A comparison of interview data on 595 patients with newly incident cutaneous melanoma, excluding lentigo maligna melanoma and acral lentiginous melanoma, with data from comparison subjects drawn from the general population, showed that melanoma risk increased in association with the frequency and severity of past episodes of sunburn, and also that melanoma risk was higher in subjects who usually had a relatively mild degree of suntan compared to those with moderate or deep suntan in both winter and summer. Clinical manifestation of neurofibromatosis include multiple cafe-au-lait spots, axillary freckles, congenital glaucoma, relative macrocephaly, radiologic findings of overtubulation of the long bones, and precocious puberty. Photochemotherapy(PUVA)-induced freckles were found in 25 patients (41%) who had received more than 1,000 J/cm2 of PUVA. Persistent ashen-gray maculae 5-7 mm in diameter and freckles developed after prolonged PUVA therapy of psoriasis. Histopathology of maculae showed extensive collections of melanophages in the upper dermis. The melanocytes were strongly stimulated at the site of the freckles. The number of melanosomes, mitochondria, endoplasmic reticulum and dendrites were increased. Pathological features such as large amounts of lipid droplets and lysosome-melanosome complexes were also found within the melanocytes.

Epidermal sunburn (SB) cells were counted 24 h after exposure of the skin to suprathreshold doses of solar simulated radiation. The sites compared were evenly pigmented black skin and freckled white skin. SB cell numbers were dose related in all cases. There were fewer SB cells in black than white skin following equivalent erythemogenic doses of UVR. Equal doses of UVR produced fewer SB cells in freckled than non-freckled white skin. Freckles provide a significant degree of protection against acute sunburn.

Freckles were induced experimentally by single exposures of lower back skin to ultraviolet radiation. The minimal freckling dose was between 6 and 10 times the minimal erythema dose. Repeated less intense exposures did not induce freckles. Freckles could be induced only in subjects with natural freckles on exposed skin. Freckles induced by single exposures to ultraviolet radiation have distinctive characteristics which enable their clinical recognition. We have called these "sunburn freckles".

Seventy-nine Nigerian oculocutaneous albinos were investigated. Fifty-six had typical tyrosinase-positive albinism (TPA) and 23 had brown albinism (BA), a new oculocutaneous type.
