Topical capsaicin in dermatologic and peripheral pain disorders.

Topical capsaicin has been introduced in the U.S. and Canada as a cream indicated for temporary relief of neuralgia following episodes of herpes zoster infections and in the treatment of diabetic neuropathy. Although capsaicin is clinically used as an external analgesic for temporary relief of neuralgia, it has also been widely used as a research tool to study peripheral pain. Capsaicin apparently works to release substance P from sensory nerve fibers and after repeated applications, depletes neurons of substance P. Clinical investigations of topical capsaicin include trials in chronic pain sy

[Multiple perineal fistulas caused by suppurative hidrosadenitis (Verneuil's disease)].

The attention of all those having to deal in any way with diseases of the perianal region is called to a particular pyodermatitis known as suppurative hidradenitis, or Verneuil's disease, frequently confused with other inflammatory pathologies. A clinical case exemplifying the diagnosis and treatment of the disease is reported.

[Ten cases of an apocrine carcinoma of the breast and a review of cases in the Japanese literature].

To encounter an apocrine carcinoma of the breast is very rare. We have experienced, however, 10 cases of an apocrine carcinoma (0.65%) among 1541 primary breast carcinoma cases. The mean age of the patients was 58.1 years, and the tumor size on palpation ranged from 1.1 to 6.0 cm (mean 3.5 cm). A lymph node metastasis was seen in 5 cases. While one patient determined as being in stage III b of the disease died of the cancer approximately 5 months after surgery, all of other patients are still alive. The prognosis of this type of disease appears to be better than that of a common invasive ducta

[Immunohistochemical studies of breast and extra-mammary Paget's disease indicate an apocrine differentiation].

Using the PAP technique, paraffin-embedded and formalin-fixed tissue sections taken from 11 patients suffering from Paget's disease (5 mammary, 6 extramammary cases) were stained with antibodies against keratin, actin, S 100 protein, CEA, HMFG 1, cytokeratin 6 and 18 (Dako-CK-1), as well as the lectin SBA. We did not find any differences between mammary and extramammary Paget's disease. In both forms of the disease, all tumor cells showed reaction to antibodies against HMFG 1 and CEA and, in 50 to 100%, to the lectin SBA. The other antibodies investigated did not react with Paget's cells. Sinc

Immunohistochemistry of GCDFP-24 and zinc alpha2 glycoprotein in benign sweat gland tumors.

The immunohistochemical localization of two other proteins that are present in breast gross cystic disease fluid, GCDFP-24 and zinc alpha 2 glycoprotein (Zn2GP), were studied in normal skin and in 41 benign sweat gland tumors. GCDFP-24 was localized to apocrine glands. There was no staining of eccrine glands or ducts. There was positive staining in the following sweat gland tumors: apocrine hidrocystoma (four of five), hidradenoma papilliferum (two of four), syringocystadenoma papilliferum (six of seven), mixed tumor (one of one), and glandular elements of cylindroma (one of four). No staining

A monoclonal antibody, SKH1, reacts with 40 Kd sweat gland-associated antigen.

Several monoclonal antibodies (MAB) have been produced using an eccrine carcinoma cell line as an immunogen. One such MAB, SKH1, reacted with both the secretory portion and coiled duct of the eccrine and with the secretory portion of apocrine gland. SKH1, however, did not react with myoepithelial cells, intradermal ducts of both types of sweat gland, or with other components of normal axillary skin including the epidermis and follicular apparatus. The reaction was strongest if the specimen was fixed with 80% methanol, and moderate on non-fixed or acid-alcohol-fixed specimens. Only weak reactio

Prolactin, human nutrition and evolution, and the relation to cystic fibrosis.

Cystic fibrosis (CF) is a lethal, genetically transmitted disease of Caucasian populations. Its prevalence is highest (ca 1:2000 live births) among Western and Central Europeans and their descendants. Major clinical symptoms are chronic, obstructive, pulmonary disease, impaired intestinal digestion and absorption and elevated concentration of salt in sweat. The last is important, not only for diagnosis, but because it is an example of an electrolyte transport defect present in other epithelial tissues. Numerous other clinical manifestations are generally present. The history of prolactin (PRL)

[Extramammary Paget's disease. Immunocytochemical study and histogenetic considerations].

We report a case of extramammary Paget's disease arising in the anogenital region in association with an underlying sweat gland microcarcinoma. Paget's cells were investigated with monoclonal anti-EMA and anti-cytokeratin antigen and with mono- and polyclonal anti-CEA antigen. Positive immunostaining was observed in Paget's cells and in the underlining tumor, whereas keratinocytes and melanocytes did not stain. CEA was also detected in cells and secretions of normal apocrine glands. The immunohistochemical use of polyclonal anti-keratin and anti-S-100 protein antigen is helpful in differentiat

[Apocrine chromhidrosis].

