Not Just a Birthmark: A Case Report of a Subtle Port-Wine Stain Heralding Sturge-Weber Syndrome.

Sturge-Weber syndrome is a rare neurocutaneous disorder due to a somatic GNAQ mutation, which is characterised by facial capillary malformation, leptomeningeal angiomatosis, and possible ocular involvement. Identifying these faint birthmarks is important because even mild skin involvement may indicate significant underlying intracranial vascular malformations and early neurological complications. We report a seven-year-old boy with focal seizures beginning at two years of age, often progressing to generalised tonic-clonic events, and associated with intermittent left-sided headaches. Examinati

Type 3 Sturge-Weber Syndrome Presenting With Concurrent Epilepsy and Migraine.

Sturge-Weber syndrome is a rare congenital disorder with vascular malformations affecting the brain, skin, and eyes. Type 3 Sturge-Weber syndrome, the rarest form, presents with isolated leptomeningeal angiomas, often causing diagnostic delays. This report highlights an unusual presentation where both recurrent focal seizures and migraine-like headaches were prominent, long-standing features. A 30-year-old man presented with two focal seizures affecting the right side of his body, preceded by aura and followed by postictal confusion, right-sided weakness, and migraine-like headache with photop

Anetoderma Presenting as Neoplasms of Uncertain Behavior in an Elderly Female: A Case Report.

Anetoderma is a rare cutaneous disorder characterized by focal loss of dermal elastic tissue, often presenting as atrophic or herniated plaques. However, clinical presentations can vary, and atypical forms may resemble neoplastic or inflammatory lesions, complicating diagnosis. We present the case of a 78-year-old female who was evaluated for asymptomatic skin growths on the posterior neck, chest, and torso, reported to have been present for five years and slowly spreading. On physical examination, firm papules and subcutaneous nodules were noted on the left rib cage, left medial breast, and m

Clinically discordant siblings with spinal muscular atrophy: insights from their patient-specific iPSC-derived motor neurons and literature review.

Spinal muscular atrophy (SMA) is a neurodegenerative disease caused by homozygous loss of the SMN1 gene. Copy number of the nearly identical paralog, SMN2, correlates with disease severity. SMN2 is the primary modifier of SMA, with only a few other modifiers reported. We reviewed the literature of rare siblings affected by SMA that show discordance in clinical presentation despite having the same number of SMN2 copies, which predicts the presence of genetic modifiers. We further recruited a sibling pair with discordant clinical presentations and performed detailed characterization. We utilized

Hyperkeratosis Lenticularis Perstans (Flegel's Disease) in a Middle-Aged Man: A Rare Keratinization Disorder.

Flegel disease, also known as hyperkeratosis lenticularis perstans, is a rare cutaneous disease. Here, we report a case of a 56-year-old man who presented with a history of asymptomatic skin lesions that had gradually increased in number over the past year, in the absence of any significant medical history or known predisposing factors for Flegel disease. Skin examination revealed multiple punctate, scaly, blanchable, erythematous papules measuring 1-2 mm in diameter and distributed symmetrically on both lower extremities. Scales were adherent to the peripheral edges of the papules. Palms, sol

Parry-Romberg Syndrome Associated With En Coup De Sabre: A Clinical Case Report.

Parry-Romberg syndrome (PRS), also known as progressive facial hemiatrophy, is a rare neurocutaneous disorder characterized by progressive, unilateral atrophy of the facial tissues. It usually appears during childhood or early adulthood; however, adult-onset cases are increasingly being recognized. PRS significantly overlaps with localized scleroderma, especially the en coup de sabre (ECDS) subtype, indicating a common pathophysiological mechanism. We present a 52-year-old female patient with adult-onset PRS associated with ECDS. The patient was first diagnosed with localized scleroderma in 19

Cutaneous α-Synuclein and Age Spots in Neurodegeneration: A Systematic Review and Testable Hypothesis.

Recent studies have identified phosphorylated α-synuclein in the skin of individuals with neurodegenerative disease. The levels of this protein in the skin have been correlated with disease severity. This protein has been specifically studied in alpha-synucleinopathies such as Parkinsons' Disease (PD) and Multiple System Atrophy (MSA). Given that skin biopsy studies have often shown high levels of phosphorylated α-synuclein in the neck area, and that other clinical studies have described easily identifiable changes in facial skin features, this systematic review explores whether age spots, whi

A Case of Mandibular Osteomyelitis Occurring in a Patient With Parry-Romberg Syndrome.

Progressive hemifacial atrophy, also known as Parry-Romberg syndrome (PRS), is an idiopathic disorder characterized by progressive unilateral facial depression caused by atrophy of the skin and subcutaneous tissue. We present a case of mandibular osteomyelitis occurring in a patient with this condition, together with a review of the literature. The clinical findings in this case suggest that autoimmune disease and trauma are involved in contributing factors to disease progression. Possible involvement of neurovascular and neuroskeletal dysregulations in the pathogenesis of PRS is also discusse

Ulcerative Necrobiosis Lipoidica in the Setting of Epstein-Barr Virus-Positive Diffuse Large B-Cell Lymphoma and Glucose-6-Phosphate Dehydrogenase Deficiency.

