Acquired Reactive Perforating Collagenosis-A Rare Entity Occurring Within Common Disorders: A Systematic Review and Our Personal Experience.

Background/Objectives: Acquired reactive perforating collagenosis (ARPC) is a rare entity usually occurring in adults with systemic diseases such as diabetes mellitus, chronic kidney disease (CKD), cardiovascular diseases, and malignancies, although drug-related and trauma-induced cases have also been reported. Given its rarity and the lack of consensus on optimal management, we conducted a systematic review to summarize updated diagnostic and therapeutic insights into ARPC. Additionally, we report a case of ARPC associated with CKD. Methods: This study was conducted in accordance with the PRI

Acquired Reactive Perforating Collagenosis Complicated by Diabetes Mellitus and Hypertension: A Case Report.

INTRODUCTION: Acquired reactive perforating collagenosis (ARPC) is a rare cutaneous disorder frequently associated with systemic diseases such as diabetes mellitus and chronic kidney disease. Its diagnosis remains challenging due to clinical overlap with other perforating dermatoses, necessitating advanced diagnostic tools for confirmation. CASE PRESENTATION: A 55-year-old male with a 15-year history of diabetes mellitus and hypertension developed progressively pruritic papules and nodules on both lower limbs over 2 months. Initial misdiagnosis as a fungal infection led to unsuccessful antifu

Acquired Perforating Dermatosis With Giant Lesions on the Scalp: A Unique Presentation in a Hemodialysis Patient.

Acquired perforating dermatosis is a rare skin disease characterized by umbilicated hyperkeratotic papules or nodules and transepidermal elimination of dermal components such as collagen and elastic fibers. It has been reported in association with various systemic disorders, particularly chronic renal failure and diabetes mellitus. Common locations include the extensor extremities and trunk. We describe a patient who developed multiple lesions on the scalp only, an unusual and rarely described location.

A Case of Noonan Syndrome and Kyrle's Disease: Coincidence or Causality?

A 39-year-old Caucasian woman affected by Noonan Syndrome (NS) mutated in RAF1 was referred to us with itchy lesions on her limbs that had appeared two months earlier. Clinically, there were multiple umbilicated papules with a hyperkeratotic central plug, localized on the upper and lower limbs (Figure 1, a-b). The patient had no personal history of diabetes mellitus and no chronic renal failure, but suffered from hypertrophic cardiomyopathy. Blood tests showed no abnormalities. On histological examination of a skin lesion, an ectatic hair follicle with hyperkeratotic ostium was observed with f

An Unusual Case of Kyrle Disease.

Kyrle disease is one of the acquired perforating disorders (APDs), commonly associated with type 2 diabetes, chronic kidney disease, and other pruritic conditions. Here, we report a case of Kyrle disease with characteristic transepidermal elimination of dermal contents on histopathology. However, the only abnormal laboratory finding in our patient was hyperuricemia, and none of the commonly associated underlying conditions were found. The precise etiopathogenesis of APD is poorly understood. Abnormal glycation of collagen in diabetes and defective follicular keratinization following chronic fr

Perforating Gout: Expanding the Differential for Transepidermal Elimination.

Perforating dermatoses are dermatologic disorders with transepidermal elimination (TE) of dermal substances. While TE is typically associated with collagen and elastin, it can also occur as a secondary event in other processes, and it is important to keep a broad differential. We present a case of perforating tophaceous gout, which underscores the need for a thoughtful approach to perforating disorders. An updated review of recent literature is also presented.

Perforating pseudoxanthoma elasticum with secondary elastosis perforans serpiginosa-like changes: dermoscopy, confocal microscopy and histopathological correlation.

Pseudoxanthoma elasticum is a rare congenital inherited elastolytic disorder that has sometimes been observed in association with transepidermal elimination of altered and calcified elastic fibers resulting in elastosis perforans serpiginosa-like changes. In this case, histopathology is usually performed to rule out other conditions. The case of a 38-year-old woman with two slowly enlarging asymptomatic plaques occurring on the neck and surrounded by coalescing yellowish papules with a typical cobblestone appearance, evaluated by polarized light dermoscopy and reflectance confocal microscopy w

Clinicopathological features of 25 patients with acquired perforating dermatosis.

