Viral shedding dynamics in coxsackievirus A6 hand, foot, and mouth disease: a prospective, individual-level analysis.

BACKGROUND: Coxsackievirus A6 (CVA6) has emerged as the predominant cause of hand, foot, and mouth disease (HFMD) in China since the introduction of enterovirus A71 (EV-A71) vaccination. A precise definition of the infectious period is critical for effective containment; however, data on the duration and kinetics of CVA6 shedding remain scarce. We therefore aimed to characterize the viral shedding dynamics of CVA6 and to identify the determinants of viral clearance. METHODS: We conducted a prospective longitudinal study enrolling pediatric inpatients with laboratory-confirmed CVA6 HFMD betwee

Historical reconstruction of the earliest enterovirus A71 epidemics in Japan in the 1960s.

Enterovirus A71 was first isolated in California in 1969, with the earliest retrospective detection traced back to 1963 in the Netherlands, but its early spread remains unclear. Using age-specific seroprevalence data from children aged 1-10 years in Kawasaki City, Japan, collected annually from 1966-1973, we applied serocatalytic models to estimate annual force of infection during 1959-1973. Several models were tested, incorporating different assumptions about time-varying force of infection, age-dependent susceptibility, and seroreversion, to identify the best fit to the data. Model compariso

Emergence of Echovirus 11 in Severe and Neonatal Enterovirus Infections: A 9-Year Retrospective Study in Taiwan Before and After the COVID-19 Pandemic.

Enteroviruses (EVs) cause a wide range of pediatric illnesses. In Taiwan, changes in serotype distribution and COVID-19-related public health measures may have influenced recent EV trends. We retrospectively reviewed EV-positive pediatric inpatients (< 18 years) at National Taiwan University Hospital from 2015 to 2023. Cases were identified via PCR or viral culture. Among 678 cases, incidence declined markedly during the COVID-19 period but rebounded after public health restrictions were relaxed, with seasonal peaks in June persisting. Echovirus 11 emerged as the predominant serotype among neo

A Rapid and Ultrasensitive Detection of Coxsackievirus A16 Using Reverse Transcription Multiple Cross Displacement Amplification Combined with the CRISPR-Cas12a-Based Biosensing System.

Coxsackievirus A16 (CVA16) is one of the primary viral etiological agents of hand, foot, and mouth disease (HFMD) in infants and children under five years of age. Prompt and reliable detection of CVA16 is crucial for guiding immediate therapeutic interventions and for implementing effective epidemic prevention and control strategies, particularly in settings with limited resources. Herein, a diagnostic platform for CVA16 (CVA16-RT-MCDA-CRISPR) was developed by combining reverse transcription multiple cross displacement amplification (RT-MCDA) with CRISPR-Cas12a-based detection. In this system,

A single residue in the VP3 capsid protein governs virulence and informs live-attenuated vaccine design for coxsackievirus A6.

UNLABELLED: Coxsackievirus A6 (CVA6) is a major pathogen of hand, foot, and mouth disease, yet the viral determinants of its severe pathogenicity remain undefined, hampering intervention strategies. Here, by comparing a highly lethal clinical isolate (CVA6-HeB) with an attenuated strain (CVA6-TW141), we identified the capsid-encoding P1 region as the primary virulence determinant in neonatal mice. Fine-mapping identified a single critical residue at position 238 of VP3 (VP3-238): glutamic acid (E) in virulent CVA6-HeB versus alanine (A) in attenuated CVA6-TW141. Introducing E238 into the atten

Construction of the Multi-Epitope HFMD Vaccine Based on an Attenuated CVB3 Vector and Evaluation of Immunological Responses in Mice.

Background/Objectives: Hand, foot, and mouth disease (HFMD) is a major public health concern primarily caused by human enterovirus A71 (EV-A71), coxsackievirus A16 (CVA16), coxsackievirus A6 (CVA6), and certain coxsackievirus B serotypes. Currently available EV-A71 vaccines lack cross-protective efficacy against other serotypes, highlighting the urgent need for multivalent and broadly effective enterovirus vaccines. Methods: Immunoinformatics approaches were used to predict highly immunogenic B-cell and T-cell epitopes, which were assembled to construct a novel multivalent epitope vaccine, rCV

Advances in Antiviral Drug Development Targeting Enteroviruses: From Viral Proteins to Host Factors.

Enteroviruses represent important human pathogens, posing a substantial disease burden, particularly in children under 5 years of age. Enteroviruses are the primary causative agents of hand-foot-and-mouth disease (HFMD) and are strongly associated with acute flaccid myelitis (AFM), with severe cases potentially resulting in significant neurological complications. Inactivated vaccines against EV-A71 based on the C4 genotype are currently available. However, there are no licensed direct antiviral agents for severe cases. By focusing on viral proteins and host factors, researchers have made great

Genistein inhibits the replication of enterovirus A71.

BACKGROUND: Genistein, an isoflavone abundant in soybeans and other legumes, has shown antiviral activity against several viruses. Its effects on enteroviruses, particularly EV71-the causative agent of hand, foot, and mouth disease (HFMD)-remain incompletely understood. METHODS: In vitro assays were used to evaluate replication inhibition of EV71, assess mechanistic effects, and determine the compound's broad-spectrum activity against other enteroviruses (EV68 and CA6). In vivo experiments in neonatal mice were conducted to further elucidate the role of genistein in EV71 replication. RESULTS

In silico design and immunogenicity evaluation of a multi-epitope vaccine against EV-A71.

Enterovirus A71 (EV-A71), the primary causative agent of hand, foot, and mouth disease (HFMD), can cause severe neurological complications and even death, particularly in young children. Despite the availability of inactivated vaccines, their protective efficacy has been compromised due to frequent intra- and intertypic recombination events and ongoing mutations among circulating EV-A71 strains. To address this, we employed immunoinformatic approaches and identified conserved epitopes and constructed a multi-epitope vaccine (MEV) candidate against EV-A71. A total of 1,627 structural protein se

String-Powered Microfluidic Chip Integrating Heparin-Mediated One-Pot RT-RPA/CRISPR-Cas12a for Multiplex Detection of HFMD Viruses.

Hand, foot, and mouth disease (HFMD) is a common childhood infection caused by enteroviruses, which exhibit distinct regional and seasonal epidemiological patterns. Wastewater-based epidemiology is a crucial tool for monitoring population infection dynamics and viral subtype distribution. However, the lack of effective on-site viral detection methods limits timely early warning and effective surveillance of infectious disease outbreaks. This study developed a one-pot RT-RPA/CRISPR-Cas12a assay-based, string-powered flywheel microfluidic chip for the multiplex detection of HFMD viruses in waste

A Spatially Encoded Fluorescent Biosensor for Multiplexed and High-Throughput Detection of HFMD Viruses via Parallel Entropy-Driven Circuits and Combinatorial Photonic Crystal Arrays.

Hand-foot-mouth disease (HFMD), mainly caused by Enterovirus 71 (EV71), Coxsackievirus A6 (CVA6), and Coxsackievirus A16 (CVA16), poses a serious threat to children due to its high transmissibility and potential to induce severe complications. Herein, a spatially encoded fluorescence biosensing platform that integrates a parallel entropy-driven circuit (EDC) with a combinatorial photonic crystal (cPC) array was reported for ultrasensitive, multiplex, and rapid detection of HFMD viruses. In this system, three parallel entropy-driven circuits were designed to specifically recognize the VP1 gene
