BACKGROUND: Genistein, an isoflavone abundant in soybeans and other legumes, has shown antiviral activity against several viruses. Its effects on enteroviruses, particularly EV71-the causative agent of hand, foot, and mouth disease (HFMD)-remain incompletely understood.
METHODS: In vitro assays were used to evaluate replication inhibition of EV71, assess mechanistic effects, and determine the compound's broad-spectrum activity against other enteroviruses (EV68 and CA6). In vivo experiments in neonatal mice were conducted to further elucidate the role of genistein in EV71 replication.

Enterovirus A71 (EV-A71), the primary causative agent of hand, foot, and mouth disease (HFMD), can cause severe neurological complications and even death, particularly in young children. Despite the availability of inactivated vaccines, their protective efficacy has been compromised due to frequent intra- and intertypic recombination events and ongoing mutations among circulating EV-A71 strains. To address this, we employed immunoinformatic approaches and identified conserved epitopes and constructed a multi-epitope vaccine (MEV) candidate against EV-A71. A total of 1,627 structural protein se

Hand, foot, and mouth disease (HFMD) is a common childhood infection caused by enteroviruses, which exhibit distinct regional and seasonal epidemiological patterns. Wastewater-based epidemiology is a crucial tool for monitoring population infection dynamics and viral subtype distribution. However, the lack of effective on-site viral detection methods limits timely early warning and effective surveillance of infectious disease outbreaks. This study developed a one-pot RT-RPA/CRISPR-Cas12a assay-based, string-powered flywheel microfluidic chip for the multiplex detection of HFMD viruses in waste

INTRODUCTION: Hand, foot and mouth disease (HFMD) is influenced by meteorological and air pollution factors. However, few studies have evaluated their joint and interactive effects under real-world multi-exposure conditions. This study examined the health impacts of environmental mixtures and identified susceptible populations in western China.
METHODS: A total of 484 928 HFMD cases in children under 6 years old were collected from western China. Bayesian kernel machine regression was used to estimate the effects of simultaneous exposure to temperature, precipitation, shortwave radiation, pre

BACKGROUND: Enterovirus 71 (EV-A71), a neurotropic member of the Picornaviridae family, is a positive-strand RNA virus comprising of seven recognized genogroups (A-G) and has caused significant outbreaks of hand, foot, and mouth disease worldwide. Despite prior identification of D and G genogroups in India, comprehensive full-genome characterization of these indigenous lineages remains unreported.
METHODS: We performed next-generation sequencing (NGS) and molecular phylogenetic analyses on two Indian isolates from acute flaccid paralysis cases: R13223-IND-02 (genogroup D) and V11-2209-01 (gen

BACKGROUND: Due to insufficient routine surveillance, the nationwide disease burden of hand, foot and mouth disease (HFMD) caused by coxsackievirus A6 (CVA6), an emerging serotype, in China remains unclear. This study aimed to estimate the incidence of CVA6-associated HFMD across the Chinese mainland.
METHODS: CVA6 positive data from 511 locations across the Chinese mainland during 2008-2022 were integrated from the national pathogen surveillance system and literature, and reported HFMD cases during the same period were obtained from the national infectious disease surveillance system. The pr

Hand-foot-mouth disease (HFMD), mainly caused by Enterovirus 71 (EV71), Coxsackievirus A6 (CVA6), and Coxsackievirus A16 (CVA16), poses a serious threat to children due to its high transmissibility and potential to induce severe complications. Herein, a spatially encoded fluorescence biosensing platform that integrates a parallel entropy-driven circuit (EDC) with a combinatorial photonic crystal (cPC) array was reported for ultrasensitive, multiplex, and rapid detection of HFMD viruses. In this system, three parallel entropy-driven circuits were designed to specifically recognize the VP1 gene

BACKGROUND: Coxsackievirus A6 (CVA6) has emerged as a major etiological agent of hand, foot, and mouth disease (HFMD), yet no commercial vaccine is available against CVA6. CVA6 exhibits poor adaptability to Vero cells, a WHO-approved substrate for human vaccine production, and the mechanisms underlying its infectivity and pathogenesis remain incompletely understood.
METHODS: A Vero cell-adapted CVA6 strain was generated through serial passaging in vitro. Two recombinant viruses derived from the earlier and later generations, corresponding to passage 10 and passage 45, were generated and desig

BACKGROUND: Coxsackievirus A5 (CVA5) is an emerging pathogen associated with severe hand, foot, and mouth disease. Protective antibodies are critical for protection from CVA5 infection, yet the antigenic determinants of CVA5 remain poorly defined. In this study, we aimed to map the conformational neutralizing epitopes of CVA5 using monoclonal antibodies (mAbs) and escape mutant analysis.
METHODS: Two IgG, one IgA, and five IgM neutralizing mAbs were purified and characterized for binding affinity and neutralization potency. Antibody pressure selection was applied to generate immune escape mut

