Deep Phenotyping and Molecular Elucidation of a New Syndrome: Ectodermal Dysplasia Caused by IRF6 Variants.

The diagnosis of an ectodermal dysplasia (ED) is often made by dermatologists. Some of the more than 50 distinct ectodermal dysplasias, however, are still largely unknown and their pathogenesis is poorly understood. Since we recently discovered that variants of the Interferon Regulatory Factor 6 (IRF6) gene IRF6 may cause ED, we have further investigated the link between this gene and maldevelopment of tissues originating from the embryonic ectoderm. Disease characterization in two previously reported subjects and one newly identified patient (mosaic status) included systematic clinical and ge

Neuro-ophthalmic observation and 16-month follow-up of horner syndrome after thyroidectomy: A case report.

RATIONALE: Horner syndrome (HS) is a rare but underrecognized complication of thyroidectomy, typically resulting from intraoperative injury to the cervical sympathetic chain. Its clinical manifestations include ptosis, miosis, enophthalmos, anhidrosis, and vasodilation. However, data on causative factors, progression of ocular signs, and clinical recovery timing remain limited. This study aimed to clarify these aspects to establish reference intervals for patient management. PATIENT CONCERNS: A 29-year-old female developed postoperative miosis and ptosis following endoscopic thyroidectomy. He

Cellular stress-induced eccrine gland dysfunction as a potential mechanism in acquired idiopathic generalized anhidrosis.

Acquired idiopathic generalized anhidrosis (AIGA) is a rare disorder characterized by systemic anhidrosis or hypohidrosis of unknown etiology. Although autoimmune responses targeting eccrine glands and/or ducts have been proposed as a potential mechanism, the pathophysiology remains largely unclear. Corticosteroid pulse therapy is widely used for treatment, yet its mechanism of action is not fully understood. To elucidate the underlying mechanisms, we analyzed skin lesions from patients with AIGA before and after corticosteroid pulse therapy through histological and single-cell RNA-sequencing

Renal Disease in ROSAH Syndrome Masquerading as Lupus Nephritis: A Case Report and Cohort Analysis.

ROSAH (retinal dystrophy, optic nerve edema, splenomegaly, anhidrosis, and headache) syndrome is a rare, autosomal dominant autoinflammatory disorder caused by gain-of-function mutations in ALPK1. These mutations activate NF-κB signaling and drive development of a multisystemic disease. Beyond the hallmark features captured in the ROSAH acronym, affected individuals may experience recurrent low-grade fevers, deforming arthritis, and gastrointestinal inflammation. Prior to the identification of a unifying genetic cause, patients can be misdiagnosed with a range of immune-mediated conditions, su

Unilateral facial flushing and anhidrosis: a case of idiopathic Harlequin syndrome with successful botulinum toxin treatment.

Dear Editor, Harlequin syndrome is a rare autonomic disorder characterized by unilateral facial flushing and sweating caused by dysfunction of sympathetic innervation. Although generally considered a benign condition, it may occasionally represent the first manifestation of underlying neurological or structural abnormalities, including brainstem lesions, multiple sclerosis, or thoracic spinal cord disorders. Due to its striking clinical appearance, Harlequin syndrome may be misinterpreted as an allergic reaction, chronic urticaria, or other dermatological conditions, potentially delaying diagn

Inositol 1,4,5-Trisphosphate Receptor Mutations Associated with Human Disease: Insights into Receptor Function and Dysfunction.

Inositol 1,4,5-trisphosphate receptors (IP3Rs) are ubiquitous intracellular Ca2+ release channels. Their activation, subcellular localization, abundance, and regulation play major roles in defining the spatiotemporal characteristics of intracellular Ca2+ signals, which are in turn fundamental to the appropriate activation of effectors that control a myriad of cellular events. Over the past decade, ∼100 mutations in ITPRs associated with human diseases have been documented. Mutations have been detailed in all three IP3R subtypes and all functional domains of the protein, resulting in both gain

A case of relapsed gAChR-positive autoimmune autonomic ganglionopathy treated by plasma exchange and mycophenolate mofetil.

Autoimmune autonomic ganglionopathy (AAG) is a rare and acquired immune-mediated disease that leads to wide autonomic failure, mainly characterized by orthostatic hypotension, gastrointestinal dysfunction, anhidrosis and poorly reactive pupils. This disorder is usually associated with autoantibodies to the ganglionic nicotinic acetylcholine receptor (gAChR-Ab). In this study, we describe a case of a gAChR-Ab-positive AAG patient with two therapeutic stages. The patient responded well after the first stage of methylprednisolone pulse therapy and subsequent low-dose prednisone. However, AAG rela

Intermittent Sinus Pause/Asystole in the Setting of Anticholinergic Overdose.

Anticholinergic toxicity typically presents with symptoms of cutaneous vasodilation, delirium, mydriasis, urinary retention, hyperthermia, anhidrosis, and tachycardia. This case report presents a 68-year-old female patient who exhibited some of these signs and symptoms after ingesting an unknown quantity of dicyclomine. However, she displayed one notable exception to the classic toxidrome. On hospital day 2, the patient experienced multiple incidences of prolonged sinus pause, culminating in witnessed asystole lasting 5-10 seconds. The patient continued to have numerous episodes of sinus pause

Topical insulin in pediatric neurotrophic keratopathy associated with CIPA syndrome.

We present a case, with a 2-year follow-up, of the use of topical insulin in managing neurotrophic keratopathy in a 3-year-old boy diagnosed with congenital insensitivity to pain with anhidrosis (CIPA). Initially, the patient had a corneal perforation in the right eye, for which he underwent tectonic amniotic membrane transplantation. On examination, a central corneal ulcer with stromal infiltration was identified in the left eye, accompanied by a complete absence of corneal sensation in both eyes. Systemic features consistent with CIPA, such as the absence of a normal response to pain stimuli

A missense mutation in the KCNE4 gene is not predictive of equine anhidrosis.

Anhidrosis is defined as a decreased or absent ability to sweat in response to heat and exercise. In horses, this condition can increase the risk of life-threatening hyperthermia. A prior study has suggested that equine anhidrosis is associated with a missense variant (rs68643109) in the Potassium Voltage-Gated Channel Subfamily E Regulatory Subunit 4 (KCNE4) gene. This project aimed to validate this association in a population of well-phenotyped horses and to determine the allele frequency of this variant in publicly available whole-genome sequence data. Fifty horses within the University of

Cutaneous manifestations of Fabry disease: A systematic review.

Fabry disease (FD) is a rare X-linked lysosomal storage disorder resulting in potential debilitating accumulation of glycosphingolipids in organs such as skin, nerves, heart, kidneys, lungs, and the central nervous system. Skin is easily investigated and can guide clinicians to diagnose FD, minimizing delay of enzyme substitution therapy. This systematic review followed the PICO and PRISMA guidelines. Using Web of Science, PubMed, and Embase, a total of 968 studies were retrieved by January 1, 2024. All clinical studies describing the skin characteristics and abnormalities of FD patients were
