Onychomadesis: A Rare Manifestation of Coxsackievirus A6 Infection in an Adult Patient.

Hand, foot, and mouth disease (HFMD) is a self-limiting viral illness predominantly affecting children under 10 years of age, with peak incidence in those aged one to four years. Recent outbreaks of Coxsackievirus A6 (CVA6) have increased its incidence across all age groups, including adults. Onychomadesis, a proximal nail plate detachment due to transient matrix arrest, is a recognized but underreported delayed complication of HFMD. We report a case of a 60-year-old man who developed onychomadesis three weeks after clinical recovery from CVA6-induced HFMD. Initial symptoms included fever, sor

Targeted sequence capture of coxsackievirus A6 using nanopore sequencing directly from clinical specimens.

Coxsackievirus A6 (CVA6) has emerged as a predominant causative agent of hand, foot, and mouth disease worldwide. Efficient genomic surveillance is crucial for understanding its evolution and spread. Nanopore sequencing presents a promising platform for rapid and cost-effective viral genome sequencing directly from clinical samples. We developed a tiling amplicon-based nanopore sequencing protocol for CVA6. Eight clinical CVA6-positive samples, spanning a wide range of Ct values (15.54-25.28), were each serially diluted to three concentrations (undiluted, 10⁻¹, and 10⁻²), generating a total of

Moracin M inhibits EV71 infection primarily by blocking viral attachment to host cells.

The global outbreak of hand, foot, and mouth disease (HFMD), mainly caused by enterovirus A71 (EV71) and coxsackievirus A16 (CA16) of the genus enterovirus, poses a serious threat to the health of young children, and severe neurological manifestations and associated symptoms are primarily attributed to EV71 infection. There is currently no specific antiviral drug available for HFMD. In this study, we identified Moracin M (MM), a flavonoid compound derived from Smilax china, as a potent inhibitor of EV71 and CA16 infection. Mechanistic investigation revealed that MM exerts its antiviral effects

Dairy Worker Infection Prevention Knowledge, Beliefs, and Behaviors.

INTRODUCTION: Recent outbreaks of infectious diseases on dairy farms, with workers becoming ill, have focused attention on the potential for zoonotic disease transmission of pathogens between animals and humans in dairy production. While many dairy farms have a biosecurity plan and protocols in place to reduce the possibility of cow-cow transmission of pathogens such as foot-and-mouth disease, previous surveys have indicated that few farms have organized ongoing infection‑prevention programs to prevent the spread of infection to workers. OBJECTIVE: The purpose of the current study is to descr

Impact of EV-A71 vaccination and non-pharmaceutical interventions on hand, foot, and mouth disease: A 14-year interrupted time series analysis.

BACKGROUND: Hand, foot, and mouth disease (HFMD), particularly EV-A71, has caused large-scale epidemics in the Asia-Pacific region, with China alone reporting over 2 million annual cases and causing a substantial morbidity and mortality. While EV-A71 vaccination was introduced in 2016 and COVID-19 non-pharmaceutical interventions (NPIs) were implemented from 2020-2022, comprehensive evidence on their independent and combined effects on HFMD epidemiology remains scarce. We aimed to quantify the impact of vaccination, NPIs, and post-pandemic policy relaxation on HFMD incidence and pathogen distr

Differential immune responses elicited by two tetravalent epitope-based DNA vaccines differently designed for human hand-foot-and-mouth disease.

Hand-foot-and-mouth disease (HFMD) remains a significant public health concern for children. Enterovirus A71 (EV71) and coxsackievirus A16 (CVA16) have been the main causes of HFMD; however, CVA10 and CVA6 have become more prevalent in recent years. A universally effective vaccine that covers all four major enteroviruses is of importance. In this study, we developed and assessed the immunogenicity of two differently designed tetravalent DNA-based HFMD vaccines in a murine model. The new immunogen, EVCVAme, was designed with a string of reported B-cell epitopes from the four enteroviruses, whil

TLR2-Driven NLRP3 Inflammasome Activation Mediates EV71-Induced Neuropathogenesis in Neonatal Mice.

