A randomized clinical study comparing a hydrocellular dressing to a hydrocolloid dressing in the management of pressure ulcers.

This study compared an adhesive hydrocellular dressing with a leading hydrocolloid dressing in the management of pressure ulcers. Forty adult patients of both sexes who had Stage II or III pressure ulcers (according to the Agency for Health Care Policy and Research system) were enrolled in the study and were randomized to either the hydrocellular or hydrocolloid dressing. Dressing changes were done as required, and each ulcer was assessed on a weekly basis. Patients were followed for 8 weeks, until ulcer closure was achieved, or until the patient was withdrawn from the study, whichever occurre

Clinical efficacy and cost-effectiveness of a new synthetic polymer sheet wound dressing.

Stage II and III pressure ulcers present product development and product choice challenges to manufacturers and professional wound care clinicians respectively. We evaluated the clinical performance and cost of use associated with a new synthetic polymer dressing for the management of these wounds. A total of 10 home healthcare patients, each with a Stage II or III pressure ulcer, were enrolled and randomized for wound treatment using either the new polymer hydrogel wound dressing or the leading market hydrocolloid dressing. Dressings were changed on an as needed basis only. The wounds were as

The clinical and cost effectiveness of externally applied negative pressure wound therapy in the treatment of wounds in home healthcare Medicare patients.

Pressure ulcers, a devastating and costly healthcare problem, often occur in home healthcare settings. We sought to determine if these and other chronic wounds treated at home with negative pressure wound therapy close faster and reduce treatment costs compared to conventional therapies. Records for 1,032 Medicare home healthcare patients with 1,170 wounds that failed to respond to previous interventions--and were subsequently treated with negative pressure wound therapy--were reviewed. Reductions in wound area and volume were compared to rates reported by Ferrell in 1993, and costs were analy

Description of pressure ulcer pain at rest and at dressing change.

PURPOSE: To describe and compare the pain experienced by patients with stage II, III, and IV pressure ulcers both at rest and during dressing change. PATIENTS AND SETTING: Thirty-two subjects were recruited from acute, extended, and home care settings in the Midwest. The sample population ranged in age from 47 to 95 years (mean age 74.7 years, SD = 12.8), were white, and evenly divided between male and female patients. METHODS: Patients used the McGill Pain Questionnaire to rate their pressure ulcer pain at rest and again at dressing change. RESULTS: Twenty-eight subjects (87.5%) reported p

Becaplermin.

Becaplermin is a recombinant human platelet-derived growth factor B-chain homodimer (rh-PDGF-BB) for topical treatment of full-thickness diabetic neuropathic ulcers and pressure ulcers. rh-PDGF-BB promotes wound healing, probably by increasing granulation tissue formation via increased migration and proliferation of cells, with subsequent extracellular matrix deposition. Accelerated healing has been demonstrated in healthy and healing-impaired animals and in patients with lower extremity diabetic neuropathic ulcers and pressure ulcers. Pharmacokinetic studies in humans have shown that systemic

Evaluating results of laparoscopic surgery for esophageal achalasia.

BACKGROUND: Extramucosal myotomy of the lower esophagus and cardia, combined with anterior fundoplication, is, in our opinion, the procedure of choice to treat stage I-III esophageal achalasia. METHODS: After a successful experience with open surgery in over 280 patients, from January 1992 through February 1997, 61 patients underwent laparoscopic Heller-Dor for stage I-III achalasia. Conversion to laparotomy was done in three cases. All procedures were performed under intraoperative endoscopic control. Intraoperative complications were seven mucosal tears, which were sutured laparoscopically

Effects of hydrotherapy on pressure ulcer healing.

Pressure ulcers are a prevalent and potentially serious medical problem encountered in both the medical and rehabilitation settings. Because the progress of rehabilitation is often interrupted by the presence of pressure ulcers, the efficient care of these wounds is of great interest to the rehabilitation team. Patients in two acute care facilities with Stage III or IV pressure ulcers were identified and consented to participate in the study contained herein. All wounds were mechanically debrided of necrotic tissue, and then the patients were randomly assigned to the conservative treatment gro

The importance of repeated risk assessment for pressure sores in cardiovascular surgery.

BACKGROUND: Patients undergoing cardiovascular surgery are at high risk for sores because of impaired perfusion, the time spent on the operating room table, and restricted mobility in the immediate postoperative period. OBJECTIVE: To identify risk factors for sores. METHODS: In a 900-bed teaching hospital, 163 patients who underwent cardiovascular interventions were enrolled. Risk measurement included skin assessment, Braden and Norton scales, physic and biologic data and specific risk factors. The development of the most severe stages of pressure sores was followed (Stages II and III). RES

Pressure ulcers. Local wound care.

Local care of pressure ulcers includes wound cleansing, débridement, and dressings. Wound cleansing should remove loose debris and exudate but should not damage viable tissue. Saline irrigation is the standard. Débridement is often necessary for Stage III and IV pressure ulcers and can be performed autolytically, mechanically, enzymatically, or sharply. Prompt débridement is essential for infected wounds. Dressings should keep the wound bed continuously moist, should not be toxic to granulation tissue, and should keep the surrounding intact skin dry. Randomized, controlled clinical trials are

The usefulness of topical application of essential fatty acids (EFA) to prevent pressure ulcers.

This study investigated whether the topical application of essential fatty acids improves hydration and elasticity and helps prevent skin breakdown in individuals with poor nutritional status. Between June 1995 and July 1996, 86 patients, equally divided into two groups, underwent double-blind research (mean age = 60; range 26-78). All patients had a Norton Scale score of 9 and were fed orally a high-protein diet and/or received parenteral nutrition (92% were severely malnourished). Pressure ulcer prevention was the same for both groups. Every 8 hours, approximately 20 ml of solution A (1.6 gr

Fourth national pressure ulcer prevalence survey.

The fourth national pressure ulcer prevalence survey was conducted on November 9, 1995, with stuff at 265 acute-care hospitals surveying 39,874 patients for the presence of pressure ulcers. Data were collected on patient demographics, ulcer site, ulcer stage, and support surface. The goal was to determine the aggregate prevalence of pressure ulcers and to compare the results to those of the previous three surveys. The overall prevalence was 10.1% (range 1.4% to 36.4%), with the sacrum and heels the most common pressure ulcer sites. The predominant age group of patients with pressure ulcers was

Policy implications of using reverse staging to monitor pressure ulcer status.

In 1995, the National Pressure Ulcer Advisory Panel held its Fourth National Conference, "Pressure Ulcer Healing: Controversy to Consensus, Assessment Methods and Outcomes." At that time, agreement was reached on uses and misuses of the current pressure ulcer staging system. Participants agreed that pressure ulcer staging definitions should not be used in reverse order to measure improvement in an ulcer. Negative outcomes of reverse staging were seen as (1) denial of acute or skilled care after Stage IV ulcers were restaged as Stage II ulcers; (2) withdrawal of pressure-reducing support surfac
