Adult male patients with DKC1 mutations present early-onset pulmonary fibrosis and severe prognosis.

BACKGROUND: Mutations in telomere-related genes (TRGs) are the main cause of monogenic familial pulmonary fibrosis. Dyskerin, encoded by the X-localised gene DKC1, is involved in telomere maintenance. METHODS: This retrospective study aimed to further characterise the pulmonary phenotype of DKC1-deficient patients with pulmonary fibrosis identified between 2010 and 2025 in our laboratory. RESULTS: We reported eight, as yet undescribed, to our knowledge, probands affected by pulmonary fibrosis associated with X-linked DKC1 deficiency. The median age at interstitial lung disease (ILD) diagnosi

Scalable TCR synthesis and screening enable antigen reactivity mapping in vitiligo.

T cells initiate targeted immune responses using T cell receptors (TCRs) to recognize specific antigens. Mapping TCRs to antigens at scale remains a major challenge. Here, we developed an approach to synthesize and functionally screen tens of thousands of TCRs simultaneously. TCR rapid assembly for functional testing (TCRAFT) uses a modular strategy to rapidly and inexpensively construct large pools of TCRs from sequences while maintaining TCRα/β pairing. We applied TCRAFT to reconstruct over 3,800 TCRs from vitiligo blister fluid and mapped these TCRs to specific peptide-major histocompatibil

From immunological mechanisms to targeted therapies: a bibliometric analysis of vitiligo treatment research (2005-2025).

BACKGROUND: Vitiligo is a chronic autoimmune disorder characterized by CD8+ T cell-mediated destruction of melanocytes. It affects up to 2% of the global population and has a profound psychosocial impact. Over the past two decades, advances in understanding immunological mechanisms, particularly IFN-γ signaling and JAK-STAT pathway dysregulation, have reshaped therapeutic approaches from empirical regimens to targeted immunomodulatory interventions. OBJECTIVE: To analyze global vitiligo treatment research trends from 2005 to 2025, examining publication patterns, collaboration networks, and th

Adipose-Derived Mesenchymal Stem Cell Secretome for Post-Steroid Hypopigmentation and Skin Atrophy: A Case Report.

Skin hypopigmentation and atrophy are the most commonly reported local adverse effects following corticosteroid application. While these changes are typically reversible, the recovery may be delayed or incomplete in older adults due to diminished physiological reserves and age-related reductions in melanocyte function and dermal regenerative capacity. Adipose-derived mesenchymal stem cells (ADMSCs) secretome may offer substantial benefits in reversing the alterations. In this report, we present the clinical evaluation of an elderly patient with steroid-induced cutaneous manifestations receivin

Oxidative stress activates HIF-1α to mediate the secretion of CXCL9 and CXCL10 in keratinocytes and trigger the abnormal immune response in vitiligo.

Vitiligo, a common autoimmune dermatosis, has a pathogenesis hypothesized to involve oxidative stress-induced immune-mediated melanocyte death. However, the role of oxidative stress in triggering autoimmunity during the early stages of vitiligo, along with its regulatory mechanisms and complex interactions, remains incompletely understood. Keratinocytes, as key initiating cells in vitiligo, secrete various chemokines, primarily C-X-C motif ligand 9 and 10 (CXCL9,10), under oxidative stress to recruit autoreactive immune cells targeting melanocytes. Hypoxia-inducible factor-1α (HIF-1α), a highl

Defining Important Aspects of Repigmentation in Vitiligo: Insights From Patients' Perspectives.

Repigmentation is an essential outcome in vitiligo assessment, yet it remains broadly defined. While perspectives from vitiligo experts have been explored, gaining insights from patients is crucial for a more comprehensive understanding. Ultimately, this could contribute to the development and refinement of core outcome sets for vitiligo. This study aimed to determine aspects of repigmentation patients consider most important. Two surveys and one focus group were conducted, involving 34 patients/patient caregivers and 20 patient representatives. In the surveys, aspects were deemed important if

Advances in Janus Kinase Inhibitors for Vitiligo Treatment.

Vitiligo is a cutaneous autoimmune disease characterized by the destruction of epidermal melanocytes leading to white patches with a global prevalence of about 0.5-2%, and patients' quality of life are greatly affected by the change in appearance and social discrimination caused by the disease. Most of the key cytokines in the pathogenesis of vitiligo act through the Janus kinase/signal transducer and activator of transcription signaling pathway, which is an effective therapeutic target. The first generation Janus kinase inhibitors, i.e., tofacitinib and ruxolitinib, inhibit a variety of Janus

Type I Interferons Induce CD8+ T Cell Chemotaxis Through Regulating Melanocyte but Not Keratinocyte CXCL9/10/11 Secretion in the Initiation of Vitiligo.

Early intervention is essential for a successful therapy and a favorable prognosis in vitiligo treatment. While emerging evidence indicates an association between the type I interferons (I-IFNs) signature and the initiation of the immune response, it remains unclear whether I-IFNs function as a critical mediator in the vitiligo initiation phase. Their precise molecular mechanisms of action necessitate further investigation. I-IFNs mainly include subtypes IFNα, IFNβ, and IFNκ. In our study, we observed markedly elevated levels of IFNβ in both peripheral blood and perilesional skin of vitiligo p

Metastatic melanoma of unknown primary: an unusual dermatologic presentation.

Metastatic melanoma without an identifiable primary lesion presents a unique diagnostic challenge and raises consideration of immune-mediated regression or non-cutaneous tumour origin. We describe the case of a man in his 80s diagnosed with metastatic melanoma, for whom a thorough skin examination and staging workup revealed no definitive primary lesion. Dermatologic evaluation revealed vitiligo-like depigmentation and a grey macule with clinical features suggestive of regression. While a definitive primary site could not be established, the constellation of clinical and dermatologic findings

Clinical and molecular markers of vitiligo: a narrative review of early-stage insights into disease activity and treatment response.

Vitiligo fluctuates between progressive and quiescent states, complicating decisions about when to intervene, how to select therapy and how to monitor response. In this review, we integrate bedside phenotypes with multilevel molecular readouts to summarize emerging evidence linking clinical signs and biologic signals to disease activity and treatment response in clinical studies. For staging, tool-free and standardized assessments such as Koebner phenomenon, confetti-like depigmentation, Wood's lamp and dermoscopy provide visible clues associated with active disease.
