Lichen Sclerosus-Like Changes in Acrochordons: A Case Series.

Lichen sclerosus (LS) is a chronic inflammatory skin disease typically affecting anogenital regions, with occasional extragenital involvement; however, its occurrence in acrochordons is seldom reported. We present eight cases of LS-like changes within acrochordons in patients with no prior history of LS elsewhere. Histopathologic examination confirmed characteristic LS features localized to the acrochordons. These findings raise questions about whether such changes represent a localized response to friction and/or chronic intermittent pressure-type trauma or a form of primary extragenital LS.

Genital Lichen Sclerosus: From Hidden Mechanisms to Healing Horizons.

Lichen sclerosus (LS) is a chronic inflammatory dermatosis that primarily affects the genital mucosa in both sexes. Although most prevalent in women and characterized by distinct age peaks, its clinical presentation, marked by ivory-white atrophic patches and plaques, often overlaps with other conditions. Delayed diagnosis, frequently due to asymptomatic progression, poses a significant risk of malignant transformation. The etiology of LS is multifactorial, involving a complex interplay of genetic, epigenetic, environmental, and autoimmune factors. Pathogenetically, the disease is characterize

Genital lichen sclerosus with complete introital obliteration presenting with chronic urinary retention.

Lichen sclerosus (LS) is a chronic inflammatory dermatosis with predilection for the anogenital region. Progressive scarring may cause labial adhesions, introital stenosis and in advanced cases, complete obliteration with urinary obstruction. We report a postmenopausal woman with hypothyroidism who presented with chronic urinary retention due to LS-related introital fusion. Surgical adhesiolysis and urethral calibration restored normal voiding. This case underscores the severe sequelae of untreated LS, the value of early diagnosis and the importance of long-term follow-up to prevent recurrence

The urological burden of lichen sclerosis: implications for multidisciplinary surveillance and management.

BACKGROUND: Lichen sclerosis (LS) is a chronic inflammatory condition predominantly affecting the anogenital skin, with genitourinary involvement leading to scarring, phimosis, and voiding dysfunction. The long-term urological sequelae, including urological interventions, remain unexplored in large cohorts. METHODS: Using the TriNetX database, a retrospective propensity score-matched cohort analysis was conducted, comparing 36,974 patients with LS to 36,974 controls matched by demographics, diabetes mellitus, tobacco use, recurrent urinary tract infections, immunosuppressant use, and prior cy

Transcriptomic characterization of mesenchymal and skin tissues in Cervus elaphusxanthopygus antler and identification of growth-related candidate genes.

BACKGROUND: Deer antler is the only mammalian organ capable of periodic complete regeneration. Its astonishing growth rate-reaching several centimeters per day-provides a unique model for research in tissue regeneration and developmental biology. This rapid growth relies on the protective and signaling functions of the skin, as well as the vigorous proliferation and differentiation capacity of mesenchymal cells. Although previous studies have identified some key factors involved in antler growth, systematically comparing the transcriptomic profiles of these two core tissues (skin and mesenchym

Exploring better methods for treating urethral stricture caused by lichen sclerosus.

Lichen sclerosus (LS) is a chronic inflammatory dermatosis significantly associated with urethral stricture disease (USD), particularly affecting the genitalia in both sexes. While topical corticosteroids remain the first-line therapy for LS, their efficacy for deep urethral involvement is limited. Surgical intervention, primarily urethroplasty utilizing buccal mucosa grafts or lingual mucosa grafts, is often required for LS-associated USD but is associated with risks of recurrence and complications. This review explores the etiology, highlighting the roles of immune dysregulation, genetic fac

Elevated expression of immune checkpoints and pro-inflammatory cytokines as potential biomarkers in pediatric Vulvar Lichen Sclerosus.

Vulvar Lichen Sclerosus (VLS) is a chronic inflammatory dermatosis of unknown etiology affecting the external genitalia. In pediatric patients, it can lead to significant discomfort and progressive structural changes in tissues. In recent years, there has been increasing interest in the participation of immune checkpoints and inflammatory mediators in the pathogenesis of chronic diseases, including VLS. Immune checkpoints, such as PD-1, PD-L1, CTLA-4, CD200, and CD200R, play a crucial role in modulating the immune response and may serve as potential diagnostic markers and therapeutic targets.

Platelet-rich plasma in the management of genital lichen sclerosus: A review of mechanisms, evidence, and future directions.

Lichen sclerosus (LS) is a chronic, relapsing, inflammatory skin condition that primarily affects the anogenital region. Treatment with potent topical corticosteroids (TCS) can help control symptoms for many patients, but complications such as atrophy and scarring often persist. Platelet-rich plasma (PRP) has emerged as a regenerative therapy with potential application in LS. We review the clinical evidence and highlight gaps and future research needs. We searched PubMed and Web of Science databases for studies of PRP in LS. The outcomes evaluated included symptom relief, quality of life (QoL)

A Case of Extragenital Lichen Sclerosus: Beyond the Anogenital Region.

Lichen sclerosus et atrophicus is a chronic inflammatory dermatosis that predominantly affects the genital region. Its exclusively extragenital presentation is rare. We describe the case of a 78-year-old male patient with a history of colon cancer treated with chemotherapy and radiotherapy, who presented with lesions clinically, dermoscopically, and histopathologically consistent with extragenital lichen sclerosus. The patient received treatment with high-potency topical corticosteroids (0.05% clobetasol propionate) and calcineurin inhibitors (0.1% tacrolimus), achieving significant clinical i

940-nm Photobiomodulation Enhances ADSC-Mediated Tracheal Cartilage Repair through TRPV1-Associated Calcium Signaling and Mitochondrial Redox Remodeling.

BACKGROUND: : Azathioprine and mercaptopurine are commonly used in chronic active Crohn's disease. They share the disadvantage of a delayed onset of action and potentially serious side-effects, and are metabolized to thioguanine nucleotides which are thought to be the active metabolites. The direct use of 6-thioguanine may offer a more rapid and safer alternative. We conducted an open prospective study to investigate the efficacy and safety of 6-thioguanine in chronic active Crohn's disease. METHODS: : Thirty-seven patients with chronic active Crohn's disease and a Crohn's disease activity in

Evidence- and consensus-based guideline on lichen sclerosus.

The German-language, consensus- and evidence-based S3 guideline on lichen sclerosus (LS) was developed based on the European "EuroGuiDerm Guideline on lichen sclerosus" under the leadership of the German Dermatological Society (DDG) and the German Society for Gynecology and Obstetrics (DGGG). Particular emphasis was placed on adapting the recommendations to the healthcare conditions in German-speaking countries. The interdisciplinary guideline development group consisted of 24 experts from 16 medical societies and actively included patient representatives in the development process. The guidel

Assessing General Practitioners' Confidence in Undertaking Assessment and Ongoing Management of Patients With Diagnosed, Stable Vulval Lichen Sclerosus (VLS): Laying the Foundation for a Shared-Care Model.

BACKGROUND: Vulval Lichen Sclerosus (VLS) is a chronic inflammatory condition requiring lifelong management to prevent severe complications including malignancy. A shared care model between general practitioners (GPs) and public non-GP specialists (dermatologist or gynaecologist) for patients with stable LS would alleviate the demand on public vulval services. This study aims to assess GPs' level of comfort in performing vulval examinations for patients with VLS which would underpin the feasibility of shared care. It also explores GPs' level of confidence in vulval health, preferences for futu
