Practice guidelines for the prediction and prevention of pressure ulcers: evaluating the evidence.

Clinical practice guidelines for the prediction and prevention of pressure ulcer (Agency for Health Care Policy and Research [AHCPR], 1992) were evaluated in a Canadian, university-affiliated, acute care hospital. Through a prospective study, the prevalence of pressure ulcers was determined, and pressure ulcer incidence was tracked to evaluate the accuracy of the Braden Scale for risk assessment. The prevalence rate for stage II or greater pressure ulcers was 13.6%; the rate was 29.7% when stage I (persistent redness) was included. In evaluating the Braden Scale's accuracy in predicting risk,

Efficacy of hydrocolloid occlusive dressing technique in decubitus ulcer treatment: a comparative study.

The efficacy of hydrocolloid occlusive dressing technique was compared with that of the conventional wet-to-dry gauze dressing technique in decubitus ulcer of stage I and II. Forty-four patients were randomly divided into two treatment groups and each received treatment according to the two different protocols. As a result, 80.8% of the hydrocolloid occlusive dressing group (group 1) and 77.8% of the conventional wet-to-dry gauze dressing group (group 2) healed completely with no statistically significant difference between the two groups. However, the time required for complete healing was sh

Implication of cytokines in the aggravation of malnutrition and hypercatabolism in elderly patients with severe pressure sores.

The objectives of this work were to study the production of the cytokines Interleukin 1 (IL-1), Interleukin 6 (IL-6), and tumour necrosis factor (TNF) in elderly patients with severe pressure sores and to assess their potential contribution to the aggravation of malnutrition. Nineteen bedridden patients with stage III or IV pressure sores, 12 bedridden patients free from pressure sores, but at risk of them, and 12 control patients without risk of pressure sores were studied. Nutritional status was evaluated using anthropometry, serum albumin, prealbumin, retinol-binding protein analyses, and d

The use of the Modified Norton Scale in nursing-home patients.

Patients in three nursing-home wards in Sweden were in 1991 assessed by the Modified Norton Scale (MNS). Of the 71 patients, 38 (49%) were considered to be at risk of developing pressure sores. Six of the patients had a total of eight pressure sores. Five sores were assessed as stage II, two sores as stage III and one sore as stage IV. Only two of the six patients with pressure sores had any preventive equipment in their beds. The nursing and medical documentation of the existing pressure sores was not satisfactory. Individual programs for skin care and routine assessment with a pressure sore

Managing sacral pressure ulcers with hydrocolloid dressings: results of a controlled, clinical study.

One-hundred and three patients with Stage II and III sacral pressure ulcers were enrolled in a prospective, controlled, multi-center clinical study to evaluate and compare dressing performance, safety and efficacy. Fifty-two patients were randomized to treatment with a triangle-shaped hydrocolloid border dressing and 51 patients were randomized to a different, oval shape, hydrocolloid dressing. The majority of patients (70 percent) utilized a pressure reducing mattress or bed. Most ulcers were Stage II, had existed for < 1 month and exhibited no change utilizing previous treatments. Patients a

Wound staging: can nurses apply classroom education to the clinical setting?

Traditionally, education on wound staging has been conducted in the classroom using drawings, photographs or slides to illustrate examples of wound stages. These methods portray wounds two-dimensionally, but clinically, wounds are three-dimensional. Seven home care nurses in central Florida were given a pre-test and post-test of 16 slides, four of each stage. Field visits with three of these nurses were then conducted by an ET nurse to evaluate the application of this education into clinical practice in the home setting. The questions studied were: Do nurses learn to accurately assess stages o

Pressure ulcers: the Minimum Data Set and the Resident Assessment Protocol.

As Americans age and live longer with chronic disabilities, more elderly citizens will reside in nursing homes. The nationally mandated Minimum Data Set (MDS) and Resident Assessment Protocol (RAP) were designed to help plan for the care of the frail elderly residing in nursing homes. One of the areas specifically targeted was pressure ulcers. This study was designed to describe the prevalence, incidence, and current management practices relating to pressure ulcers in nursing homes based on MDS data. In addition, a cross-sectional investigation was performed to examine the MDS items found in t

[Recurrent bleeding from colonic varices in portal hypertension. The successful prevention of recurrence by the implantation of a transjugular intrahepatic stent-shunt (TIPS)].

HISTORY AND CLINICAL FINDINGS: Sclerotherapy was performed in a 52-year-old patient with alcoholic liver cirrhosis (Child-Pugh stage A) for recurrent bleeding from oesophageal varices. Half a year later he again was admitted to hospital because of recurrent passage of bloody stools. The cardiovascular status was stable; the liver was enlarged by 15 cm in the medioclavicular line. INVESTIGATIONS: Endoscopy revealed several varices in the colon near the right flexure. One of the varices had an ulcer of 5 mm size. Duplex sonography revealed portal hypertension with cirrhosis of the liver and par

Epidural spinal cord stimulation in the treatment of severe peripheral arterial occlusive disease.

Epidural spinal cord stimulation (ESCS) has been suggested to improve microcirculatory blood flow and reduce amputation rates in patients with severe peripheral arterial occlusive disease (PAOD). Pain relief, limb salvage, and skin circulation were studied in 177 patients with ischemic pain caused by nonreconstructible PAOD who were receiving ESCS. Medical or surgical therapy had failed and vascular reconstruction was impossible in all cases. Clinical status was classified as Fontaine's stage III (chronic ischemic rest pain) in 114 patients and Fontaine's stage IV (ischemic pain and ulcers or

Sacral ulcers following cardiac surgery: incidence and risks.

This study examined the incidence and risk factors that relate to the development of sacral pressure ulcers following elective cardiac surgery. Perioperative variables were assessed using 136 adult patients. Ulcers, including 16 Stage I and 21 Stage II-III, occurred on 27.2% (n = 37) of the patients. Factors such as older age, transfer from another hospital, and a history of diabetes, were associated with ulcer development. These findings may be used to identify patients at risk for pressure ulcers during cardiac surgery.

Pressure ulcers: quality management, prevalence, and severity in a long-term care setting.

The entire resident population of a long-term care (LTC) facility was followed for four years. Protocols for quality management reduced the prevalence rate of pressure ulcers from 7% in 1988 to 4% in 1992. The average percentage of residents who developed pressure ulcers in the LTC setting was 3.4% while 20% of the residents admitted from the acute care setting had pressure ulcers. Chi square statistics showed that the pressure ulcers originating in the acute care setting tended to be much more severe than pressure ulcers that developed in the LTC setting. The majority (59.7%) of pressure ulce