Necrobiosis lipoidica (NL) is a chronic granulomatous disease most commonly manifesting as yellow, atrophic plaques and ulcerations on the lower extremities. We present an exacerbation of NL in a patient with a known history of glucose-6-phosphate dehydrogenase deficiency, who, following a clinical and histopathologic diagnosis of NL, was found to have oropharyngeal Epstein-Barr virus-positive diffuse large B-cell lymphoma. After implementation of therapies for lymphoma, chemotherapeutic complications, and NL, the patient reported improvement in both pain and ulcerations. Though noninfectious

AID Expression in Vulvar SCC Arising From Lichen Sclerosus et Atrophicus: A Retrospective Study.

Lichen sclerosus et atrophicus (LSA) is a chronic, progressive inflammatory disorder with a risk of malignant transformation to squamous cell carcinoma (SCC). This study investigated the role of activation-induced cytidine deaminase (AID) in UV-independent SCC arising from LSA. We retrospectively analyzed 19 female patients, including 5 SCC cases. AID expression was significantly associated with SCC development, pruritus, and shorter disease duration. AID was detected in SCC lesions and adjacent LSA tissues, mainly in basal cells. Reduced AID expression was observed in higher-stage tumors, sug

A phase 2b basket trial approach to treat multiple rare and fibrotic skin diseases.

Fibrotic skin diseases are rare, chronic, and often debilitating conditions characterized by excessive extracellular matrix deposition, leading to tissue scarring and functional impairment. Despite their severity, diseases-such as lichen sclerosus et atrophicus (LSA), frontal fibrosing alopecia (FFA), radiation-induced skin fibrosis (RISF), eosinophilic fasciitis (EF), pansclerotic disabling morphea (PDM), and linear circumscript sclerodermia (LCS)-lack approved therapies and are underrepresented in clinical research. This phase 2b multicenter basket trial proposes a novel approach to evaluate

Histopathological Characterization and Differential Expression of miRNAs in Male Pediatric Patients With Lichen Sclerosus.

BACKGROUND: Lichen sclerosus is a chronic, inflammatory, scarring disease of the skin that manifests mostly in the genital region. OBJECTIVE: We studied the histomorphological characteristics, grade, and pattern of inflammation in male pediatric patients with lichen sclerosus. We also compared the expression of selected miRNAs in lichen sclerosus tissue, adjacent non-lichen sclerosus tissue from the same patient, and healthy male pediatric patients. RESULTS AND DISCUSSION: According to the type of inflammation/lymphocytic distribution, we categorized patients into four groups with the follow

Putative Tissue-Based Biomarkers of Male Genital Lichen Sclerosus-A Systematic Literature Review.

Male genital lichen sclerosis (mGLS) is a chronic progressive inflammatory disease with potential complications including urethral strictures and penile cancer. Despite its clinical significance, the molecular mechanisms underlying mGLS remain poorly understood, and male-specific data are limited. This systematic review consolidates current evidence on tissue-based gene and protein expression in mGLS, aiming to identify commonly investigated biomarkers and highlight gaps in the literature. Using the JBI Sumari interface, our research strategy identified a total of 24 studies with considerable

Pediatric Lichen Sclerosus and Non-Accidental Trauma: A Systematic Review of Distinguishing Features and Co-Occurrence.

BACKGROUND/OBJECTIVES: Lichen sclerosus (LS) is a chronic autoimmune inflammatory skin disease predominantly affecting the anogenital area that can be mistaken for non-accidental trauma (NAT) in pediatric patients, such as sexual abuse. Such misdiagnoses can be distressing for families. This systematic review examined pediatric patients investigated for both LS and NAT to provide clinicians with guidance on distinguishing these conditions. METHODS: MEDLINE, Embase, and Web of Science were searched. Retrospective studies, case series, and case reports describing pediatric patients investigated

Extragenital Bullous Lichen Sclerosus in a Male Pediatric Patient and Dermoscopic Keys: A Case Report and Literature Review.

We describe the case of a 12-year-old male who presented with a one-and-a-half-month history of an asymptomatic, rapidly expanding erythematous hyperkeratotic plaque on the left shoulder. On clinical exam, the patient was also found to have an ivory white atrophic plaque on the left clavicle. Given the classic lichen sclerosus (LS) lesion on the patient's clavicle, the lesion in question on his left shoulder was diagnosed as extragenital bullous LS (EBLS). The bullous variant of LS, while common in adult literature, is rarely reported in the pediatric population. The patient had a moderate cli
