Pilot study to determine the effectiveness of diamorphine gel to control pressure ulcer pain.

Pressure ulcer pain, a common problem among palliative care patients, does not respond well to oral analgesics. There have been case reports in the medical literature describing the successful use of topical opioids for painful skin conditions. So far, these topical opioids have not been compared to placebo. To determine the effectiveness of diamorphine gel to control pressure ulcer pain and compare it with placebo, a randomized, double blind, placebo-controlled crossover trial was conducted in 13 patients with painful grade II or III pressure ulcers. Patients resided on the inpatient unit at

Accuracy of a web-based system for monitoring chronic wounds.

This study evaluated the accuracy of a store-and-forward telemedicine system for assessing the status of chronic wounds, including those surgically repaired. Digital photos and other patient and wound data were collected by a nurse using a laptop and transmitted via the Internet to a database, which organized and posted the data onto a web page for access by the telemedicine physician. Two Veterans' Affairs (VA) medical centers and two specialties (plastic surgery, physical medicine and rehabilitation) participated in the study. Study patients included inpatients and outpatients with pressure

Interface pressure under a ready made compression stocking developed for the treatment of venous ulcers over a period of six weeks.

BACKGROUND: The most important component in the treatment of venous ulcers is compression therapy. The function of compression therapy is the pressure exterted on the extrafascial and intrafascial venous system. Therefore the interface pressure of specially designed compressions stockings for the treatment of venous ulcers are in the focus of interest. PATIENTS AND METHODS: We examined 20 patients (6 men and 14 women) with chronic venous insufficiency in CEAP stages CS0-4EPAS1-3PR). Interface pressure was measured under the Venotrain Ulcertec over an observation period of six weeks with piezo

An evaluation of an adhesive hydrocellular foam dressing and a self-adherent soft silicone foam dressing in a nursing home setting.

To assess the performance of two wound dressings in the management of chronic wounds, a 5-year retrospective descriptive study was conducted involving 4,200 wounds in 1,891 residents of 30 nursing homes in the state of Florida. Wound and patient variables were abstracted from charts completed by the wound care clinicians providing care in the nursing homes. The chart abstraction time period was chosen to capture a change in wound dressing regimens in these nursing homes. Patient demographic and wound assessment variables, including evidence of surrounding skin stripping information, were abstr

Efficacy of low-level laser therapy in the management of stage III decubitus ulcers: a prospective, observer-blinded multicentre randomised clinical trial.

Low-level laser therapy (LLLT) has been suggested as a promising treatment option for open wounds. In view of the absence of randomised studies with sufficiently large sample sizes, we assessed the efficacy of LLLT in the treatment of stage III decubitus ulcers. We performed a prospective, observer-blinded multicentre randomised clinical trial to assess the effect of LLLT as adjuvant to standard decubitus care. A total of 86 patients were enrolled into the study. Treatment was the prevailing consensus decubitus treatment (n=47); one group (n=39) had LLLT in addition, five times a week over a p

Efficacy of pulsed low-intensity ultrasound in wound healing: a single-case design.

Therapeutic ultrasound is used clinically to enhance healing of pressure ulcers. Limited clinical research is available and no consensus exists regarding the efficacy of ultrasound for treating pressure ulcers, particularly full-thickness pressure ulcers, in the elderly. To assess the efficacy of pulsed low-intensity ultrasound on wound healing, a double-blind, single-case, baseline-AB study was conducted. The participant, a patient in a skilled nursing facility, was a 75-year-old woman with a Stage III pressure ulcer over the coccyx. Pulsed low-intensity ultrasound was compared to placebo ult

Economic evaluation of noncontact normothermic wound therapy for treatment of pressure ulcers.

New adjunctive treatments for pressure ulcers have become available to complement standard care. The economic benefits of new advanced wound care treatments like noncontact normothermic wound therapy are related to: the costs of adequately providing standard care treatment, the baseline probability of healing a pressure ulcer to closure with standard care, the relative improvement in healing rates with the advanced wound care treatment and the acquisition cost of the advanced treatment. Healing data from preliminary clinical trials suggest that pressure ulcer healing in long-term care patients

Integration of pressure ulcer treatment protocol into practice: clinical outcomes and care environment attributes.

The purpose of this study was to determine if a research-based protocol for pressure ulcer treatment that had been successfully implemented in a long-term care facility was sustained over time. A secondary aim was to describe the attributes of the care environment that may have contributed to or impeded the maintenance of this protocol. A retrospective chart review was conducted of all patients who developed incident stage II, III, or IV pressure ulcers in the facility over a one-year period five years after initial implementation of a pressure ulcer treatment protocol. Data regarding ulcer ch

9 clinical cases of nonhealing pressure ulcers in patients with spinal cord injury treated with an anabolic agent: a therapeutic trial.

Anabolic steroid agents may potentially promote wound healing in individuals with spinal cord injury who have long-standing wounds. Nine patients hospitalized on the Spinal Cord Injury Service of the VA Medical Center, Bronx, NY, received treatment with an anabolic agent and amino acid supplement. Selection of patients was based on having at least 1 of the following criteria: (1) nonhealing pressure ulcer of at least 2 months' duration with no change or worsening status, and/or (2) full-thickness pressure ulcer through fascia into muscle, tendon, or bone. Previous and current pressure ulcer hi

Applying electric and electromagnetic energy as adjuvant treatment for pressure ulcers: a critical review.

Chronic pressure ulcers are a significant health problem especially in the aging population. National estimated annual treatment costs are in the billions of dollars. Only two treatment-related recommendations receive high ratings for reported experimental evidence of validity: Use of moist wound dressings and adjunctive electrotherapy for unresponsive Stage III and IV and recalcitrant Stage II ulcers. A critical literature review pertaining to electrotherapy reveals a myriad of electrical treatment modalities varying greatly in electric current type, strength, direction, frequency, waveform,

[Comparison of the effectiveness and cost of treatment with humid environment as compared to traditional cure. Clinical trial on primary care patients with venous leg ulcers and pressure ulcers].

INTRODUCTION: The discovery of moist environment dressings as alternatives to the traditional treatments based on exposing wounds to air, opened new expectations for the care and treatment of chronic wounds. Over the years, these expectations have led to the availability of new moist environment dressings which have made it possible to improve the care provided to patients suffering this kind of wounds, as well as providing important reasons to weigh in terms of cost-benefit-effectiveness at the time of selecting which type of treatment should be employed.
