A Rare Case of Fatal Balamuthia mandrillaris Encephalitis in a Pediatric Patient: Diagnostic Challenges.

Balamuthia mandrillaris is a rare and lethal amoeba that causes granulomatous amoebic encephalitis (GAE), a severe brain infection with a high mortality rate. This manuscript presents a case study of a 5-year-old female patient with B. mandrillaris amoebic encephalitis. She presented from an outside hospital (OSH) with daily fevers, fatigue, and worsening mental status. She was without comorbidities, and sick contacts were denied. Diagnosis was challenging due to nonspecific encephalitis symptoms and nonspecific neurological imaging. Neuroimaging revealed symmetric diffuse restriction in the m

Early plasma cell-free DNA diagnosis of fatal Acanthamoeba granulomatous amebic encephalitis in a liver transplant recipient.

BACKGROUND: Acanthamoeba granulomatous amebic encephalitis (GAE) is a rare, highly lethal opportunistic infection in immunocompromised hosts. Diagnosis is difficult because clinical and radiographic findings are nonspecific and conventional cerebrospinal fluid (CSF) studies may be nondiagnostic. CASE: We report a 68-year-old woman who developed fever, seizures, and progressive encephalopathy 13 days after orthotopic liver transplantation. Brain MRI demonstrated multifocal supratentorial and infratentorial enhancing lesions with vasogenic edema and peripheral diffusion restriction. Initial CSF

The Type 6 Secretion System (T6SS) plays a role in the pathogenesis of strain NMEC15.

Neonatal meningitis Escherichia coli (NMEC) is the second leading cause of sepsis and meningitis in newborns with fatality rates of 15-40 %. Survivors may suffer lifelong neurological sequelae such as cerebral palsy, seizures, hearing loss, and delayed development. Despite identification of virulence factors contributing to meningitis, the pathogenesis of NMEC remains to be elucidated. The Type 6 secretion system (T6SS) has been identified in the type strain NMEC RS218 and its associated Hcp proteins were shown to play a role in invasion, cytoskeleton rearrangement, and apoptosis of human brai

Different roads to the brain: A brief overview of divergent pathways and convergent mechanisms in amoebic neuroinvasion.

Free-living amoebae such as Acanthamoeba spp., Balamuthia mandrillaris, and Naegleria fowleri are opportunistic protists capable of causing devastating infections of the central nervous system. Although these organisms differ in ecology, morphology, and clinical progression, they converge on shared strategies that enable adhesion, immune evasion, and neuroinvasion. Here, we briefly present a comparative analysis of their routes of entry, molecular determinants, and neuropathogenic mechanisms with an eye to understand parallel and divergent pathways. A complete understanding of shared and disti

Exploration of Naegleria-preferentially secreted proteins for identifying diagnostic candidates to detect Naegleria fowleri.

Naegleria fowleri is a free-living amoeba that can cause primary amebic meningoencephalitis (PAM), a very serious infection of the central nervous system. Early diagnosis of PAM is challenging, and the condition is almost always fatal. In this study, we conducted 2-dimensional gel electrophoresis (2-DE) analysis using N. fowleri trophozoite lysates and conditioned media to identify preferentially secreted proteins. As a result of the 2-dimensional gel electrophoresis analysis, 1 protein was found to increase, 5 proteins were found to decrease, 3 proteins showed a qualitative increase, and 15 p

The Activity of FDA-Approved Prodrug Isavuconazonium Sulfate and Its Major Metabolite Isavuconazole Against Naegleria fowleri.

Objectives: Free-living amoeba Naegleria fowleri causes primary amoebic meningoencephalitis (PAM). While infection is rare, PAM's fatality rate exceeds 97%. The recommended treatment includes combination therapy, which does not result in uniform survival. Thus, there is a critical unmet need for finding better therapy for PAM. Drug repurposing can expedite the discovery of effective treatment for PAM. Isavuconazonium is approved for the treatment of fungal infections. Given that isavuconazole is the major metabolite of isavuconazonium and isavuconazole penetrates into the brain with high effic

Anticipation of periodic events influences cell motility in amoeba proteus.

All migrating cells (both single-cell organisms and as part of multicellular organisms) sense environmental conditions and respond to physical and molecular cues by changing the direction and speed of cellular movement. Adaptive behaviors enable motile cells to thrive in dynamic environments. A study with slime mold suggested an ability to anticipate dry and cold periods. However, experimental evidence for anticipation in other single-cell organisms is lacking. Here, we investigated whether Amoeba proteus can anticipate unfavourable periodic stimuli. Amoeba proteus react to blue light (405 nm)

A case report of primary amebic meningoencephalitis in a Chinese adult male.

Primary amebic meningoencephalitis (PAM) is a rare neurological infectious disease with an extremely high mortality rate. This disease is caused by Naegleria fowleri invading the brain, and patient infections are usually linked to exposure to contaminated warm freshwater in summer. This case report describes a 56-year-old adult male, who presented with increased intracranial pressure, headache, blurred vision accompanied by dizziness, periorbital edema, fever (38.5 ℃), nausea, vomiting, and chills. The patient had bathed in a wild hot spring one week prior to symptom onset. In this case, we pe

Identification of Potential Therapeutic Agents for Primary Amebic Meningoencephalitis Using Text Mining and Bioinformatics Analyses.

Naegleria fowleri, the brain-eating ameba, causes primary amebic meningoencephalitis (PAM), a fatal infectious disease that affects the central nervous system (CNS). We aimed to evaluate the functions and potential drugs targeting PAM using text mining and bioinformatics analyses. PAM-associated genes were identified using a disease database and mined from literature. To identify candidate drugs targeting PAM, 218 genes were analyzed using PanDrugs, drug Manually Annotated Targets and Drugs Online Resource (MATADOR), and the drug Comparative Toxicogenomics Database (CTD) by text mining. Acanthamoeba castellanii: Non-Steroidal Anti-Inflammatory Drugs Affect Adhesion, Motility, and Encystment, Suggesting a Link with a gp63-like Protein Candidate.

