Metformin: Old Drug, New Therapeutic Potential in the Skin? A Brief Narrative Review.

Metformin, a well-established, first-line therapy for type 2 diabetes mellitus, is still gaining interest for its potential applications beyond glycaemic control. In addition to its insulin-sensitising effects, metformin exhibits anti-inflammatory, antioxidant, anti-proliferative, and immunomodulatory properties, rendering it a promising candidate for various dermatological conditions. Emerging evidence suggests that metformin may play a beneficial role in inflammatory skin diseases, such as acne, hidradenitis suppurativa, acanthosis nigricans, rosacea and psoriasis. Furthermore, recent studie

Oral microbiota signatures in obesity with or without acanthosis nigricans in a Chinese cohort.

Introduction. The oral microbiota is the second most complex microbial community in the human body. It has been suggested that poor oral health may be associated with an increased risk of obesity.Hypothesis/Gap Statement. However, both previous observational and mechanistic studies on oral microbiota do not take into account the obesity-related acanthosis nigricans (AN), which is the most common dermatological manifestation in individuals with obesity.Aim. This study aimed to investigate the altered composition, function and diagnostic value of the oral microbiota in obesity with or without ac

Genealogical Rabson-Mendenhall syndrome caused by INSR gene mutation.

Rabson-Mendenhall syndrome (RMS) is a rare autosomal recessive disorder caused by mutations in the insulin receptor gene (INSR), leading to severe insulin resistance. Clinical manifestations of RMS include hypertrichosis and acanthosis nigricans. A 3-yr-old male patient presented with darkened skin on the neck, without any apparent precipitating factors, and did not exhibit symptoms of polyuria or polydipsia. Both the patient and his older sister displayed signs of hypertrichosis and acanthosis nigricans. Laboratory investigations revealed significantly elevated levels of insulin and C-peptide

Acanthosis Nigricans as a Clinical Risk Marker for Metabolic Dysfunction-Associated Steatotic Liver Disease.

BACKGROUND: Despite the association of insulin resistance (IR) in the pathophysiology of Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD), few studies have evaluated the utility of acanthosis nigricans (AN) as a clinical predictor for this condition. Thus, we aimed to determine the role of AN as a risk factor and clinical predictor of MASLD. METHODS: This cross-sectional study conducted a comprehensive clinical history and physical examination. The presence of acanthosis nigricans was assessed in the neck, elbows, axillae, and knuckles. Liver biochemical parameters were measur

Bardet-Biedl Syndrome: Report of a Classical Case from North India.

Bardet-Biedl syndrome (BBS) is a genetic disorder affecting cilia and its functions and manifests floridly in late childhood as a multisystem disorder with the classical triad being polydactyly, atypical retinal dystrophy, and central obesity. So far, very few case reports have been reported from India and even fewer from North India. We are reporting a classic case of a 14-year-old boy presenting to the medicine outpatient department with features of lower respiratory tract infection, who, on work-up, was diagnosed as a case of BBS on clinical grounds. He underwent conservative treatment and,

Type 2 Diabetes Mellitus Diagnosed at the Age of Eight Years: A Case Report.

The onset of type 2 diabetes mellitus (T2DM) in prepubertal age is not uncommon nowadays. Here we are reporting on a young boy who was diagnosed with diabetes at the age of eight years. Diagnosis of diabetes was made based on symptoms of hyperglycemia (new onset bed wetting, also polyphagia and polydipsia) and laboratory evidence of high plasma glucose. The patient was obese (body mass index, BMI > 95th percentile) and had acanthosis nigricans during presentation. The type of diabetes was not confirmed at the time of diagnosis based on the supporting laboratory investigations. His initial high

High Prevalence of Fatty Liver in Bangladeshi Adolescents and Young Adults with Type 2 Diabetes: Key Predictors and Screening Recommendations.