Apocrine chromhidrosis is an extremely rare and impressive skin disorder. Primarily after emotional stress patients complain of coloured sweat secretion (black, green, blue or yellow). The deposition of lipofuscin in the apocrine glands may be the cause of this disease, the different colours of sweat being due to various oxidation stages of lipofuscin. We report two patients, one with blue and one with green axillae; the clinical features, histopathology, differential diagnosis, and pathogenesis of apocrine chromhidrosis are discussed.

Treatment of apocrine chromhidrosis with topical capsaicin.

A 30-year-old white woman with facial apocrine chromhidrosis was treated successfully with topical capsaicin once or twice daily. Paired comparison of treated versus untreated sides and active drug-treated versus vehicle-treated sides showed suppression of the chromhidrosis only when capsaicin was used. After treatment was discontinued, the chromhidrosis returned within 2 days.

Surgical management of severe mammary hidradenitis suppurativa.

Hidradenitis suppurativa is a devastating disease of the apocrine sweat glands. The chronic form is characterized by a malodorous discharge from multiple draining sinuses embedded in dense fibrous tissue, and physical discomfort. The disease commonly affects the axillary, perineal, and perianal regions. A case of hidradenitis suppurativa of the mammary gland necessitating mastectomy is presented here to illustrate the severity of the disease at an uncommon site. As in other regions of the body, aggressive surgical intervention provides a chance for rapid recovery. The surgical treatment should

Acute radiation injury of ocular adnexa.

Eyelids and lacrimal glands of monkeys examined histologically 24 and 48 hours after receiving 2.5 to 20.0 Gy (250 to 2000 rad) of cobalt 60 radiation contained acute inflammation. Cells of meibomian glands, other sebaceous glands, sweat glands, and goblet cells displayed no significant alterations. Acinar cells of major and accessory lacrimal glands and, to a lesser degree, lacrimal duct epithelium showed degeneration and necrosis that were increased in extent in proportion to the radiation dose. Reduced numbers of serous acini and decreased size of remaining acini indicate that atrophy of la

The symptomatology of hidradenitis suppurativa in women.

In order to establish the possible role of androgen in the development of hidradenitis suppurativa the symptomatology of the disease was studied in a group of 70 female patients. The results were compared with those obtained from an age-matched control group of healthy women. The incidence of women with signs of androgenization in the two groups did not differ significantly. The only significant differences found were a shorter menstrual cycle and a longer duration of menstrual flow in the women suffering from hidradenitis, and that women with hidradenitis were more likely to have a positive f

Electrosurgery using insulated needles: treatment of axillary bromhidrosis and hyperhidrosis.

This paper deals with operative techniques and clinical aspects of electrically treating axillary bromhidrosis and hyperhidrosis using insulated needles. Most patients were treated 5 or 6 times using 2 types of needles, one for electroelimination of sweat glands and the other mainly for epilation. The results for most patients were satisfactory, with scarring occurring only in the early developmental stages of the technique.

Pathophysiology of cystic fibrosis with emphasis on salivary gland involvement.

Cystic fibrosis (CF) is a fatal autosomal recessive disorder which affects all exocrine glands, or perhaps all epithelial surfaces. The three organs most consistently affected are the eccrine sweat gland, which produces excessively salty sweat; the lung, in which chronic obstructive pulmonary disease invariably develops and is usually the cause of mortality; and the pancreas, which fails to produce adequate bicarbonate ion and water in nearly all patients, and produces inadequate digestive enzymes in most, giving rise to pancreatic insufficiency. However, the liver, reproductive tracts, intest

Disorders of the autonomic nervous system: Part 1. Pathophysiology and clinical features.

Autonomic dysfunction may result from diseases that affect primarily either the central nervous system or the peripheral autonomic nervous system. The most common pathogenesis of disturbed autonomic function in central nervous system diseases is degeneration of the intermediolateral cell columns (progressive autonomic failure) or disease or damage to descending pathways that synapse on the intermediolateral column cells (spinal cord lesions, cerebrovascular disease, brainstem tumors, multiple sclerosis). The peripheral autonomic nervous system may be damaged in isolation in the acute and subac

Generalized anhidrosis associated with Fabry's disease.

A 28-year-old Korean man suffered from generalized acquired anhidrosis and heat intolerance that were confirmed by a sweat test. Other clinical features consistent with Fabry's disease were fever and severe pain of the lower extremities and leg edema. Although the patients lacked cutaneous angiokeratomas, ultrastructural studies of the normal skin demonstrated diagnostic intracytoplasmic inclusions in the endothelial cells and pericytes of dermal vessels, fibroblasts, perineural cells, and secretory cells of eccrine glands. alpha-Galactosidase assay of the patient's leukocytes showed markedly

[Effects of chronic pancreatitis on salivary secretion].

In chronic pancreatitis, increased concentrations of Na+ and Cl- in sweat have been attributed to increased parasympathetic drive. It was postulated that similar changes might occur in saliva. To avoid masking increased parasympathetic drive, saliva was collected without stimulation. In patients with chronic pancreatitis, there were significant increases of concentrations of Na+ and Cl- in basal salivary concentrations. These differences disappeared when salivary secretion was stimulated with citric acid. Anatomic or neurologic modification of the salivary glands seemed unlikely as stimulated