Acanthamoeba castellanii, an opportunistic free-living amoeba, causes severe infections including Acanthamoeba keratitis. This exploratory study evaluated whether three non-steroidal anti-inflammatory drugs (NSAIDs)-acetylsalicylic acid, ibuprofen, and diclofenac (100 µM)-modulate pathogenicity-related processes in A. castellanii and explored the involvement of a gp63-like protein during encystment and adhesion. Trophozoites were continuously exposed to each drug and analyzed for adhesion, migration on host-derived discontinuous brain micropatterns, encystment efficiency, and parasite-induced

Repurposing CNS drugs Zonisamide and Perampanel conjugated with silver nanoparticles as an anti-amoebic agent in combating Granulomatous Amebic Encephalitis (GAE) and Primary Amebic Meningoencephalitis (PAM) infection caused by Acanthamoeba castellanii and Naegleria Fowleri.

Acanthamoeba castellanii (A. castellanii) and Naegleria fowleri (N. fowleri) are free-living amoebae that cause Granulomatous Amoebic Encephalitis (GAE) and Primary Amoebic Meningoencephalitis (PAM), respectively rare but often fatal central nervous system infections. Current treatment options are limited by poor blood-brain barrier (BBB) penetration and inconsistent therapeutic efficacy. In this study, we explored the repurposing of the central nervous system drugs zonisamide and perampanel, conjugated with silver nanoparticles (AgNPs), as a novel anti-amoebic strategy. Fatal granulomatous amoebic encephalitis due to Acanthamoeba sp. with bacterial co-infection in an immunocompetent critically ill patient: A case report and review of the literature.

INTRODUCTION: Free-living amoebae are common, ubiquitous environmental saprophytic protozoa that can cause granulomatous encephalitis and other serious neurological infections. Despite being uncommon, these infections primarily affect immunocompromised patients and have a poor prognosis if treatment and diagnosis are delayed. CASE PRESENTATION: We describe the case of a 48-year-old woman who was admitted to the hospital due to convulsions linked to a persistent fever, right-sided motor deficit, and consciousness issues. She had no prior medical history. An MRI of the brain showed nodular gran

Primary Amebic Meningoencephalitis Mimicking Acute Bacterial Meningitis: Survival Following Early Empirical Therapy.

Primary amebic meningoencephalitis (PAM) is a rare but rapidly fatal infection of the central nervous system caused by the thermophilic free-living amoeba Naegleria fowleri. The disease often presents with clinical and cerebrospinal fluid (CSF) findings that closely mimic acute bacterial meningitis, making early diagnosis particularly challenging. We report the case of a 29-year-old woman with poorly controlled type 1 diabetes who presented with acute fever, severe headache, vomiting, and meningeal signs. Within 24 hours, her condition progressed to visual and auditory hallucinations, generali

Host factors, inflammatory markers, and clinical outcomes of Naegleria fowleri meningoencephalitis.

BACKGROUND: Primary amoebic meningoencephalitis (PAM) caused by Naegleria fowleri carries historical case fatality rates (CFR) exceeding 97%. The 2025 Kerala outbreak, the largest documented globally, provided an unprecedented opportunity to identify host factors and inflammatory correlates influencing survival under standardised management. METHODS: We conducted a prospective observational study of 200 laboratory-confirmed PAM cases across six districts of Kerala, India (January-November 2025). All patients received protocolised amphotericin B ± miltefosine. Demographic, clinical, and labora

Identification and Characterization of New Therapeutic Candidates for Naegleria fowleri- brain-eating Amoeba: A Multi-target Approach Based on 3D-QSAR, Molecular Docking, MM-GBSA, ADMET and Molecular Dynamics.

BACKGROUND: Naegleria fowleri is the most destructive and common brain-eating amoeba because it badly affects the human Central Nervous System (CNS), when amoeba in water goes up into the nose. N. fowleri also leads to PAM (primary amoebic meningoencephalitis) in humans. The lack of effective therapies for the treatment of PAM is a great drawback. The synthesis of new drug candidates for the treatment of this disease is time consuming and costly process. AIMS & OBJECTIVES: Therefore, to reduce time and cost, already FDA-approved brain cancer drugs can be repurposed to get an effective drug fo

Unmasking Balamuthia mandrillaris Through PCR in a Case of Multifocal Brain Lesions.

Balamuthia mandrillaris is a rare free-living amoeba that causes granulomatous amoebic encephalitis (GAE), a frequently fatal central nervous system infection. Diagnosis is often delayed because of nonspecific clinical presentation and radiographic findings. We describe a 70-year-old man with recent soil exposure in rural Mexico who developed progressive neurologic decline and multifocal enhancing brain lesions initially concerning for malignancy. Extensive infectious, oncologic, and autoimmune evaluations were unrevealing. Brain biopsy demonstrated dense granulomatous inflammation without ide

Primary amoebic meningoencephalitis: discovery and understanding of a novel disease.

Primary amoebic meningoencephalitis (PAM) or "brain-eating amoeba disease" is a relatively rare but typically fatal infectious disease of increasing global concern. Discovery of PAM and that Naegleria fowleri is the causative agent responsible are frequently credited to a paper published by the Australian pathologists Malcolm Fowler and Rodney F. Carter in 1965. It was significant as confirmation of an earlier suspicion that free-living, non-parasitic amoebae may be opportunistic pathogens of the human central nervous system. Like many discoveries, however, the historical details are rather mo
