A Case Series of Tofacitinib Treatment for Chronic Spontaneous Urticaria with Inadequate Response to Omalizumab.

BACKGROUND: Chronic spontaneous urticaria (CSU) is a refractory dermatosis characterized by recurrent wheals and pruritus. Although omalizumab is recommended for patients failing antihistamine therapy, some patients remain unresponsive. Tofacitinib is an oral JAK inhibitor that exerts immunosuppressive effects by blocking the key intracellular inflammatory signaling pathway (JAK-STAT). Currently approved for the treatment of rheumatoid arthritis, psoriatic arthritis, ulcerative colitis, and other conditions, it is also widely used in clinical practice for atopic dermatitis, vitiligo, and refra

Integrative Multi-Omics and Experimental Analyses Identify TAPBP as a Key Mediator Linking Genetic Susceptibility to Melanocyte Dysfunction in Vitiligo.

BACKGROUND: Vitiligo is an autoimmune depigmenting disorder characterized by selective melanocyte loss; however, the specific molecular bridges between genetic risk variants and melanocyte-intrinsic immune vulnerability remain largely unexplored. OBJECTIVE: To elucidate the molecular link between genetic susceptibility and melanocyte injury in vitiligo by integrating multi-omics data. METHODS: We integrated large-scale genome-wide association data (FinnGen R12, n=466,064), expression quantitative trait loci (eQTLgen, n=31,684), and single-cell transcriptomic profiles to identify genes mediat

Diagnosis and Management of Comorbid Autoimmune Disorders in Children with Chronic Spontaneous Urticaria.

Chronic urticaria (CU) is characterized by the recurrent occurrence of wheals and/or angioedema for more than six weeks. CU comprises two principal subtypes: chronic spontaneous urticaria (CSU) and chronic inducible urticaria (CIndU). Increasing evidence suggests that CSU has an autoimmune basis, and patients with CSU exhibit a higher prevalence of concomitant autoimmune disorders. While this association has been well documented in adults, data in pediatric populations remain limited. Available studies indicate that the overall burden of autoimmunity in children with CSU may be lower than in a

Unanchored by two hits: IFNγ and mechanical stress synergize to undermine melanocyte adhesion and promote vitiligo.

BACKGROUND: Vitiligo is a chronic depigmentation disorder caused by selective melanocyte loss. Autoreactive CD8+ T cells are known contributors, but impaired melanocyte-keratinocyte adhesion due to E-cadherin dysfunction has also been implicated. OBJECTIVES: This study aimed to identify the key adhesion molecules mediating melanocyte-basement membrane interactions and to investigate their modulation in response to vitiligo-associated factors, including IFNγ and mechanical stress. METHODS: Primary human epidermal melanocytes (PHEMs) and ex vivo human skin tissues were exposed to IFNγ and mech