BACKGROUND: Acquired perforating dermatosis (APD) is a rare group of skin disorders characterized by transepidermal elimination of dermal tissue material. There are only a few studies of APD. OBJECTIVE: We aimed to review the clinicopathologic features of patients with APD and evaluate the outcomes of treatments. MATERIAL AND METHODS: The medical records of 25 patients who were clinically and histopathologically diagnosed as having APD between 2006 and 2013 were retrospectively reviewed. The histopathologic sections and the results of elastic, trichrome and periodic acid Shiff (PAS) staining

Elastosis Perforans Serpiginosa in a patient with Down syndrome treated with imiquimod 5% cream.

Elastosis Perforans Serpiginosa (EPS) is a rare skin disease characterised by hyperkeratotic papules, transepidermal elimination of abnormal elastic fibres, and focal dermal elastosis. The aetiology is unknown, but an association with underlying systemic disorders, including Down syndrome has been described. Treatment is often difficult. A 45-year old man with Down syndrome presented with symmetrical annular elements on forearms and femora. The elements were erythematous with atrophic hypopigmented central healing and peripherally, infiltrated keratotic papules with desquamation. A punch biops

Perforating disorder caused by salt-water application and its experimental induction.

BACKGROUND: Perforating disorders are uncommon diseases characterized by transepidermal elimination histopathologically and include reactive perforating collagenosis, elastosis perforans serpiginosa, Kyrle's disease and perforating folliculitis. In addition, perforating disorders can develop in patients with diabetes mellitus, renal failure and even by accidental exposure of calcium salts. METHODS: We report two cases of perforating disorder caused by chemical burn with commercially available salt-water application for self-treatment of chronic dermatitis or pruritus. RESULTS: The commercial

Familial elastosis perforans serpiginosa.

BACKGROUND: Elastosis perforans serpiginosa (EPS) is an uncommon skin disease characterized by transepidermal elimination of abnormal elastic fibers. The disease is frequently associated with congenital connective tissue disorders or Down's syndrome. The pathogenesis of EPS is still unclear. There are a few reports in the literature about a familial occurrence of EPS in which different modes of inheritance are suggested. To support the hypothesis of a congenital origin of the disease, we have studied another family with EPS. OBSERVATIONS: In this study, we describe a family in which two siste

Primary localized cutaneous amyloidosis.

Fifty-seven patients with primary localized cutaneous amyloidosis (PLCA) were clinically and histopathologically reviewed. Two-thirds of patients had macular amyloidosis (MA). Intermediate cases having macular lesions with micropapules and/or lichens were identified. A spectrum for the disease is proposed, in which the less itchy classical macular variant occurs at one end and the very pruritic traditional lichen variant at the other. The salient histopathologic findings were similar in the macules, micropapules, and lichens, but were more prominent in the lichens. These consisted of hyperkera

[Perforating cutaneous amyloidosis].

Two patients, a 79-year-old man suffering from familial lichen amyloidosus and a 44-year-old women with biphasic cutaneous amyloidosis, developed small, centrally ulcerated papules. Histological and ultrastructural examination confirmed transepidermal elimination of amyloid. In contrast to other transepidermal elimination diseases, the material eliminated in perforating cutaneous amyloidosis is of epidermal origin.

Granuloma annulare with follicular perforation.

An instance of perforating granuloma annulare was observed in a 42-year-old female patient in which the necrobiotic material in the dermis was eliminated by a hair follicle. This had previously been described by the author in only 1 other case of perforating granuloma annulare. Transepithelial elimination may occur by the transepidermal and/or transfollicular route. Both pathways are possible in perforating granuloma annulare. Some factors are discussed which may be responsible for the predominance of one route over the other in various disorders of transepithelial elimination.
