Congenital insensitivity to pain with anhidrosis.

Congenital Insensitivity to Pain with Anhidrosis (CIPA) is characterised by disruption of sensory neurons caused by genetic mutations in the Neurotrophic Tyrosine Kinase 1 (NTRK1) gene which leads to impaired pain sensation, accompanied by anhidrosis (the inability to sweat) and varying degrees of intellectual disability. Herein, we report a case of CIPA that presented with infection of an amputated left toe, with prior admissions due to various injuries. The patient had no apparent intellectual disability. A general physical examination yielded various scars with a predilection for the elbows

20-Year Efficacy of Endoscopic Thoracic Sympathectomy for Primary Hyperhidrosis: A Cohort Study.

Background: Primary focal hyperhidrosis, characterized by excessive sweating primarily affecting the hands and axillae, can significantly impact quality of life. Bilateral thoracic sympathectomy is the gold standard treatment, providing permanent resolution of palmar hyperhidrosis. Most studies evaluating outcomes and patient satisfaction after sympathectomy focus on short- to medium-term follow-up, typically up to 5 years. This study aimed to assess anxiety, satisfaction, and sweat redistribution 20 years after bilateral endoscopic thoracic sympathectomy. Methods: Between January 2002 and Dec

Congenital Insensitivity to Pain With Anhidrosis: First Reported Case in Nepal.

Congenital insensitivity to pain with anhidrosis is a rare autosomal recessive disorder characterized by anhidrosis, self-mutilation, and insensitivity to pain and temperature. While genetic testing confirms the diagnosis, it is not always feasible, making clinical recognition crucial in resource-limited settings. Early diagnosis and a multidisciplinary approach help prevent complications like severe injuries, infections, and hyperpyrexia.

A rare presentation of Horner's syndrome following cervical epidural steroid injection.

Horner's syndrome is a clinically significant condition caused by disruption of the oculosympathetic nerve pathway, typically presenting with ptosis, miosis, and facial anhidrosis. It can result from serious, life-threatening conditions such as trauma, carotid artery dissection, or malignancy; however, it may also arise from iatrogenic reasons, including stellate ganglion blocks, by other surgical interventions of the head and neck. While exceedingly rare, Horner's syndrome has been reported to occur after a cervical epidural steroid injection. Given the various potential etiologies of Horner'

Improved Outcomes in Congenital Insensitivity to Pain with Anhidrosis (CIPA) via a Multidisciplinary Clinic Model.

Background: Congenital insensitivity to pain with anhidrosis (CIPA) is a rare genetic disorder, often leading to injuries and serious infections. In 2018, we established a multidisciplinary clinic (MDC) to provide structured, proactive care. We assessed the MDC's impact on hospitalizations, surgeries, and infection rates. Methods: A retrospective study of genetically confirmed CIPA patients, treated from 2014 to 2024. Data from electronic medical records were compared between the pre-MDC (2014-2017) and post-MDC (2018-2024) periods. The core MDC team includes an infectious disease specialist,

Horner syndrome following endoscopic thyroid surgery (ETS): a report of two cases and literature review.

Horner syndrome (HS), a rare complication of endoscopic thyroid surgery (ETS), manifests as ptosis, miosis, and anhidrosis resulting from oculosympathetic pathway disruption. This study explores HS etiology through two case reports and literature analysis. Case 1 involved a 43-year-old female who underwent unilateral thyroidectomy via a bilateral areolar approach for a thyroid oncocytic adenoma. On postoperative day 1, ptosis and miosis were observed, and the patient was diagnosed with HS. Despite initial glucocorticoid and neurotrophic therapy, symptoms resolved spontaneously by 6 months. Discovery of a selective alpha-kinase 1 inhibitor for the rare genetic disease ROSAH syndrome.

ROSAH (retinal dystrophy, optic nerve edema, splenomegaly, anhidrosis, and headache) syndrome is a rare genetic disease caused by variants in alpha-kinase 1 (ALPK1) resulting in downstream pro-inflammatory signaling mediated by the TIFA/TRAF6/NF-κB pathway. Here, we report the design of an ALPK1 inhibitor, DF-003, with pharmacokinetic properties suitable for daily oral dosing. In biochemical assays, DF-003 potently inhibits human ALPK1 (IC50 = 1.5 nM) and the ROSAH disease-causing mutant ALPK1[T237M] (IC50 = 16 nM). When tested against a panel of 394 human kinases, DF-003 exhibits ≥860-fold se

Ross syndrome following COVID-19 infection in an 18-year-old Syrian male patient: a case report.

