How much sweating is enough? Predicting thermoregulatory capacity in patients with ectodermal dysplasia based on a minimal pilocarpine-induced sweat volume.

BACKGROUND: Pathogenic EDA gene variants underlying X-linked hypohidrotic ectodermal dysplasia cause either anhidrosis (null mutations) or hypohidrosis (hypomorphic mutations). Due to their inability to sweat, heat-stressed individuals with EDA null mutations are unable to regulate their body temperature without external cooling and are thus vulnerable to heat-induced injury. For the purpose of developing a therapeutic intervention it would be necessary to know what sweat production is sufficient for normal thermoregulation, e.g., by correlating perspiration in a standard test with thermoregul

Familial ROSAH Syndrome Associated with the ALPK1 p.Thr237Ala Variant: Divergent Ocular Phenotypes and Partial Response to IL-6 Blockade.

PURPOSE: To report phenotypic variability and treatment response in familial ROSAH syndrome associated with the ALPK1 p.Thr237Ala variant. METHODS: Longitudinal observational familial case series of a mother-daughter pair with ALPK1-associated ROSAH syndrome. The daughter carried a heterozygous ALPK1 p.Thr237Ala variant, whereas her mother showed an apparent homozygous genotype. Both underwent multimodal ophthalmic imaging, visual field testing, systemic assessment, genetic analysis, and 24 months of follow-up, including 17 months after starting tocilizumab. RESULTS: The daughter presented w

Segmental Pallor and Hair Loss in a Patient With Ross Syndrome: Case Report.

A 30-year-old male sought medical consultation due to sudden anisocoria. Clinical examination confirmed Adie's tonic pupil. Subsequently, the presence of segmental anhidrosis with compensatory hyperhidrosis and areflexia established the diagnosis of Ross syndrome. In a 15-year follow-up, uncommon manifestations developed in the facial territory affected by compensatory hyperhidrosis: unilateral facial pallor and alopecia following a midline-defined pattern. This case emphasizes the importance of recognizing neurocutaneous manifestations in dermatology practice and suggests that Ross syndrome m

A rare complication of vagus nerve stimulation surgery: the Horner Syndrome. Case report and systematic review.

Vagus nerve stimulation (VNS) is a therapy used for drug-resistant epilepsy, refractory status epilepticus and treatment-resistant depression. VNS can reduce seizure severity and frequency, improving quality of life. It is generally well tolerated, with rare complications, which can be early (surgical) or late (device-related or stimulation-related). Common side effects include vocal cord issues, cough, and infections. We present Horner syndrome as a rare complication of VNS caused by damage to the sympathetic innervation of the ipsilateral eye and characterized by ptosis, miosis and rarely an

Horner syndrome after ethanol ablation for radioactive iodine therapy refractory papillary thyroid carcinoma.

Papillary thyroid carcinoma (PTC) is the most prevalent form of thyroid malignancy. Although it carries a favorable prognosis, especially for individuals under the age of 45, tumor recurrences in the thyroid bed still occur. In fact, recurrence rates are observed in approximately 20% of patients following initial treatment. For patients who are not candidates for surgery or radioactive iodine therapy, ethanol ablation is a treatment option for recurrences in the thyroid bed and lymph node metastases. This case report details a 31-year-old female patient with a PTC who underwent total thyroidec

Raeder's paratrigeminal neuralgia secondary to internal carotid artery dissection mimicking trigeminal autonomic cephalalgia: a case report.

Raeder's paratrigeminal neuralgia is a rare clinical syndrome characterized by ipsilateral periorbital pain, partial Horner syndrome (ptosis and miosis without anhidrosis), and sensory disturbances in the ophthalmic division (V1) of the trigeminal nerve1. The syndrome results from involvement of sympathetic fibers traveling along the internal carotid artery (ICA), often in association with vascular or parasellar pathologies2. Among its etiologies, ICA dissection is particularly important, as it may present with unilateral headache and cranial autonomic features that closely mimic trigeminal au

Determination of the ED90 and ED99 of Liposomal versus Conventional Bupivacaine for Ultrasound-Guided Stellate Ganglion Block: A Biased-Coin Design Study.