BACKGROUND: Obesity and insulin resistance are the key pathogenic factors for both metabolic dysfunction-associated steatotic liver disease (MASLD) and type-2 diabetes (T2DM), and their rising prevalence is driving an increase in MASLD cases. OBJECTIVE: To assess the presence of fatty liver disease by ultrasonography (USG) and to find out its predictors in newly diagnosed young-onset T2DM. METHODS: This cross-sectional study enrolled 137 newly diagnosed adolescents and young adult T2DM subjects (aged 10-34 years) by non-probability purposive sampling at a tertiary hospital in Bangladesh. Comparison of the Effectiveness of Fractional Carbon Dioxide Laser and Retinoic Acid Peel in the Treatment of Acanthosis Nigricans: A Randomized Controlled Trial.

Introduction Acanthosis nigricans (AN) is a common skin disorder with varied aetiology and a wide palette of treatment options. Despite multiple options available for management, laser as a modality has recently evolved and shown better outcomes. Some fractional ablative lasers induce a very narrow tissue column of thermal damage and neocollagenesis, thereby improving textural irregularities, whereas retinoids, a first-line drug, help in superficial skin peeling. The study was conducted to determine the efficacy of fractional carbon dioxide (CO2) laser and retinoic acid peel and to decide whic

A Comprehensive Review of Acanthosis Nigricans: Pathogenesis, Clinical manifestation and Management.

INTRODUCTION: Acanthosis Nigricans is a dermatological condition characterized by hyperpigmented velvet plaques that can be observed in flexural areas such as the neck, axilla, and groin. AN is frequently associated with insulin resistance and obesity, however, it can also appear in non-obese people and as a paraneoplastic disease. Its prevalence varies across different populations, with higher rates observed in individuals with obesity, diabetes, and certain genetic syndromes. Classification of AN can be based on underlying etiology, distinguishing primary and secondary forms. Pathogenesis is

Acquired Generalized Lipodystrophy with Extensive Autoimmune Involvement: A Case Report and Review of the Literature.

INTRODUCTION: Acquired generalized lipodystrophy (AGL) is a rare condition characterized by body fat loss and, usually, metabolic syndrome with or without associated autoimmune diseases. Clinical phenotypes of AGL are heterogenous, making diagnosis and management challenging. Here, we present a case of AGL with a unique and dominant autoimmune phenotype that prompts discussion of this rare disorder, the prolonged clinical course, and its optimal management. CASE PRESENTATIONS: A two and a half-year-old girl presented with thinning of her arms and legs and vitiligo. She subsequently developed

New Perspectives on Nutraceutical Insulin Sensitizing Agents in the Treatment of Psoriasis and Other Dermatological Diseases.

Insulin resistance (IR) plays a pivotal role in the pathogenesis of several dermatological diseases, including psoriasis, acne, acanthosis nigricans, and hidradenitis suppurativa (HS). These conditions are characterized by chronic inflammation, oxidative stress, and metabolic dysfunction, which are exacerbated by IR. This narrative review examines the emerging role of nutraceutical insulin-sensitizing agents (ISAs), including myo-inositol, alpha-lipoic acid, vitamin D, vitamin C, and folic acid, in managing IR-related dermatological disorders. A comprehensive literature search was conducted ac

Association of Dermatological Manifestations in Patients With Type 2 Diabetes Mellitus With Respect to Duration of Diabetes.

Objective Diabetes mellitus is a disease that can affect various organs, and so all the symptoms should be investigated. Nonetheless, skin symptoms have received less attention than others. The duration of diabetes plays a critical role in the development and progression of systemic and dermatological complications. Existing evidence suggests that many cutaneous complications, such as diabetic dermopathy, xerosis, and infections, tend to increase in frequency and severity with longer disease duration. Therefore, this study assessed and compared the demographic, clinical, lifestyle, and dermato

Cutaneous manifestations of diabetes mellitus: a narrative review.

Diabetes mellitus is a highly prevalent human endocrine disorder. Skin lesions are reported in approximately one-third of all diabetes mellitus patients. The clinical presentation and frequency vary according to the subtype of diabetes mellitus, metabolic control, and clinical course, with certain skin diseases occurring before diagnosing hyperglycemia. In this regard, the correct definition of cutaneous manifestations associated with diabetes mellitus can help define the etiology of hyperglycemia as well as the need to optimize glycemic control. In this narrative review, the most common cutan
