Epidemiological shifts in coxsackievirus A-induced hand, foot, and mouth disease in China, 2008-2024.

INTRODUCTION: Hand, foot, and mouth disease (HFMD) is primarily caused by enterovirus A71 (EV-A71) and coxsackievirus A (CV-A) serotypes. The introduction of the inactivated EV-A71 vaccine in China in 2016 significantly reduced EV-A71-related severe and fatal HFMD cases but did not confer cross-protection against other enteroviruses. Epidemiological data from China show that while the overall incidence of HFMD has declined since 2018, there has been a significant shift in the pathogen spectrum, with non-EV-A71 enteroviruses particularly CV-A6 and CV-A10 emerging as the predominant serotypes.

Molecular epidemiological and genetic characteristics of coxsackievirus A6 circulating in mainland China from 1992 to 2023.

Hand, foot, and mouth disease (HFMD) is a common global infectious disease in children. Coxsackievirus A6 (CVA6) has emerged as the most important pathogen of HFMD. Molecular epidemiological and evolutionary surveillance of CVA6 is vital for the prevention and control of HFMD outbreaks. This study comprehensively analyzed the molecular epidemiology and genetic characteristics of CVA6 circulating in mainland China during 1992-2023. Phylogenetic analysis based on VP1 sequences revealed that in addition to the previously reported subgenotypes B1, B2, C1, D2, D3a, and D3b, genotype A was first ide

Systematic Proteomic Characterization of EV-A71-Infected Mice Identifies Dynamic Molecular Changes and Therapeutic Targets.

Enterovirus A71 (EV-A71) is recognized as the primary causative agent of hand, foot, and mouth disease (HFMD) and is prevalent worldwide. However, the precise pathogenic mechanisms of EV-A71 remain unclear, and specific drugs targeting it have yet to be successfully developed. To explore the mechanisms underlying EV-A71 pathogenesis and to identify potential therapeutic opportunities, we performed a comprehensive proteogenomic characterization of muscle tissues from BALB/c mice infected with EV-A71, integrating transcriptomic, proteomic, and phosphoproteomic analyses. Our results showed that p

Clinical Utility of Small Extracellular Vesicles as Liquid Biopsy for Oral Mucosal Disease Diagnostics: Emerging Perspectives.

Some diseases affecting the oral mucosa can be life-threatening and/or associated with life-threatening complications. Conventional diagnostic methods for most oral mucosal diseases are usually employed at a fully established disease state. All these peculiarities usually result in late diagnosis, poor prognosis, poor treatment outcomes, and reduced overall survival rates, hence the need for novel methods for the early detection of these disease conditions. Small extracellular vesicle (sEV)-based diagnosis carries great potential for early diagnosis of oral mucosal diseases, as sEVs reflect th

Exploration of the expression and regulatory mechanisms of platelet miR-223-3p in hand, foot, and mouth disease (HFMD) complicated by encephalitis.

BACKGROUND: Hand, foot, and mouth disease (HFMD) complicated by encephalitis is a severe condition with an acute onset, yet its pathogenesis remains incompletely understood. This study aimed to investigate the role of platelet miR-223-3p in HFMD complicated by encephalitis. We sought to determine its expression dynamics, verify its direct target F-box and WD repeat domain containing 7 (FBXW7), and elucidate its functional impact on neuronal apoptosis, thereby exploring a novel microRNA (miRNA) mediated mechanism in the pathogenesis of this severe complication. METHODS: We initially enrolled 2

The burden of hand, foot, and mouth disease attributable to climate variability: A nationwide time-series modeling study in Japan.

Substantial evidence suggests that meteorological variables are associated with hand, foot, and mouth disease (HFMD) incidence; however, geographic variation in these associations and in the corresponding attributable burden remains insufficiently characterized. We conducted a nationwide time-series analysis to quantify the burden of HFMD attributable to non-optimal ambient temperature and relative humidity in Japan, while examining between-prefecture variation in the estimated associations. For each of the 47 prefectures (2000-2019), exposure-lag-response functions for temperature and humidit

Viral shedding dynamics in coxsackievirus A6 hand, foot, and mouth disease: a prospective, individual-level analysis.

