Transcutaneous oxygen pressure as predictive parameter for ulcer healing in endstage vascular patients treated with spinal cord stimulation.

OBJECTIVE AND DESIGN: This study was designed to test the hypothesis that initial TcPO2 helps predict clinical outcome in vascular patients treated with spinal cord stimulation. A randomized-controlled study with one year follow-up was made in 86 Fontaine stage IV patients with endstage peripheral arterial occlusive disease (PAOD) undergoing 21 day intravenous prostaglandin E1 (PGE1) therapy for nonhealing ulcers. MATERIALS AND METHODS: All patients had arteriosclerosis, 13 also diabetes mellitus. Entry criteria included: non-reconstructible PAOD as proven by intra-arterial angiography or pat

The use of the Modified Norton Scale in nursing-home patients.

Patients in three nursing-home wards in Sweden were in 1991 assessed by the Modified Norton Scale (MNS). Of the 71 patients, 38 (49%) were considered to be at risk of developing pressure sores. Six of the patients had a total of eight pressure sores. Five sores were assessed as stage II, two sores as stage III and one sore as stage IV. Only two of the six patients with pressure sores had any preventive equipment in their beds. The nursing and medical documentation of the existing pressure sores was not satisfactory. Individual programs for skin care and routine assessment with a pressure sore

A literature assessment of the use of miscellaneous topical agents, growth factors, and skin equivalents for the treatment of pressure ulcers.

BACKGROUND: Agents reviewed for this manuscript were both rare and popular topical treatments for pressure ulcers. OBJECTIVE: To review topical treatments for pressure ulcers and evaluate them based on available literature of controlled, blinded, and randomized trials. METHODS: A MEDLARS database (1966-1993) search and a thorough review of reference article lists of key articles produced over 100 manuscripts. Studies were considered for review if they were conducted on humans with chronic stage II to IV pressure ulcers, in a properly controlled and randomized fashion. RESULTS: The use of zi

Managing sacral pressure ulcers with hydrocolloid dressings: results of a controlled, clinical study.

One-hundred and three patients with Stage II and III sacral pressure ulcers were enrolled in a prospective, controlled, multi-center clinical study to evaluate and compare dressing performance, safety and efficacy. Fifty-two patients were randomized to treatment with a triangle-shaped hydrocolloid border dressing and 51 patients were randomized to a different, oval shape, hydrocolloid dressing. The majority of patients (70 percent) utilized a pressure reducing mattress or bed. Most ulcers were Stage II, had existed for < 1 month and exhibited no change utilizing previous treatments. Patients a

Wound staging: can nurses apply classroom education to the clinical setting?

Traditionally, education on wound staging has been conducted in the classroom using drawings, photographs or slides to illustrate examples of wound stages. These methods portray wounds two-dimensionally, but clinically, wounds are three-dimensional. Seven home care nurses in central Florida were given a pre-test and post-test of 16 slides, four of each stage. Field visits with three of these nurses were then conducted by an ET nurse to evaluate the application of this education into clinical practice in the home setting. The questions studied were: Do nurses learn to accurately assess stages o

Pressure ulcers: the Minimum Data Set and the Resident Assessment Protocol.

As Americans age and live longer with chronic disabilities, more elderly citizens will reside in nursing homes. The nationally mandated Minimum Data Set (MDS) and Resident Assessment Protocol (RAP) were designed to help plan for the care of the frail elderly residing in nursing homes. One of the areas specifically targeted was pressure ulcers. This study was designed to describe the prevalence, incidence, and current management practices relating to pressure ulcers in nursing homes based on MDS data. In addition, a cross-sectional investigation was performed to examine the MDS items found in t

[Recurrent bleeding from colonic varices in portal hypertension. The successful prevention of recurrence by the implantation of a transjugular intrahepatic stent-shunt (TIPS)].

HISTORY AND CLINICAL FINDINGS: Sclerotherapy was performed in a 52-year-old patient with alcoholic liver cirrhosis (Child-Pugh stage A) for recurrent bleeding from oesophageal varices. Half a year later he again was admitted to hospital because of recurrent passage of bloody stools. The cardiovascular status was stable; the liver was enlarged by 15 cm in the medioclavicular line. INVESTIGATIONS: Endoscopy revealed several varices in the colon near the right flexure. One of the varices had an ulcer of 5 mm size. Duplex sonography revealed portal hypertension with cirrhosis of the liver and par

Pressure ulcers in extended care facilities: report of a survey.

