual skin-resident cell types remain unclear, and dermal fibroblasts have been largely overlooked compared with keratinocytes. To address this, we compared the transcriptional responses of primary human dermal fibroblasts and keratinocytes following in vitro stimulation with IL-17A, TNF, or both, using bulk RNA sequencing and Western blotting. Dermal

Predictors for Remission Versus Refractory Disease Following Wide Local Excision Surgery for Hidradenitis Suppurativa: A Single-Center Case-Control Analysis.

Hidradenitis suppurativa (HS) is a chronic skin disease with a significant psychosocial burden. Skin of color (SOC) populations remain underrepresented in HS literature. Surgical excision is often pursued after failed medical management, but predictors of postsurgical disease recurrence are not well characterized. This study aimed to identify nonsurgical risk factors associated with persistent, refractory HS despite medical and surgical intervention in a predominant population of patients with SOC. A retrospective chart review was conducted on 31 adult patients with HS having prior excision su

Syringomatous Tumor Originating From an Axillary Supernumerary Mammary Tissue: First Report of an Axillary Tumor Case.

Syringomatous tumor typically occurs in the nipple of female patients and is regarded as a breast-specific tumor rather than a cutaneous adnexal tumor, owing to its nipple location and its differentiation toward large breast ducts. Herein, we report the first axillary case of syringomatous tumor arising from supernumerary mammary tissue lacking a supernumerary nipple. A 25-year-old woman presented with a 7-mm nodule in the left axilla. Histopathologic examination revealed a poorly circumscribed tumor composed of small tubules embedded in abundant fibrosing stroma, extending from the mid-dermis

Robotic ramicotomy for palmar and axillary hyperhidrosis.

Ramicotomy is a surgical approach that focuses on dividing the rami communicantes of the sympathetic chain and has been introduced as a treatment for palmar and axillary hyperhidrosis with the goal of reducing the incidence of compensatory sweating. Evidence from recent randomized controlled studies and meta-analyses suggests that this technique effectively reduces localized excessive sweating while resulting in lower rates of compensatory hyperhidrosis and less postoperative hand dryness compared with traditional sympathetic chain interruption. Despite these advantages, ramicotomy has been li

Evaluating the diagnostic accuracy of a screening questionnaire for detecting hidradenitis suppurativa: a pooled analysis of accuracy measures from the Global Hidradenitis Suppurativa Atlas (GHiSA) study.

BACKGROUND: Hidradenitis suppurativa (HS) is an inflammatory skin condition that is associated with a prolonged diagnostic delay of approximately 7-10 years. The diagnostic delay can be attributed to various factors, including low awareness of diagnostic criteria among nondermatological healthcare professionals often leading to misdiagnosis. Screening questionnaires have been proposed for the diagnosis of HS. One such questionnaire has been validated and used in the Global Hidradenitis Suppurativa Atlas (GHiSA) Global Prevalence Study (GPS). OBJECTIVES: To evaluate and provide a summary of th

The Emerging Role of Gut Microbiota in Inflammatory Skin Diseases: A Systematic Review.

The human gut microbiota is involved in immune regulation, metabolism, and skin homeostasis. In recent years, gut microbiota alterations have been linked with several inflammatory skin disorders, such as atopic dermatitis (AD), psoriasis, and hidradenitis suppurativa (HS). This systematic review synthesises current evidence on gut microbiota composition and functional alterations in these dermatoses. A comprehensive literature search was conducted in the PubMed database, identifying studies from inception to January 2025. Eligible studies included human observational, interventional, and genet

Defining Moderate Disease and Progression in Hidradenitis Suppurativa: An Expert Framework to Unlock the Window of Opportunity for Prompt Treatment.

The concept of a therapeutic window of opportunity, defined as the period from symptom onset during which treatment initiation yields the most favorable patient outcomes, is applied in routine clinical practice across a range of inflammatory conditions. It has become an increasingly important area of interest in hidradenitis suppurativa (HS), a disease in which recurrent inflammation and accumulating damage can lead to irreversible destruction of skin architecture. Biologic therapies, aiming to suppress the inflammatory burden and prevent disease progression, are currently only permitted in mo

Efficacy of Biologics for Hidradenitis Suppurativa: A Network Meta Analysis and Meta Regression for Baseline Disease Severity.