EV71 is a leading cause of HFMD-associated neurological complications, yet its CNS pathogenic mechanisms remain unclear. Using neonatal BALB/c mice, we identified 10 days post-infection (dpi) as the disease peak, characterized by neuronal necrosis and robust NLRP3 inflammasome activation. We demonstrate that EV71 upregulates TLR2, triggering the NF-κB pathway to prime and activate the NLRP3 inflammasome within microglia. This process drives pro-inflammatory cytokine release and neuroinflammatory damage. Pharmacological blockade of TLR2 with C29 suppressed NF-κB phosphorylation and NLRP3 assemb

Asyndromic surveillance of New York City emergency department diagnoses with the tree-temporal scan statistic.

OBJECTIVES: Illness trends are typically monitored by reportable disease and syndromic surveillance systems, but unanticipated health issues might not be captured. Using diagnosis codes, the New York City Health Department developed a novel data mining process to detect unusual increases in emergency department (ED) visits for any reason. METHODS: We applied the tree-temporal scan statistic in TreeScan software to ICD-10-CM diagnosis codes for ED visits. We searched for unusual citywide increases in ED visits or hospital admissions, over any recent time period, and at any part of and level on

Molecular characterization and whole-genome sequencing-based genetic evolution of coxsackievirus A16 in Taiyuan City, 2023-2025.

Coxsackievirus A16 (CVA16) is a major causative agent of hand, foot, and mouth disease (HFMD) in China. Based on 35 CVA16-positive strains isolated in Taiyuan (2023-2025), we characterized the post-pandemic evolutionary dynamics. Genomic analysis revealed the highest conservation in VP4 and the lowest in the 3' UTR. Temporally, a shift from B1a/B1b co-circulation to a three-lineage pattern (including a novel B1c subgenotype) was observed. Intertypic recombination was the primary driver, specifically targeting the 5'UTR and VP1/2A junction (B1a with EV71; B1b with CVA4; B1c with both). Cruciall

Foot-and-mouth disease virus regulates glycolysis through autophagy to drive viral replication in vivo and in vitro.

Viruses, lacking an intrinsic metabolic network, exploit host cell metabolism in order to hijack resources for their own replication. However, the regulatory relationship between foot-and-mouth disease virus (FMDV) and energy metabolism remains incompletely elucidated. Here, metabolomic analysis was performed on PK-15 cells and tonsils infected with FMDV. Metabolites involved in glycolysis were significantly upregulated, whereas metabolites associated with the tricarboxylic acid (TCA) cycle were markedly downregulated in vivo and in vitro, which indicated that FMDV induced reprogramming of hos

Myricetin prevents lethal enterovirus infection by broadly targeting a conserved surface-exposed loops on the VP1 capsid.

BACKGROUND: Enteroviruses impose a significant global health burden, causing outbreaks of Hand, Foot, and Mouth Disease (HFMD) and severe neurological complications. Currently, no specific and broadly effective antiviral therapeutics are available due to the rapid mutation and co-circulation of diverse viral strains. The viral capsid protein VP1 is critical for viral integrity and host cell entry, making it an attractive target for drug development. Myricetin (MC) is a highly safe, naturally occurring flavonol widely distributed in medicinal plants (e.g., Myrica rubra) with well-documented eth

Atypical Hand, Foot, and Mouth Disease in an Immunocompetent Adult.

Hand, foot, and mouth disease (HFMD) is a common viral illness of childhood but is increasingly recognized in adults with often atypical and severe presentations. We report a case of HFMD in an immunocompetent man in his sixties who presented with a generalized, tender vesiculopapular eruption involving the groin, trunk, extremities, palms, and soles. An extensive infectious evaluation was initially pursued, and empiric broad-spectrum antimicrobials were administered. Dermatology consultation led to the diagnosis of HFMD, and antibiotics were discontinued with subsequent clinical improvement.