A survey including 15 extended care facilities with access to ET nurse consulting services was conducted to assess current wound care protocols, practices, and educational backgrounds as well as requirements. Sixty-seven percent of facilities surveyed used a system to describe pressure ulcers. Wound care was provided by licensed practical nurses in 90% of the institutions and product decisions were made by the physician in 65% of facilities surveyed. Gauze-type dressings were used most frequently on all stages and types of pressure ulcers. A significant correlation between ulcer type and dress

Sacral ulcers following cardiac surgery: incidence and risks.

This study examined the incidence and risk factors that relate to the development of sacral pressure ulcers following elective cardiac surgery. Perioperative variables were assessed using 136 adult patients. Ulcers, including 16 Stage I and 21 Stage II-III, occurred on 27.2% (n = 37) of the patients. Factors such as older age, transfer from another hospital, and a history of diabetes, were associated with ulcer development. These findings may be used to identify patients at risk for pressure ulcers during cardiac surgery.

Pressure ulcers: quality management, prevalence, and severity in a long-term care setting.

The entire resident population of a long-term care (LTC) facility was followed for four years. Protocols for quality management reduced the prevalence rate of pressure ulcers from 7% in 1988 to 4% in 1992. The average percentage of residents who developed pressure ulcers in the LTC setting was 3.4% while 20% of the residents admitted from the acute care setting had pressure ulcers. Chi square statistics showed that the pressure ulcers originating in the acute care setting tended to be much more severe than pressure ulcers that developed in the LTC setting. The majority (59.7%) of pressure ulce

A longitudinal study of risk factors associated with the formation of pressure ulcers in nursing homes.

OBJECTIVE: To determine risk factors associated with the formation of stage II-IV pressure ulcers in nursing homes. DESIGN: Since the incidence rate for pressure ulcer formation varies among nursing homes, the homes were divided into tertiles based on these rates. Pooled logistic regression was used to model which factors are associated with the formation of pressure ulcers in both high and low incidence homes. SETTING: 78 National HealthCorp nursing homes. SUBJECTS: We studied 4232 nursing home residents free of pressure ulcers on admission to a nursing facility and at 3-months follow-up.

National pressure ulcer prevalence survey.

In the 1993 survey, the prevalence of pressure ulcers was 11.1% in 177 hospitals. This is higher than the 9.2 prevalence rate Meehan found in 1989. The sacrum, with 38% (n = 2,168) of all reported ulcers, remains the most common site for pressure ulcer occurrence. Non-reactive hyperemia sites, Stage I, were responsible for most reported ulcers, at 46.95% (n = 2,839), while Stage II ulcers comprised 32.66% (n = 1,975). Patients between the ages of 70 and 89 years of age had 54% of the ulcers found. The average number of ulcers per patient was 1.73. Dark-skinned, African-American patients again

[Collagens and growth factors in heterotopic ossification].

Heterotopic Ossification (HO) occurs as a consequence of several diseases and of various forms of trauma. HO is particularly frequent in paraplegic patients with spinal cord lesions. It is obvious that extraskeletal cells are able to differentiate into an osteogenic direction. However, the mechanisms of the induction process of HO and the stimulating agents are not precisely known. A novel tool for studying the ossification process at the level of transcription is the technique of non-radioactive in situ hybridisation. Using digoxigenin labeled cDNA probes we investigated the distribution patt

Risk factors for and prevalence of pressure ulcers among hospitalized patients.

A pressure ulcer prevalence study was conducted at a large, university-based hospital in the Midwest to establish a baseline of information for the evaluation of equipment and the interventions used for pressure ulcer prevention. Presence and stage of pressure ulcers, demographic data, attending service, type of mattress surface, and patient classification scores were recorded. The Braden Scale was used to measure the patients' risk for the development of pressure ulcers. Fifty-three of 440 patients studied had 85 ulcers, a prevalence rate of 12% including stage I ulcers. Thirty-eight percent

Seeking quality care for patients with pressure ulcers.

Improving quality care for patients with pressure ulcers resulted in a two-year intensive effort to educate staff, keep morale high, standardize care plans by stage of ulcer, and participate in a comparison study of two support surfaces (Thera-Pulse, Kinetic Concepts and Geo-Matt, SpanAmerica). When all pressure ulcers were considered, the analysis of covariance revealed no statistically significant difference in the healing of pressure ulcers with respect to type of support surface used (F[1, 78] = 0.35, p > .05). For patients with stage III and IV pressure ulcers, the proportion of patients