BACKGROUND: Ross syndrome is an exceptionally rare disorder characterized by tonic pupils, areflexia, and anhidrosis, with a prevalence that appears to be higher in women than in men, typically presenting in individuals during their 30s. While Adie Syndrome closely mimics Ross syndrome, Adie syndrome lacks anhidrosis, making Ross syndrome clinically differentiable. This condition can manifest with a variety of symptoms, including cardiovascular anomalies attributable to autonomic nervous system dysfunction. We present the first case in the literature of COVID-19 being a potential trigger for R

Diffusion characteristics and efficacy of letibotulinum toxin a in forehead wrinkle treatment.

BACKGROUND: Facial wrinkles, caused by aging and repetitive muscle contractions, are commonly treated with botulinum neurotoxin type A (BoNT-A). However, excessive toxin diffusion can cause side-effects like muscle weakness. OBJECTIVES: This study aimed to compare the diffusion, efficacy, and safety of letibotulinum toxin A with two other BoNT-A products for treating forehead wrinkles. METHODS: In a double-blind, randomized, split-face controlled trial, 20 participants with moderate-to-severe horizontal forehead wrinkles received letibotulinum toxin A on one side and prabotulinum or onabotul

KCTD1/KCTD15-Mediated Repression of AP-2α/AP-2β Is Required for Proper Skin Appendage Development and Epidermal Homeostasis.

The transcriptional regulation of skin appendage morphogenesis during development is poorly understood. We identified a previously unknown critical role of KCTD1/KCTD15 complexes in keratinocytes for the proper formation of skin appendages. Individuals with dominant-negative mutations in KCTD1/KCTD15 have sparse hair and anhidrosis, and mice lacking KCTD1 and KCTD15 in keratinocytes have diminished sweat glands and sebaceous glands as well as progressive hair sparseness. In this study, we show that these phenotypes are a consequence of the lack of the repressor function of KCTD1/KCTD15 complex

Apical Malignant Pleural Mesothelioma Presenting as Horner's Syndrome: A Case Report.

Horner's syndrome is a well-recognized presentation of apical lung tumors, though rarely associated with malignant pleural mesothelioma (MPM), with only a few cases documented. We report the case of an 82-year-old man with persistent right-sided chest pain who developed unilateral right-sided ptosis, miosis and a sunken appearance to the eye, in the context of known asbestos exposure. Initial chest radiograph was reported as normal; however, subsequent computed tomography revealed an irregular right apical pleural mass measuring 7.0×4.3×7.5 cm. Biopsy with immunohistochemistry confirmed sarcom

Delineating the Adult Phenotype of PGM2L1 -Related Neurodevelopmental Disorder.

PGM2L1 is a crucial enzyme exhibiting glucose 1,6-bisphosphate synthase activity, with predominant expression in brain tissue. In 2021, biallelic pathogenic variants in the PGM2L1 gene were first linked to a neurodevelopmental disorder characterized primarily by developmental delay in four pediatric cases. In this study, we aimed to delineate the adult phenotype associated with the PGM2L1-related neurodevelopmental disorder and to perform functional characterization of the identified variant. Two siblings presenting with neurodevelopmental delay were evaluated clinically and genetically. Exome

Bilateral Carotid Artery Dissection After a Fall: A Case of Horner Syndrome Revealed on Examination.

INTRODUCTION: Carotid artery dissections are uncommon but critical vascular injuries. They involve a tear to the intima, the innermost layer of the arterial wall, leading to formation of a false lumen. This false lumen can disrupt blood flow, weaken the wall, and lead to thrombus or rupture of the artery. Carotid artery dissections can occur spontaneously or in the setting of trauma. Traumatic carotid artery dissections (TCAD) are rare and occasionally present with third-order Horner syndrome, characterized by ipsilateral ptosis, miosis, and anhidrosis. The presence of subtle physical exam sig

Raising awareness through a case of recurrent fractures in a child with congenital insensitivity to pain and anhidrosis in Saudi Arabia.

Patients with congenital insensitivity to pain and anhidrosis (CIPA) exhibit an inability to perceive pain, recurrent fractures, self-mutilation, and impaired thermoregulation, which lead to severe complications and high morbidity. Children with CIPA often sustain multiple unnoticed fractures that may be mistaken for child abuse because of the presence of fractures at different stages of healing. If unrecognized or inadequately managed, these injuries can cause permanent damage. We report the case of a 9-year-old boy who presented with recurrent fractures, chronic osteomyelitis, heterotrophic