BACKGROUND: Local anesthetics commonly used for ultrasound-guided stellate ganglion block (SGB) have a relatively short duration of action (6-12 hours). Liposomal bupivacaine, utilizing a multivesicular liposome sustained-release technology, can extend the block duration to over 72 hours, offering a potential option for prolonged sympathetic blockade. Determining its optimal effective dose relative to conventional bupivacaine is crucial for balancing prolonged efficacy with safety. METHODS: All patients received ultrasound-guided SGB, successful blockade was explicitly defined as the onset of

Brown-Séquard syndrome caused by cervical disc herniation with associated Horner's syndrome: a case report.

Brown-Séquard Syndrome (BSS) is a rare incomplete spinal cord injury. Its association with Horner syndrome (HS) secondary to cervical disc herniation is exceptionally rare, presenting a significant diagnostic and therapeutic challenge. A 37-year-old man presented with neck pain, right shoulder pain, and progressive right upper limb motor weakness. The patient also exhibited gait disturbances and ipsilateral facial signs of HS, including ptosis, miosis, and anhidrosis. Cervical magnetic resonance imaging revealed a large right-sided disc extrusion at the C3-4 level, which resulted in severe spi

Persistent Horner Syndrome Following Benign Gynaecological Surgery: A Case Report and Review of the Literature.

Horner syndrome is an uncommon neurological disorder resulting from the disruption of the oculosympathetic pathway and classically presents with ipsilateral ptosis, miosis and, variably, facial anhidrosis. Although recognised following cervical, thoracic and vascular procedures, it is rarely reported following benign gynaecological surgery. We present the case of a 44-year-old woman who developed persistent right-sided Horner syndrome following elective subtotal abdominal hysterectomy with bilateral salpingectomy performed under general anaesthesia with spinal anaesthesia. Right-sided ptosis d

Horner Syndrome Following an Upper Thoracic Vertebral Column Resection for Congenital Scoliosis: A Case Report.

CASE: A 12-year-old boy with congenital scoliosis underwent posterior spinal fusion from C7 to T7 with a vertebral column resection (VCR) of T3 to T4. Postoperatively, he developed left-sided ptosis and miosis with an unremarkable MRI of the head, cervical, and thoracic spine. Apraclonidine testing confirmed Horner syndrome. The patient required no ophthalmologic intervention and demonstrated continued functional and symptom improvement at 6 months with stable spinal alignment. CONCLUSION: This case highlights Horner syndrome as a rare potential complication of VCRs without epidural analgesia

Whole exome sequencing of paediatric patients with Cogan's syndrome to identify monogenic mimics.

OBJECTIVES: Cogan's syndrome (CS) is a rare variable vessel vasculitis, describing sensorineural hearing loss (SNHL), inflammatory ocular disease and vestibular dysfunction. We hypothesized that within paediatric-onset (p)CS, a proportion would have monogenic disease, either autoinflammatory and/or associated with SNHL. METHODS: Whole exome sequencing (WES) was performed and analysed using an in-house pipeline incorporating virtual gene panels for inflammation and SNHL; copy number variant analysis (ExomeDepth); and phenotype-driven variant prioritization (Exomiser). Genetic variants were int

Japanese intractable and rare dermatologic diseases: From the official skin-centered nanbyo core to an illustrative mechanism-based framework.

Japan's designated intractable disease (nanbyo) framework combines a population threshold with unresolved pathogenesis or treatment, long-term care needs, objective diagnostic criteria, and severity-linked assistance with medical expenses. We conducted a targeted narrative review of official Japanese sources and biomedical literature through August 3, 2026. Evidence ranged from regulatory approvals and randomized trials to observational, case-report, and preclinical data, so therapeutic maturity was assessed separately from mechanistic plausibility. We distinguished the 12 official skin/connec

X-Linked Anhidrotic Ectodermal Dysplasia in A 19-Year-Old Male: A Classic Phenotype.

X-linked anhidrotic/hypohidrotic ectodermal dysplasia (XLHED), also known as Christ-Siemens-Touraine syndrome, is a rare genetic disorder characterized by the abnormal development of ectodermal structures, primarily affecting sweat glands, hair, and teeth. It results from mutations in the Ectodysplasin A (EDA) gene. Clinically, the classic triad of hypohidrosis, hypotrichosis, and hypodontia or anodontia forms the diagnostic hallmark. Herein, we report a case of a 19-year-old male who presented generalized dryness of skin, heat intolerance, and inability to sweat since early childhood. On exam
