Evidence of autoimmune etiology in children with small fiber neuropathy.

OBJECTIVE: Small fiber neuropathy (SFN) is becoming increasingly recognized as a cause of autonomic dysfunction, sensory disturbances, and neuropathic pain. While SFN was previously considered predominantly idiopathic, cases of autoimmune SFN are now well described in adults but remain underexplored in children. METHODS: In this retrospective chart review, we identified pediatric patients diagnosed with SFN of suspected autoimmune etiology. SFN was diagnosed based on epidermal nerve fiber density (ENFD) and/or sweat gland nerve fiber density (SGNFD) values from skin biopsy and/or quantitative

Robotic selective thoracic sympathectomy for hyperhidrosis: refined technique and anatomical variation.

Primary hyperhidrosis substantially impairs quality of life. Conventional sympathectomy risks compensatory hyperhidrosis through trunk disruption, whilst selective sympathectomy preserves the trunk while dividing preganglionic and postganglionic fibres. We describe a technique refinement and a previously undescribed anatomical variant. A 23-year-old man had severe refractory palmar, axillary, plantar and gustatory hyperhidrosis (SSDS 4) profoundly affecting his confidence, social functioning and mental wellbeing, despite conservative management. He underwent staged bilateral robotic selective

Dietary and Nutritional Considerations for Metabolic Dysfunction-Associated Steatotic Liver Disease in Psoriasis and Hidradenitis Suppurativa: A Narrative Review.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognised in patients with chronic inflammatory skin disease, particularly psoriasis, for which epidemiological evidence of increased MASLD prevalence is relatively strong; corresponding data in hidradenitis suppurativa (HS) remain more limited. This narrative review examines the epidemiological and biological links between psoriasis, HS and MASLD, and evaluates dietary and nutritional strategies relevant to their multidisciplinary management. Particular emphasis is placed on distinguishing direct evidence in pat

Targeting an IL-17/IL-36 pathogenic hub in hidradenitis suppurativa with a bispecific antibody.

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disorder with a high unmet need, particularly for treatment-resistant tunnels. Integrated multiomics analysis revealed that HS tunnels are spatially organized, self-amplifying inflammatory circuits driven by synergistic IL-17/IL-36γ activation of a unique keratinocyte subset, which activates neutrophils and inflammatory fibroblasts. This mechanistic insight coupled with spatial coenrichment of IL-17A and IL-36R provided the rationale for HB0043, a bispecific antibody that leverages IL-36R binding to locally dual-block both cytokines.

The prevalence of depression among patients with primary hyperhidrosis: a systematic review and meta-analysis.

BACKGROUND: A well-established clinical association is recognized between primary hyperhidrosis (PH) and depression. However, the evidence about the prevalence of depression among people with PH is inconsistent, and there is currently no systematic review on the subject. METHODS: This study systematically searched PubMed, Cochrane Library, Embase, Ovid MEDLINE, Web of Science, and PsycINFO for studies reporting on the prevalence of depression in people with PH from database inception to December 19, 2025. We included cross-sectional and cohort studies reporting depression prevalence, as well

Lung disease in crisponi/cold-induced sweating syndrome type 1: expanding the clinical phenotype spectrum.

UNLABELLED: Crisponi/Cold-Induced Sweating Syndrome Type 1 (CS/CISS1) is an ultra-rare genetic disorder caused by biallelic variants in the cytokine receptor-like factor-1 (CRLF1) gene and is associated with several comorbidities. Comprehensive data on the respiratory phenotype of this condition are lacking despite being associated with high mortality in infancy, often due to severe respiratory complications like aspiration pneumonia and sudden death related to hyperthermic crises. Our study investigates respiratory complications and lung ultrasound characteristics, enhancing the comprehension

Mammary-type Metaplastic Carcinoma Arising From Vulvar Hidradenoma Papilliferum: A Case Report.

Hidradenoma papilliferum is a benign neoplasm arising in the vulvovaginal region. These lesions are thought to originate from anogenital mammary-like glands and share morphologic and immunohistochemical features with breast tissue. Though rare, malignant transformation of hidradenoma papilliferum into mammary-type carcinoma, typically mammary-type ductal and mammary-type lobular carcinoma, has been documented. However, mammary-type metaplastic carcinoma arising from hidradenoma papilliferum is exceedingly rare. Herein, we describe a case of this entity in a patient presenting with a labial mas

Emerging Topical Therapies and Skin Care Management for Hidradenitis Suppurativa.

Hidradenitis suppurativa (HS) is a painful, chronic inflammatory condition that forms recurring nodules and abscesses in skin folds, leading to severe scarring. While the exact pathogenesis is still being determined, it is thought to involve follicular occlusion and rupture, driven by multifactorial influences such as genetics, environment, hormones, microbiome, and immune dysregulation. The complex nature of this disease entails complicated management and has led to the development of various therapies targeting these different factors. While moderate-to-severe HS has seen growing therapeutic

Low-Grade and Intermediate-Grade Apocrine Intraductal Carcinoma Arising in a Nevus Sebaceous of Jadassohn.

Nevus sebaceous (NS) is a congenital hamartoma that may give rise to a wide spectrum of benign and malignant adnexal neoplasms, although malignant transformation is uncommon. We report a case of low- and intermediate-grade apocrine intraductal carcinoma arising in a NS of Jadassohn in a 93-year-old woman. Histopathologic examination demonstrated an intraductal apocrine neoplasm with solid and cribriform architecture, focal comedonecrosis, and an intact surrounding myoepithelial cell layer highlighted by p63 and CK5/6 immunostaining. The neoplastic cells expressed CK7, epithelial membrane antig

Distinct cutaneous α-synuclein signatures in body-first and brain-first Parkinson's disease subtypes.

Parkinson's disease (PD) is characterized by pathological α-synuclein (α-syn) aggregation, yet the origin of α-syn pathology (central or peripheral) remains debated. The synuclein origin and connectome model proposes two subtypes, i.e. brain-first (pathology initiating in brain structures) and body-first (originating in peripheral autonomic nerves) subtypes. In this study, we aimed to delineate cutaneous α-syn signatures between these subtypes to validate the synuclein origin and connectome model. In this cross-sectional study, 126 brain-first PD patients, 79 body-first PD patients and 60 heal

Hidradenitis suppurativa associated with chromosomal abnormalities, genodermatoses and genetic disorders: a review.

Hidradenitis suppurativa (HS) is a severe chronic relapsing inflammatory disease of the hair follicle unit, characterized by painful nodules, abscesses, tunnels and tracts in intertriginous areas. Although the pathogenic mechanisms of HS have not yet been fully elucidated, genetics is a central component of its pathogenesis, and understanding relevant genetic factors is essential for gaining insights into the molecular mechanisms of HS. We conducted a comprehensive review of common and rare associations between HS and chromosomal and genetic diseases, exploring the pathogenetic links between t
