Spatial heterogeneity in the temperature-hand, foot, and mouth disease association among children: A multicounty time-series study in western China.

While meteorological and socioeconomic factors are well-documented modifiers of spatial heterogeneity in temperature-hand, foot, and mouth disease (HFMD) associations, substantial unexplained heterogeneity remains. This study investigates underexplored environmental modifiers-including extreme temperature events (heat waves and cold spells), air pollution, and normalized difference vegetation index (NDVI)-by analyzing 484,928 HFMD cases among children under six years old in western China (2013-2019) using distributed lag nonlinear models and meta-regression. We found that cold spells (accounte

Risk prediction of encephalitis in hand, foot and mouth disease: a nomogram model based on platelet indices for resource-limited settings.

OBJECTIVE: This study aimed to assess differences in peripheral blood platelet indices including platelet count (PLT), mean platelet volume (MPV), platelet distribution width (PDW), and the platelet-to-lymphocyte ratio (PLR) between children with hand, foot and mouth disease (HFMD) complicated by encephalitis and those with uncomplicated HFMD. A further objective was to develop a robust predictive model for HFMD complicated by encephalitis, specifically designed for resource-limited clinical settings. METHODS: This retrospective study enrolled children with HFMD who were hospitalized at the Ch

A photonic crystal sensing array based on a tandem CRISPR/Cas13a system for ultra-sensitive and high-throughput detection of the CVA6 virus.

Coxsackievirus A6 (CVA6) has emerged as a major cause of hand-foot-mouth disease (HFMD), yet no standardized detection method for it is currently available. Developing a simple, sensitive, and specific CVA6 test is crucial for HFMD control and safeguarding the health of at-risk children. Herein, a photonic crystal (PC) sensing array based on a tandem CRISPR/Cas13a system has been proposed for highly specific and ultra-sensitive analysis of CVA6 RNA, without the need for reverse transcription and amplification procedures. In this strategy, two crRNAs targeting CVA 6 RNA were designed and screen

Mutation of the highly conserved amino acids in the N-terminal region of the EV-A71 VP1 protein impairs viral RNA release during virus entry.

UNLABELLED: Enterovirus A71 (EV-A71) causes hand-foot-mouth disease, which may progress to severe neurological and systemic complications in young children and infants. To deliver viral genomic RNA into the target cell for replication, the mature EV-A71 capsid particle undergoes drastic structural changes to become an expanded A particle, which is marked by the movement of the N-terminal region of viral protein VP1 from the interior to the capsid surface and the dislodgement of the pocket factor. It is not completely understood which amino acids support these key activities of VP1 during EV-A7

Shifting epidemiological patterns and strain replacement of hand, foot, and mouth disease in southern China, 2009-2022: A longitudinal study.

BACKGROUND: China launched a universal campaign of monovalent-inactivated EV-A71 vaccine in 2016. However, the monovalent-inactivated vaccine is ineffective against HFMD with other etiologies. To date, little is known about the epidemiological patterns and the replacement of the predominant pathogens that induce HFMD after the universal vaccination campaign. METHODS: Using HFMD surveillance and VP1 sequencing data in pre- and post-vaccination periods, time series analysis, and Wavelet approaches, we identified pathogen replacement, changes in the viral spectrum and epidemiological patterns of

[Hand-foot-mouth disease with atypical clinical presentation caused by Coxsackievirus A6].

Hand-foot-mouth disease is typically a mild, self-limiting enterovirus infection, most often caused by Coxsackievirus A16 and enterovirus A71. Over the past 15 years, however, Coxsackievirus A6 (CVA6) has emerged worldwide, including in Hungary, as the main pathogen responsible for atypical clinical forms. Atypical hand-foot-mouth disease differs from the classic presentation by widespread vesiculobullous and often painful skin eruptions, frequent perioral lesions and post-infectious nail changes. We present the case of a 2-year-old girl with generalized vesicobullous eruptions, in whom CVA6 (

A Meta-Analysis of Influencing Factors for Reinfection of Hand, Foot and Mouth Disease in China, Based on Adjusted Effect Estimates.

Background: Numerous studies have reported on the epidemiology of hand, foot and mouth disease (HFMD) reinfection and its potential influencing factors; however, findings regarding reinfection rates as well as determinants such as gender, age, residence, and pathogens remain inconsistent. Due to this heterogeneity in reported outcomes, a comprehensive systematic review and meta-analysis are warranted to consolidate existing evidence. Methods: Effect estimates were expressed as reinfection rates, odds ratio (OR)/hazard ratio (HR) and 95% confidence intervals (CI). When necessary, data were conv

Cost-Effectiveness Analysis of a Bivalent Vaccine for Hand, Foot, and Mouth Disease: A Simulation-Based Study in Beijing, China.

BACKGROUND: Hand, foot, and mouth disease (HFMD) remains a major public-health concern in China. While the monovalent EV-A71 vaccine has effectively reduced EV-A71-associated cases, it offers no protection against CV-A16. The introduction of a bivalent EV-A71/CV-A16 vaccine may offer broader protection, but its economic viability under different immunization strategies remains uncertain. METHODS: We developed a dynamic transmission model integrated with cost-effectiveness analysis to assess the epidemiological and economic impact of a hypothetical bivalent EV-A71/CV-A16 vaccine in China. An anti-fouling multifunctional interface enables hierarchical validation and ultrasensitive electrochemical detection of Coxsackievirus A6 specific nucleic acids.