Hand, foot and mouth disease (HFMD) constitutes a global public health concern. Internet search data offers advantages including vast data volumes, provision of real-time information, and the potential for earlier infectious disease surveillance. The objective of this study is to utilise Baidu Search Index (BSI) to construct a predictive model for HFMD epidemiological surveillance, thereby enhancing HFMD incidence forecasting and early warning capabilities. HFMD cases reported by the National Health Commission of the People's Republic of China from January 2011 to March 2025 were collected. Ke

UNLABELLED: Enterovirus 71 (EV71) is one of the major pathogens responsible for hand-foot-and-mouth disease in children and is associated with severe neurological complications, including death. Despite the fact that EV71 naturally infects humans through the oral route, the absence of a simple and reliable oral infection model has significantly impeded investigations into its pathogenesis and the development of antiviral interventions. In this study, we established a robust oral EV71 infection model in widely used immunocompetent BALB/c mice, employing a clinical isolate devoid of adaptive mut

Coxsackievirus A4 (CVA4) is an enterovirus associated with diverse clinical syndromes, yet its global evolution and transmission dynamics remain poorly defined. This study aimed to characterize the molecular epidemiology, evolutionary history, and phylodynamic patterns of CVA4 at both local and global scales. In 2024, 34 CVA4 infections were identified during a localized herpangina outbreak in Beijing, primarily affecting children under 5 years of age, with a pronounced peak in July. High detection and isolation rates, together with temporal clustering in mid-2024, indicate a genuine short-ter

BACKGROUND: As a major etiological agent of hand, foot, and mouth disease in humans, Coxsackievirus A16 (CVA16) exerts a detrimental impact on the health of infants and toddlers. In the absence of effective antiviral drugs and vaccines targeting CVA16, a diagnostic strategy with sensitivity, specificity and rapid response is indispensable for the prevention and management of CVA16 infection.
RESULTS: In this study, a novel molecular diagnostic approach, coupling real-time fluorescence technique with reverse transcription multiple cross-displacement amplification (RT-MCDA), was developed for t

Molecular typing of enteroviruses (EVs) is essential for surveillance of hand, foot, and mouth disease (HFMD). Conventional reverse-transcription polymerase chain reaction (RT-PCR) targeting the VP4-VP2 region can be insufficiently sensitive, reducing the detectability of Enterovirus A (EV-A). We developed a single-round RT-PCR assay using a modified reverse primer design (C3R) for rapid EV detection and genotyping. Sensitivity was evaluated using EV-A71 and poliovirus type 1 reference strains, across 60 EV-positive clinical specimens. The C3R-based assay showed ~1000-fold higher sensitivity f

OBJECTIVE: Hand, foot, and mouth disease (HFMD) transmission is sensitive to temperature-humidity interactions; however, the role of wind speed in modifying these effects remains unknown. This study investigated how wind speed modifies the combined effects of temperature and humidity on HFMD burden and identified subgroups of individuals with increased vulnerability to these climate exposures.
METHODS: We analyzed data from 524,100 HFMD cases and daily meteorological measurements across Guizhou, China, between 2012 and 2019. Disease burden was quantified as the number of years lived with disa

Hand, foot, and mouth disease (HFMD) is primarily caused by Enterovirus 71 (EV71) and Coxsackievirus A16 (CA16), while excessive inflammatory responses induced by viral infection are the main cause of severe conditions. Hence, it is important to develop agents with both antiviral and anti-inflammatory activities for clinical treatment. In this study, we synthesized 20 compounds based on the hybridization of nonsteroidal anti-inflammatory drugs (NSAIDs) and organoselenium and found that YSN-1-167 exhibited significant anti-EV71 function using an EV71-GFP infection system. Mechanistic studies sh

BACKGROUND: Coxsackievirus A5 (CV-A5) has re-emerged as an important pathogen associated with hand, foot, and mouth disease (HFMD) in young children. However, the antigenic structure of CV-A5 and the key determinants responsible for neutralizing antibody responses remain largely undefined, hindering rational vaccine design and antibody-based intervention strategies.
METHODS: To investigate the relationship between viral particle composition and humoral immune responses, three forms of CV-A5 particles, including empty particles (EPs), full particles (FPs), A-particles (APs) were purified from

Enterovirus A71 (EV-A71) is the primary pathogen causing severe hand-foot-and-mouth disease (HFMD) in young children, with T-cell immune dysfunction closely linked to severe clinical outcomes. However, the molecular mechanisms underlying EV-A71-mediated T-cell impairment remain unclear, and no specific therapies are currently available. Here, we investigated the interaction between EV-A71 and T cells, and explored potential targeted therapeutic strategies. Our results showed that EV-A71 efficiently infects T cell lines (Jurkat, EL-4) and primary mouse CD3+ T cells in a dose- and time-dependent