BACKGROUND: Coxsackievirus A6 (CVA6) has emerged as the predominant cause of hand, foot, and mouth disease (HFMD) in China since the introduction of enterovirus A71 (EV-A71) vaccination. A precise definition of the infectious period is critical for effective containment; however, data on the duration and kinetics of CVA6 shedding remain scarce. We therefore aimed to characterize the viral shedding dynamics of CVA6 and to identify the determinants of viral clearance. METHODS: We conducted a prospective longitudinal study enrolling pediatric inpatients with laboratory-confirmed CVA6 HFMD betwee

Historical reconstruction of the earliest enterovirus A71 epidemics in Japan in the 1960s.

Enterovirus A71 was first isolated in California in 1969, with the earliest retrospective detection traced back to 1963 in the Netherlands, but its early spread remains unclear. Using age-specific seroprevalence data from children aged 1-10 years in Kawasaki City, Japan, collected annually from 1966-1973, we applied serocatalytic models to estimate annual force of infection during 1959-1973. Several models were tested, incorporating different assumptions about time-varying force of infection, age-dependent susceptibility, and seroreversion, to identify the best fit to the data. Model compariso

Emergence of Echovirus 11 in Severe and Neonatal Enterovirus Infections: A 9-Year Retrospective Study in Taiwan Before and After the COVID-19 Pandemic.

Enteroviruses (EVs) cause a wide range of pediatric illnesses. In Taiwan, changes in serotype distribution and COVID-19-related public health measures may have influenced recent EV trends. We retrospectively reviewed EV-positive pediatric inpatients (< 18 years) at National Taiwan University Hospital from 2015 to 2023. Cases were identified via PCR or viral culture. Among 678 cases, incidence declined markedly during the COVID-19 period but rebounded after public health restrictions were relaxed, with seasonal peaks in June persisting. Echovirus 11 emerged as the predominant serotype among neo

A Rapid and Ultrasensitive Detection of Coxsackievirus A16 Using Reverse Transcription Multiple Cross Displacement Amplification Combined with the CRISPR-Cas12a-Based Biosensing System.

Coxsackievirus A16 (CVA16) is one of the primary viral etiological agents of hand, foot, and mouth disease (HFMD) in infants and children under five years of age. Prompt and reliable detection of CVA16 is crucial for guiding immediate therapeutic interventions and for implementing effective epidemic prevention and control strategies, particularly in settings with limited resources. Herein, a diagnostic platform for CVA16 (CVA16-RT-MCDA-CRISPR) was developed by combining reverse transcription multiple cross displacement amplification (RT-MCDA) with CRISPR-Cas12a-based detection. In this system,

A single residue in the VP3 capsid protein governs virulence and informs live-attenuated vaccine design for coxsackievirus A6.

UNLABELLED: Coxsackievirus A6 (CVA6) is a major pathogen of hand, foot, and mouth disease, yet the viral determinants of its severe pathogenicity remain undefined, hampering intervention strategies. Here, by comparing a highly lethal clinical isolate (CVA6-HeB) with an attenuated strain (CVA6-TW141), we identified the capsid-encoding P1 region as the primary virulence determinant in neonatal mice. Fine-mapping identified a single critical residue at position 238 of VP3 (VP3-238): glutamic acid (E) in virulent CVA6-HeB versus alanine (A) in attenuated CVA6-TW141. Introducing E238 into the atten

Advances in Antiviral Drug Development Targeting Enteroviruses: From Viral Proteins to Host Factors.

Enteroviruses represent important human pathogens, posing a substantial disease burden, particularly in children under 5 years of age. Enteroviruses are the primary causative agents of hand-foot-and-mouth disease (HFMD) and are strongly associated with acute flaccid myelitis (AFM), with severe cases potentially resulting in significant neurological complications. Inactivated vaccines against EV-A71 based on the C4 genotype are currently available. However, there are no licensed direct antiviral agents for severe cases. By focusing on viral proteins and host factors, researchers have made great
