Prevalence of Hidradenitis Suppurativa in Bucharest, Romania.

UNLABELLED: <p>Introduction: Hidradenitis suppurativa is a chronic inflammatory disease of the apocrine gland-bearing areas frequently misdiagnosed or underdiagnosed. Its true prevalence is still unknown and there is an ongoing interest in this matter. The current study was a part of the Global Hidradenitis Suppurativa Atlas (GHiSA) initiative that is actively involved in better understanding the epidemiological characteristics of HS. METHODS: This study was a multicenter study in 2 tertiary care hospitals in Bucharest. After obtaining consent, data from 796 healthy accompanying adults were i

Extracellular matrix alterations in inflammatory skin diseases: emerging biomarkers and clinical implications.

The interaction among extracellular matrix (ECM) components (proteoglycans, elastin, laminins, collagens and metalloproteinases) is important for maintaining homeostasis of the skin. However, in chronic inflammatory diseases, tissue homeostasis is disrupted by having an uncontrolled remodelling of the ECM, resulting in an imbalance of its composition and functionality. In these pathologies, there is a dysregulation of the immune system characterized by recurrent itching, pain and inflammation in affected areas of the skin. Three of the most common inflammatory skin diseases are hidradenitis su

Genomic loci and molecular genetic mechanisms for hidradenitis suppurativa.

BACKGROUND: Hidradenitis suppurativa (HS) is a common, chronic and debilitating inflammatory disease that most commonly affects intertriginous skin. Despite its high heritability, the genetic underpinnings of HS remain poorly understood. OBJECTIVES: To identify genetic signals associated with HS, determine genetic relationships with other diseases and investigate potential molecular genetic mechanisms. METHODS: We performed a genome-wide association meta-analysis of six studies, totalling 4540 patients with HS and > 1 million control participants, and identified genetic correlations with oth

Utility of Immunohistochemistry in Diagnosis of Papillary Eccrine Adenoma/Tubular Apocrine Adenoma (PEA/TAA).

Papillary eccrine adenoma/tubular apocrine adenoma (PEA/TAA) is a rare sweat gland neoplasm. Because PEA/TAA shares morphologic features with other sweat gland tumors, including digital papillary adenocarcinoma, histologic diagnosis may be challenging. We report a case of PEA/TAA presenting as a nodule on the left foot of a 43-year-old man. Histopathologic examination revealed a well-circumscribed dermal proliferation of variably sized glands composed of ductal cells with an outer layer of myoepithelial cells. By immunohistochemistry, the ductal tumor cells showed strong, diffuse expression of

[Translated article] Hyperhidrosis, Anticholinergics, and Dementia.

In recent decades, the use of certain oral anticholinergics for the treatment of hyperhidrosis has become widespread, often off-label but supported by multiple studies, including clinical trials, demonstrating their effectiveness and an apparently good safety profile. Similarly, various studies published in recent years have associated the use of anticholinergics to the development of dementia, particularly in elderly patients. Additionally, other studies have suggested that hyperhidrosis itself may be an early symptom of developing dementia. However, to date, no research has specifically link

Hypohidrotic Ectodermal Dysplasias: Phenotypic and Genotypic Findings in 32 Cases.

Hypohidrotic ectodermal dysplasias are a genetic condition affecting ectoderm-derived structures such as hair, teeth, nails, and sweat glands, resulting from variations in the EDA, EDAR, EDARADD, and WNT10A genes. This study examined 32 cases from 25 unrelated families from Türkiye, identifying seven novel variants in the EDA, EDAR, and WNT10A genes. The distribution of genetic alterations across the cohort revealed that 44% of the families (11/25) harbored variants in EDA, whereas EDAR and WNT10A variants were identified in 32% (8/25) and 24% (6/25) of families, respectively. Clinical evaluat

OCULAR MANIFESTATIONS IN A PATIENT WITH HIDRADENITIS SUPPURATIVA: A CASE STUDY.

