A longitudinal study of risk factors associated with the formation of pressure ulcers in nursing homes.

OBJECTIVE: To determine risk factors associated with the formation of stage II-IV pressure ulcers in nursing homes. DESIGN: Since the incidence rate for pressure ulcer formation varies among nursing homes, the homes were divided into tertiles based on these rates. Pooled logistic regression was used to model which factors are associated with the formation of pressure ulcers in both high and low incidence homes. SETTING: 78 National HealthCorp nursing homes. SUBJECTS: We studied 4232 nursing home residents free of pressure ulcers on admission to a nursing facility and at 3-months follow-up.

National pressure ulcer prevalence survey.

In the 1993 survey, the prevalence of pressure ulcers was 11.1% in 177 hospitals. This is higher than the 9.2 prevalence rate Meehan found in 1989. The sacrum, with 38% (n = 2,168) of all reported ulcers, remains the most common site for pressure ulcer occurrence. Non-reactive hyperemia sites, Stage I, were responsible for most reported ulcers, at 46.95% (n = 2,839), while Stage II ulcers comprised 32.66% (n = 1,975). Patients between the ages of 70 and 89 years of age had 54% of the ulcers found. The average number of ulcers per patient was 1.73. Dark-skinned, African-American patients again

[Collagens and growth factors in heterotopic ossification].

Heterotopic Ossification (HO) occurs as a consequence of several diseases and of various forms of trauma. HO is particularly frequent in paraplegic patients with spinal cord lesions. It is obvious that extraskeletal cells are able to differentiate into an osteogenic direction. However, the mechanisms of the induction process of HO and the stimulating agents are not precisely known. A novel tool for studying the ossification process at the level of transcription is the technique of non-radioactive in situ hybridisation. Using digoxigenin labeled cDNA probes we investigated the distribution patt

Risk factors for and prevalence of pressure ulcers among hospitalized patients.

A pressure ulcer prevalence study was conducted at a large, university-based hospital in the Midwest to establish a baseline of information for the evaluation of equipment and the interventions used for pressure ulcer prevention. Presence and stage of pressure ulcers, demographic data, attending service, type of mattress surface, and patient classification scores were recorded. The Braden Scale was used to measure the patients' risk for the development of pressure ulcers. Fifty-three of 440 patients studied had 85 ulcers, a prevalence rate of 12% including stage I ulcers. Thirty-eight percent

Hospital-acquired morbidity on a neurology service.

Clinical services must monitor hospital-acquired morbidity, but what rates are expected specifically for neurology inpatients is not evident from published studies. We studied prospectively 1317 consecutive admissions to a neurology service in a university-affiliated city hospital from 1987 to 1990 and recorded all nosocomial infections, nosocomial pneumonia, and decubitus ulcers of stage III or IV. Over the 3-year period, 6.8% of patients had > or = 1 nosocomial infection (and almost half of these had a nosocomial bloodstream infection); 3.1% had > or = 1 case of nosocomial pneumonia; 1.2% de

Seeking quality care for patients with pressure ulcers.

Improving quality care for patients with pressure ulcers resulted in a two-year intensive effort to educate staff, keep morale high, standardize care plans by stage of ulcer, and participate in a comparison study of two support surfaces (Thera-Pulse, Kinetic Concepts and Geo-Matt, SpanAmerica). When all pressure ulcers were considered, the analysis of covariance revealed no statistically significant difference in the healing of pressure ulcers with respect to type of support surface used (F[1, 78] = 0.35, p > .05). For patients with stage III and IV pressure ulcers, the proportion of patients

Clinical practice guidelines. Pressure ulcers in adults: prediction and prevention. Panel for the Prediction and Prevention of Pressure Ulcers in Adults.

This clinical practice guideline makes specific recommendations for identifying at-risk adults and for defining early interventions for preventing pressure ulcers. The guideline may also be used to treat Stage I pressure ulcers (nonblanchable erythema of intact skin). These guideline recommendations are not intended as the basis of care of infants and children, nor do they apply to individuals with existing Stage II or greater pressure ulcers or to individuals who are fully mobile.

A comparison of the efficacy and cost-effectiveness of two methods of managing pressure ulcers.

To compare the efficacy and the cost-effectiveness of moist gauze dressings and a hydrocolloid wafer dressing (DuoDERM CGF), 70 patients with 97 pressure ulcers that were stage II and/or stage III were randomly assigned to one of two treatment methods: moist gauze dressings or hydrocolloid dressings. Efficacy was defined as the number of ulcers that completely healed. In this debilitated, poorly nourished group of patients, one ulcer completely healed in the moist gauze dressing group, and 11 healed in the hydrocolloid group. The per diem cost of the moist gauze dressing was $12.26; the per di

A multicenter study on the use of pulsed low-intensity direct current for healing chronic stage II and stage III decubitus ulcers.

