OBJECTIVES: Illness trends are typically monitored by reportable disease and syndromic surveillance systems, but unanticipated health issues might not be captured. Using diagnosis codes, the New York City Health Department developed a novel data mining process to detect unusual increases in emergency department (ED) visits for any reason.
METHODS: We applied the tree-temporal scan statistic in TreeScan software to ICD-10-CM diagnosis codes for ED visits. We searched for unusual citywide increases in ED visits or hospital admissions, over any recent time period, and at any part of and level on

A female toddler clinically diagnosed with hand, foot and mouth disease (HFMD) was referred by the treating paediatrician, with acute-onset painless diminution of vision for 3 days. She had papulomacular skin rashes and was unable to follow the light stimulus at presentation. On fundus evaluation, bilateral asymmetrical (left eye more severe than the right eye) multifocal full-thickness necrotising retinitis, occlusive haemorrhagic retinal vasculitis, optic disc pallor and left eye exudative retinal detachment (eRD) were noted. Due to the background of HFMD and unexpected prompt response of in

Coxsackievirus A16 (CVA16) is a major causative agent of hand, foot, and mouth disease (HFMD) in China. Based on 35 CVA16-positive strains isolated in Taiyuan (2023-2025), we characterized the post-pandemic evolutionary dynamics. Genomic analysis revealed the highest conservation in VP4 and the lowest in the 3' UTR. Temporally, a shift from B1a/B1b co-circulation to a three-lineage pattern (including a novel B1c subgenotype) was observed. Intertypic recombination was the primary driver, specifically targeting the 5'UTR and VP1/2A junction (B1a with EV71; B1b with CVA4; B1c with both). Cruciall

Viruses, lacking an intrinsic metabolic network, exploit host cell metabolism in order to hijack resources for their own replication. However, the regulatory relationship between foot-and-mouth disease virus (FMDV) and energy metabolism remains incompletely elucidated. Here, metabolomic analysis was performed on PK-15 cells and tonsils infected with FMDV. Metabolites involved in glycolysis were significantly upregulated, whereas metabolites associated with the tricarboxylic acid (TCA) cycle were markedly downregulated in vivo and in vitro, which indicated that FMDV induced reprogramming of hos

BACKGROUND: Enteroviruses impose a significant global health burden, causing outbreaks of Hand, Foot, and Mouth Disease (HFMD) and severe neurological complications. Currently, no specific and broadly effective antiviral therapeutics are available due to the rapid mutation and co-circulation of diverse viral strains. The viral capsid protein VP1 is critical for viral integrity and host cell entry, making it an attractive target for drug development. Myricetin (MC) is a highly safe, naturally occurring flavonol widely distributed in medicinal plants (e.g., Myrica rubra) with well-documented eth

Enterovirus A71 (EV-A71) is a major causative agent of hand, foot, and mouth disease (HFMD) with neurotropism, and currently no approved antiviral therapies are available. In this study, we found that the ondansetron exhibits antiviral activity against EV-A71 infection. Ondansetron significantly reducing viral RNA levels, progeny virus production, and VP1 protein expression in a dose-dependent manner. In vivo, ondansetron treatment increased survival of neonatal mice and decreased viral loads in multiple tissues. Mechanistically, ondansetron modulates cholesterol metabolism-related genes to af

The re-emergence of Coxsackievirus A6 (CV-A6) as a predominant pathogen in hand, foot, and mouth disease (HFMD) underscores the need for ongoing molecular surveillance to clarify local evolutionary dynamics. This study aimed to characterize the genetic features of CV-A6 strains circulating in Baotou, Inner Mongolia, from 2023 to 2024. Throat swabs collected from HFMD patients were screened using real-time quantitative PCR; the VP1 region and complete genomes of representative CV-A6-positive samples were amplified and sequenced. Phylogenetic and recombination analyses were subsequently performe

Hand, foot, and mouth disease (HFMD) is a common viral illness of childhood but is increasingly recognized in adults with often atypical and severe presentations. We report a case of HFMD in an immunocompetent man in his sixties who presented with a generalized, tender vesiculopapular eruption involving the groin, trunk, extremities, palms, and soles. An extensive infectious evaluation was initially pursued, and empiric broad-spectrum antimicrobials were administered. Dermatology consultation led to the diagnosis of HFMD, and antibiotics were discontinued with subsequent clinical improvement.

Hand, foot and mouth disease (HFMD) is a common viral disease caused by a group of enteroviruses within the Picornaviridae family, specifically Coxsackie A16 (Cox A16) and Enterovirus 71 (EV71). Symptoms of HFMD include fever, mouth ulcers and skin rash. It is usually mild and self-limiting but some people may have a serious illness that requires hospitalization due to neurological complications and severe organ impairment, which can result in long-term morbidity and mortality. In rare cases, it can even lead to death. There is no specific treatment for severe cases of HFMD. Objective of the s

Echovirus 3 (E-3) has recently been identified as an emerging pathogen associated with both severe and mild cases of hand, foot, and mouth disease (HFMD) in Guizhou Province, China. Although these strains exhibit distinct clinical phenotypes, their underlying molecular mechanisms remain unclear. This study reports the phylogenetic and genetic features of eight E-3 strains from severe and mild HFMD cases and investigates E-3 seroprevalence in Guizhou. Eight shared nucleotide substitutions in the untranslated regions (UTRs) and six shared amino acid substitutions in the open reading frame (ORF)

Enterovirus 71 (EV71) is a major pathogen causing severe and fatal hand, foot, and mouth disease. Its strong neurotropism and rapid evolution pose an ongoing threat to infants and young children. To date, no specific antiviral drug against EV71 has been approved, leaving a critical gap in clinical management. This review summarizes recent progress in the search for anti-EV71 drugs. The structural features of EV71 and key steps in its life cycle, including entry and replication, are first described. The evolution of drug discovery technologies is then traced, from traditional cytopathic effect-

Enterovirus A71 (EVA71) is a major pathogen responsible for hand, foot, and mouth disease (HFMD) outbreaks and poses a considerable threat to public health. In this work, we established a molecular detection system that integrates reverse transcription multiple cross displacement amplification (RT-MCDA) with CRISPR/Cas12a technology, targeting the VP1 gene region for EVA71 identification. This platform, termed EVA71-RT-MCDA-CRISPR, utilizes RT-MCDA to achieve efficient pre-amplification of the target sequence, followed by sequence-specific recognition mediated by CRISPR/Cas12a. The detection r

Severe hand, foot, and mouth disease (HFMD) poses a risk of serious multi-organ complications in children. While renal dysfunction has been observed in some cases, detailed investigations on kidney injury and molecular mechanisms remain limited. Coxsackievirus A6 (CVA6) has emerged as a major pathogen responsible for HFMD. To reveal the underlying pathways of CVA6-associated renal injury, we enrolled 90 children diagnosed with CVA6 infection and 45 healthy children. The levels of uric acid (UA), creatinine (CREA) and UREA in the serum were measured. We found that serum levels of UA, CREA, and

Coxsackievirus A10 (CVA10) is a major causative agent of hand, foot and mouth disease and utilizes KREMEN1 (KRM1) as its cellular receptor. While our previous work identifies VP2 residue K140 as a universal anchor for KRM1 binding among KRM1-utilizing enteroviruses, the functional significance of other receptor-interface residues remains poorly characterized. Here, through structure-guided mutagenesis, we demonstrate that VP3-T234, a completely conserved residue at the C-terminus of VP3, is essential for CVA10 infectivity. The T234A mutation does not affect virion assembly but abolishes both K