BACKGROUND: Patients with chronic inflammatory skin disease called hidradenitis suppurativa (HS), which can also produce systemic comorbidities, show ocular symptoms. Although HS has dermatological roots, if left untreated it can cause major ocular issues like dry eye syndrome and corneal neovascularization, therefore affecting eyesight. CASE DESCRIPTION: Our patient, a 19-year-old man with corneal neovascularization, scarring, bilateral dry eye disease, uncontrolled HS, and other medical disorders, after a year of therapy with adalimumab failed to sufficiently control the illness, secukinuma

Harlequin Syndrome in a Post-COVID-19 Infection Elderly: A Case Report.

Harlequin syndrome (HS) is a rare autonomic nervous system disorder caused by blockage of the unilateral upper-thoracic sympathetic pathway that provides vasomotor and sudomotor supply to the face. Infection and the subsequent immune response cascade are proposed as triggers for arterial occlusion due to microvascular ischemia, underlying the pathophysiology of HS in adult cases. We report this unusual case to inform readers about the possibility of causation and pathophysiological pathway of HS in post-COVID-19 infection. An 82-year-old male patient developing HS post-COVID-19 infection, pres

Magnetic Resonance Imaging for Distinguishing Perianal Hidradenitis Suppurativa from Fistulizing Crohn Disease.

INTRODUCTION: Both hidradenitis suppurativa (HS) and Crohn disease (CD) are characterized by suppuration and granulomatous inflammation, which may result in formation of tunnels, sinus tracts, or fistulas. We sought to determine the differences in characteristics of fistulizing perianal CD vs. perianal HS on magnetic resonance imaging (MRI). METHODS: This retrospective cohort study included patients diagnosed with CD, HS, or both diseases who underwent medical examination at the Mayo Clinic between January 1, 1998, and July 31, 2021. We further selected patients who had documented perianal le

A missense mutation in the KCNE4 gene is not predictive of equine anhidrosis.

Anhidrosis is defined as a decreased or absent ability to sweat in response to heat and exercise. In horses, this condition can increase the risk of life-threatening hyperthermia. A prior study has suggested that equine anhidrosis is associated with a missense variant (rs68643109) in the Potassium Voltage-Gated Channel Subfamily E Regulatory Subunit 4 (KCNE4) gene. This project aimed to validate this association in a population of well-phenotyped horses and to determine the allele frequency of this variant in publicly available whole-genome sequence data. Fifty horses within the University of

A Patient With Concurrent Hidradenitis Suppurativa and Porokeratosis Palmaris et Plantaris Disseminata: Case Report and Review of Autoinflammatory Keratinization Diseases.

The term autoinflammatory keratinization diseases (AIKDs) was recently proposed as a unifying concept for diseases characterized by inflammation in the epidermis and upper dermis which leads to hyperkeratosis, caused by genetic perturbations of the innate immune system. We present a case of a patient with hidradenitis suppurativa and porokeratosis, two AIKDs, followed by a review of these conditions as well as other AIKDs. This case was distinguished by hypertrophic porokeratoses involving cystic hair follicles, showing histopathologic features of both conditions within single biopsy specimens

Fourteen New Pediatric Cases of Shapiro Syndrome.

BACKGROUND: Spontaneous periodic hypothermia (SPH), also known as Shapiro syndrome, is a rare syndrome characterized by paroxysmal episodes of spontaneous hypothermia. The diagnosis of SPH is based on clinical features; there are no specific instrumental tests and no agreed diagnostic criteria. The pathogenetic mechanisms underlying SPH are unclear, and from the therapeutic point of view opinions are divergent. To date, very few cases have been reported in the literature and among these only 30 are pediatric cases. We aim to expand the current knowledge of pediatric-onset SPH by reporting 14 n

The Hidradenitis Suppurativa Symptom and Impact Diary: Development and psychometric evaluation of a novel set of patient-reported outcomes for hidradenitis suppurativa.

BACKGROUND: Hidradenitis suppurativa (HS) is associated with a substantial disease burden. Given the complex nature of HS-related symptoms, patient-reported outcome (PRO) measures are important to ensure that the patient experience is captured when evaluating the efficacy of treatments in clinical trials. OBJECTIVES: To develop the Hidradenitis Suppurativa Symptom and Impact Diary (HSSID©), a novel PRO measure for use in clinical trials to assess the symptoms and impacts of HS in adult patients, and to validate its psychometric properties. METHODS: The development phase involved patients wit