BACKGROUND AND DESIGN: Pulsed low-intensity direct current (300 to 600 microA) has been used in a double-blind placebo multicenter study in the treatment of stage II and stage III chronic decubitus ulcers. RESULTS: Seventy-four ulcers were treated in four centers. Forty-three patients were selected for the experimental group, and 31 control subjects used the sham instrument (placebo group). In the treated group, 25 ulcers (58%) healed in 8 weeks, whereas in the placebo group, only one ulcer (3%) healed and most ulcers increased in size. Statistical analysis, based on surface area and ulcer de

Pressure ulcer prevalence, incidence and associated risk factors in the community.

This study examined the prevalence and incidence of pressure ulcers as well as associated risk factors in the community. By using the Web of Causation and the Braden conceptual schema, barriers to effective interventions were identified and analyzed. The Web of Causation for pressure ulcer development includes socioeconomic factors and personal belief systems. It also includes the following risk factors: mobility, activity, moisture, nutrition, friction, shear, and altered sensory perception. There were 103 participants in this study. Data gathered by public health nurses included occurrence,

Multi-center evaluation of a new wound dressing.

Ninety-two wounds were evaluated for at least three dressing changes each. The vast majority of the wounds were lightly exuding Stage II pressure ulcers and skin tears. This evaluation confirmed the hypothesis that the protective, conformable, and elastic properties of Flexzan, combined with its limited absorption capacity, high moisture vapor permeability and easy to use adhesive system, provided a dressing well suited to the management of the non- to lightly exuding wounds included. In addition, it was found to be a "superior" secondary dressing for alginate dressings on highly exuding wound

Pressure ulcers in community-resident persons with spinal cord injury: prevalence and risk factors.

The prevalence and correlates of pressure ulcers in terms of their number, severity, and anatomical location were studied in a community-based sample of 100 men and 40 women with spinal cord injury. Thirty-three percent (n = 46) presented with one or more ulcers of at least one stage I severity when visually examined. Twenty-one individuals had more than one ulcer, the maximum number of ulcers being seven. Of 87 ulcers for which severity ratings were available, 30 (34.5%) were stage I, 33 (37.9%) were stage II, and 24 (27.6%) were either stage III or IV. Individuals with an ulcer exhibited mor

A comparison of Epi-Lock and saline dressings in the treatment of pressure ulcers.

The use of Epi-Lock dressings was compared to moist saline dressings as a treatment for Stage II and III pressure ulcers in 38 patients. Demographic and clinical data were collected at the onset of the investigation. At the time of enrollment and during weeks six, 12 and 24, pressure ulcers were inspected and measured, and laboratory data were collected. More healing occurred with Epi-Lock dressing than with moist saline dressing for Stage II and III pressure ulcers. Although individual Epi-Lock dressings were more expensive than saline dressings, they required less dressing changes. Therefore

Health-care professionals' views of the effectiveness of pressure ulcer treatments. A survey among nursing-home physicians, dermatologists and nursing staff in The Netherlands.

In a survey, 237 nursing-home physicians, 113 dermatologists, and 164 supervisory nurses in nursing homes rated 30 treatments for stage III pressure ulcers by scoring their effectiveness. On average, there seems to be a fair degree of consensus between these groups of health-care workers about the effectiveness of most treatments. The most notable exceptions are ultraviolet light, ultrasound, and laser therapy and myocutaneous flap operations. Here, the dermatologists disagree with both nursing-home physicians and nurses. However, within all specialties there is disagreement about the efficacy

[Value of a "floppy" Nissen in the treatment of gastroesophageal reflux. Apropos of 117 cases].

The Nissen fundoplication is the most widely used antireflux procedure. However this operation is associated with several specific complications. To prevent these complications, an alternative antireflux procedure ("floppy" Nissen) has been designed. This investigation was conducted to investigate the effects of the floppy Nissen performed on 117 patients, between 1978 and 1990. Indication for surgery was the endoscopic discovery of stage I and II oesophagitis (59 cases), III and IV oesophagitis (25 cases) or an associated disease (gallstones: 25 cases, duodenal ulcer: 8 cases), pH monitoring

Neuromuscular stimulation in spinal cord injury. II: Prevention of secondary complications.

The spinal cord injured patient has been the focus of clinical and research efforts to restore functional movement and to obtain therapeutic benefits by electric stimulation of upper motor neuron paralyzed muscles. This review article treats developments in this field from 1983 to 1990. Efforts have been directed to restoring ventilatory and bladder function, and to preventing secondary complications. Electric stimulation for improving ventilatory function has had reasonable success, and systems are commercially available. Electric stimulation for improving bladder function remains in the rese
