Improving Pressure Ulcer Staging Accuracy Through a Nursing Student Experiential Intervention.

BACKGROUND: Staging of pressure ulcers (PUs) is often inaccurate. The purpose of this study was to evaluate the effectiveness of a student-focused intervention to improve PU staging accuracy in nursing students. METHOD: Nursing students from two universities were included. The control group (n = 103) received a standard lecture about PUs; the intervention group (n = 55) completed a PU module, participated in Skin Day at a local hospital, and received the standard lecture. All students completed a PU staging tool to determine their staging accuracy. Scores were compared using analysis of varia

A new concept: 'Relative position between the external force and the bony prominence' explains location-specific occurrence of superficial injury over an undermining lesion.

AIM: A pressure ulcer is localized injury to the skin and/or underlying tissue usually over a bony prominence, as a result of pressure, or pressure in combination with shear. Although the external forces and bony prominences differ depending on ulcer location, the way in which these anatomical differences affect pressure ulcer development is not well studied. METHODS: To clarify the location-dependent factors for pressure ulcer development, we focused on superficial injuries that develop over an undermining lesion, which we have termed them bilayer pressure ulcers. Because it is thought that

Are the classic signs of infection in concordance with results from percutaneous aspiration to diagnose infection in pressure injuries?

AIM: To explore the concordance between the presence of classic signs of infection and the positive diagnosis identified by the microbiological culture of fluid collected by percutaneous aspiration. BACKGROUND: The classic signs of infection are commonly applied in some contexts to confirm infection in pressure injuries, but its accuracy has been questioned in chronic wounds. Little is known about the concordance of this method with others, such as the deep fluid culture from pressure injuries collected by percutaneous aspiration. DESIGN: Multicentre, cross-sectional observational study. ME

Expanded Flaps in Surgical Treatment of Pressure Sores: Our Experience for 25 Years.

BACKGROUND: Because the ischial region is the main weight-bearing area in sitting, it is one of the areas most frequently affected by pressure ulcers in paraplegic patients resuming the sitting position during the subacute and chronic stages. The techniques described to date have not been able to reduce the high rates of recurrence and flap dehiscence. Other groups have described successful tissue expansion in the treatment of pressure ulcers, but to date, the long-term results of the procedure have not been reported. METHODS: The long-term follow-up of 138 reconstructions of the ischial regi

A review of the surgical management of heel pressure ulcers in the 21st century.

Heel ulceration, most frequently the result of prolonged pressure because of patient immobility, can range from the trivial to the life threatening. Whilst the vast majority of heel pressure ulcers (PUs) are superficial and involve the skin (stages I and II) or underlying fat (stage III), between 10% and 20% will involve deeper tissues, either muscle, tendon or bone (stage IV). These stage IV heel PUs represent a major health and economic burden and can be difficult to treat. The worst outcomes are seen in those with large ulcers, compromised peripheral arterial supply, osteomyelitis and assoc

Pressure injury in Australian public hospitals: a cost-of-illness study.

OBJECTIVE: Pressure injuries (PI) are largely preventable and can be viewed as an adverse outcome of a healthcare admission, yet they affect millions of people and consume billions of dollars in healthcare spending. The existing literature in Australia presents a patchy picture of the economic burden of PI on society and the health system. The aim of the present study was to provide a more comprehensive and updated picture of PI by state and severity using publicly available data. METHODS: A cost-of-illness analysis was conducted using a prevalence approach and a 1-year time horizon based on

Bodily pain intensity in nursing home residents with pressure ulcers: analysis of national minimum data set 3.0.

Clinical reports suggest that superficial pressure ulcers produce pain, but that pain decreases as the wound advances in stage. This study of the relationship between pressure ulcer stage and bodily pain intensity in nursing home residents was a secondary analysis of the national Minimum Data Set 3.0 assessment data in long-term care facilities, collected from nursing home residents at least 65 years of age. Data were examined from residents with pressure ulcers who completed a bodily pain intensity interview between January and March 2012 (N = 41,680) as part of the MDS comprehensive assessme

Recurrent ischial pressure ulcer resolved with a novel tissue adhesive: a case report.

INTRODUCTION: Patients with stage III and IV pressure ulcers requiring surgical reconstruction remain a challenge. Extended hospitalization, and high costs of care per patient episode due to high rates of complications and recurrence, make efforts to reduce these rates of utmost importance to the medical community in general. We report a case in which two prior attempts at surgical resolution had failed, and which was successfully resolved with the aid of a new tissue adhesive designed for the closure of dead space. To the best of our knowledge, this is the first reported example of the use of

Comparative effectiveness of quality improvement interventions for pressure ulcer prevention in academic medical centers in the United States.

BACKGROUND: Prevention of pressure ulcers, one of the hospital-acquired conditions (HACs) targeted by the 2008 nonpayment policy of the Centers for Medicare & Medicaid Services (CMS), is a critical issue. This study was conducted to determine the comparative effectiveness of quality improvement (QI) interventions associated with reduced hospital-acquired pressure ulcer (HAPU) rates. METHODS: In an quasi-experimental design, interrupted time series analyses were conducted to determine the correlation between HAPU incidence rates and adoption of QI interventions. Among University HealthSystem C

Hospital-Acquired Pressure Ulcers at Academic Medical Centers in the United States, 2008-2012: Tracking Changes Since the CMS Nonpayment Policy.

BACKGROUND: In 2007, the Centers for Medicare & Medicaid Services (CMS) announced its intention to no longer reimburse hospitals for costs associated with hospital-acquired pressure ulcers (HAPUs) and a list of other hospital-acquired conditions (HACs), which was followed by enactment of the nonpayment policy in October 2008. This study was conducted to define changes in HAPU incidence and variance since 2008. METHODS: In a retrospective observational study, HAPU cases were identified at 210 University HealthSystem Consortium (UHC) academic medical centers in the United States. HAPU incidence