Hidradenitis Suppurativa is a challenging, heterogeneous inflammatory skin disease, with three approved biologic therapies. No head-to-head studies exist comparing these therapeutic modalities, and previous work has identified the influence of baseline disease severity upon clinical response. We aimed to perform a network meta-analysis of approved biologic therapies and meta-regression of baseline disease severity in moderate to severe HS. Pubmed, Embase, and Web of Science were searched from their inception up until 31 July 2025. Randomised Phase 3 clinical trials of biologic therapies approv

Ultrasound-guided Percutaneous Galvanic Current Compared to Ultrasound-guided Intralesional Corticosteroid Infiltration in Tunnels of Hidradenitis Suppurativa.

BACKGROUND: Hidradenitis suppurativa (HS) is a chronic inflammatory condition primarily affecting pilosebaceous units in areas with apocrine sweat glands. Although treatment options vary, intralesional therapies are often necessary, even under systemic treatment, but have limitations such as variable efficacy and inability to eliminate persistent tunnels. Percutaneous galvanic current may constitute an alternative intralesional therapy in HS. OBJECTIVE: This study aimed to compare the efficacy and safety of ultrasound-guided percutaneous galvanic current (GC) with ultrasound-guided intralesio

The youngest male breast clear cell hidradenoma case: a literature review.

BACKGROUND: Clear cell hidradenoma (CCH) of the breast is an exceedingly rare benign adnexal tumor. Its clinical and radiological presentation can closely mimic malignancy, posing a significant diagnostic challenge. CASE PRESENTATION: We report a case of a 6-year-old boy presenting with a non-healing, ulcerated nodule on the left nipple-areolar complex. Ultrasonography revealed a lobulated, cystic nodule. An ultrasound-guided core needle biopsy was performed, revealing epithelial cells with clear cytoplasm, warranting excision. The lesion was completely excised. Histopathology confirmed CCH, b

Semaglutide in patients with hidradenitis suppurativa and obesity.

BACKGROUND: Hidradenitis suppurativa (HS) is a chronic inflammatory dermatosis strongly associated with obesity and metabolic dysfunction. GLP-1 receptor agonists (GLP-1 RAs), such as semaglutide, promote weight loss and exhibit anti-inflammatory properties, but evidence regarding their effects in HS remains limited. OBJECTIVE: To evaluate the effects of 6 months of semaglutide treatment at doses >1mg/week on anthropometric, dermatologic, metabolic, inflammatory, and quality-of-life outcomes in patients with HS and obesity. METHODS: We conducted a prospective, open-label cohort study includi

Topical Ruxolitinib for the Treatment of Hidradenitis Suppurativa.

Hidradenitis suppurativa (HS) is a chronic, recurrent inflammatory skin disease characterized by painful nodules, abscesses, sinus tracts, and scarring, with significant impact on quality of life. Although biologics have expanded treatment options for moderate to severe disease, there are currently no approved therapies specifically indicated for mild HS. Emerging evidence implicates dysregulation of the Janus kinase-signal transducer and activator of transcription (JAK-STAT) pathway in HS pathophysiology, suggesting JAK inhibition as a promising therapeutic strategy. This review summarizes cu

Advances in the Management of Hyperhidrosis.

Hyperhidrosis (HH), a condition characterized by excessive sweating beyond thermoregulation, significantly impairs quality of life. Conventional medications to treat HH include aluminum chloride hexahydrate, systemic anticholinergics, and botulinum toxin injections. New treatments have emerged including glycopyrronium bromide, sofpironium bromide, targeted alkali thermolysis, and microwave devices. This paper reviews the mechanism of action, efficacy, safety, and practical considerations associated with these treatments.

Sweat components as a promising monitoring tool for systemic diseases.

Sweat is a complex biological fluid primarily responsible for thermoregulation, containing diverse organic and inorganic components derived from plasma and interstitial fluids. Its secretion is tightly regulated by hypothalamic-sympathetic pathways and reflects systemic physiological status. Recent studies reveal that sweat composition dynamically changes with various systemic diseases, providing diagnostic, mechanistic, and prognostic insights. Abnormalities in sweat components, such as electrolytes, glucose, lactate, amino acids, and proteins, mirror underlying metabolic, endocrine, immune,