Coxsackievirus A6 (CVA6) has emerged as the dominant etiological agent of hand, foot and mouth disease worldwide. At present, neither specific antivirals nor effective vaccines are available, creating an urgent need for rapid, low-cost in vitro diagnostics that can interrupt transmission. This study developed an electrochemical biosensor based on a "screening-validation" strategy. It utilizes a MoS2/MWCNT/AgNPs three-dimensional highly conductive network as the substrate, tightly anchored with p(NIPAm-co-SBMA) microgel to form a "protective-conductive" synergistic interface. The capture probe

Dysfunction of the plasmacytoid dendritic cells in enterovirus 71-induced HFMD affecting children.

The role of Plasmacytoid Dendritic Cells (pDCs) in Enterovirus-71 (EV71)-induced Hand, Foot, and Mouth Disease (HFMD) remains unclear. In this study, we investigated pDCs in severe HFMD patients infected with EV71. Flow cytometry revealed reduced pDC percentage in severe patients compared to healthy controls. In vitro, as EV71 replicated in pDCs, live cell numbers and levels of Interferon-alpha (IFN-a) and Interleukin (IL)-6 decreased, while IL-10 and Tumour Necrosis Factor-alpha (TNF-a) increased, particularly in critically ill patients. Notably, pDCs derived from critically ill patients exhi

Long-term protection of an inactivated enterovirus type 71 vaccine against hand, foot, and mouth diseases in children: a modelling study.

OBJECTIVES: Despite the availability of enterovirus type 71 (EV71) vaccines, evidence on long-term protection remains limited. We aimed to establish a mathematical model for predicting the 20-year long-term protection of the EV71 vaccine. METHODS: We utilized data from a phase 3 trial in which infants and young children were randomized to receive either two doses of 400 U EV71 vaccine or placebo. Neutralizing antibody data (NTAb) were obtained from the immunogenicity subpopulation who were followed for 5 years. We compared candidate decay models using Akaike Information Criterion (AIC) and Ba

Rapid and Visual Detection of Coxsackievirus A6 Using Reverse Transcription Multiple Cross Displacement Amplification Combined with Nanoparticle-Based Biosensor Platform.

Coxsackievirus A6 (CVA6) is the most common causative pathogen of hand, foot, and mouth disease (HFMD) in children under 5 years of age and has caused multiple outbreaks in recent years. Currently, no effective vaccines or antiviral treatments are available. The present study introduces a detection assay, termed CVA6-RT-MCDA-LFB, which combines reverse transcription multiple cross displacement amplification (RT-MCDA) with a nanoparticle-based lateral flow biosensor (LFB). Ten specific primers targeting the VP1 gene region were designed for CVA6-RT-MCDA. The assay demonstrated an analytical sen

Structural tuning of bridging linkers on M13 phages for high-affinity detection of enterovirus A71.

Enterovirus A71 (EV-A71), the principal etiological agent of hand, foot, and mouth disease, poses a serious public health concern because infection can induce severe and potentially fatal neurological complications through direct invasion and inflammation of the central nervous system. Current detection methods lack rapid and sensitive analytical capabilities, particularly in the context of wastewater surveillance. This study reports a PEG4-biotin-functionalized M13 bacteriophage engineered as a molecular receptor for the colorimetric detection of EV-A71. VP1-specific phage ligands were identi

A randomised trial of a bioreactor-produced EV-A71 vaccine for endemic control in children.

Enterovirus (EV) A71 causes endemic outbreaks of hand-foot-mouth disease (HFMD) and herpangina in young children, occasionally leading to severe complications and fatal outcomes. We evaluated EnVAX-A71, a bioreactor-produced, aluminum-adjuvanted inactivated EV-A71 (B4/E59) vaccine, in a randomized, double-blind, placebo-controlled phase 3 trial in Taiwan and Vietnam. A total of 4011 children aged 2-71 months were randomized 3:2 to vaccine or placebo; the primary endpoint was laboratory-confirmed EV-A71-associated HFMD/herpangina, evaluated from 28 days after dose two over a follow-up of 1 to 2

Real-time dynamic prediction of HFMD transmission using SEIRQ-ARIMA hybrid model optimized by multi-stage ABC-GWO algorithm.

In Hand, Foot, and Mouth Disease (HFMD) control, conventional SEIR/SIR models rely on static parameters, which limits their adaptability to dynamic real‑world conditions. To address this, we propose a SEIRQ–ARIMA hybrid model that integrates a quarantine‑enhanced SEIR framework (SEIRQ) with an ARIMA time‑series model, whose dynamic parameters are optimized via a multi‑stage Artificial Bee Colony–Grey Wolf Optimization (ABC–GWO) algorithm. This study contributes three key novelties to the field: (i) an IoT-driven dynamic SEIRQ parameterization with explicit intervention tracking via time-varyin
