Lyme borreliosis.

Lyme borreliosis is the most common tick-borne disease in the northern hemisphere. It is a zoonosis caused by several species of Borrelia burgdorferi sensu lato and transmitted by the bite of infected ticks of the Ixodes ricinus complex. Lyme borreliosis in North America and Europe differs in certain respects, likely reflecting the different Borrelia species that cause human disease in these locations. The earliest manifestation of Lyme borreliosis is the skin lesion erythema migrans, which develops at the tick bite site, typically 7-14 days after the bite. Some untreated patients will then (w

Characterization of Anti-Phospholipid Antibodies in Lyme Borreliosis Using In-House Developed ELISAs.

OBJECTIVES: Borrelia burgdorferi sensu lato, a spirochete bacterium responsible for Lyme borreliosis-the most common tick-borne infection in North America and Europe-can trigger the production of antiphospholipid antibodies. These antibodies target host lipids such as cardiolipin (CL), phosphatidic acid (PA), phosphatidylcholine (PC), and phosphatidylserine (PS), which the spirochete incorporates into its membrane from the surrounding environment. Although antiphospholipid antibodies are typically associated with antiphospholipid syndrome (APS), they may also arise during infections, including

Analytical comparison of 11 CE-IVD marked real-time PCR assays for Borrelia burgdorferi sensu lato detection.

PURPOSE: In some precise clinical manifestations of Lyme borreliosis, PCR can be helpful for diagnosis: suspicion of atypical erythema migrans, borrelial lymphocytoma, acrodermatitis chronica atrophicans, Lyme arthritis or in very early seronegative neuroborreliosis. This study aimed to evaluate: (1) the analytical sensitivity of commercial real-time PCR kits for Borrelia burgdorferi sensu lato (Bbsl), (2) their specificity, and (3) their practicability. METHODS: The performances of 11 CE-IVD marked commercial kits and the French NRC Borrelia's in-house PCR were evaluated. PCRs were performed

Guidelines for Lyme borreliosis: treatment.

The aim of the clinical practice guideline is to support healthcare professionals in the diagnosis and management of patients with proven or suspected Lyme borreliosis (LB). The guideline provides an evidence-based algorithm for antibiotic therapy in confirmed LB cases and outlines recommendations for complex presentations where the three diagnostic criteria are not fully met, suggesting a possible challenge antibiotic therapy in specialized settings. Updated in accordance with international literature and previous French recommendations (HAS, 2022), the guidelines detail antibiotic selection

Case presentation and management of Lyme disease patients: a 9-year retrospective analysis in France.

INTRODUCTION: Lyme borreliosis (LB) is the most common vector disease in temperate countries of the northern hemisphere. It is caused by Borrelia burgdorferi sensu lato complex. METHODS: To study the case presentation of LB in France, we contacted about 700 physicians every year between 2003 and 2011. An anonymous questionnaire was established allowing the collection of 3,509 cases. The information collected was imported or directly entered into databases and allowed identifying variables that were validated in a multiple correspondence analysis (MCA). RESULTS: Sixty percent of the cases wer

[Cutaneous Lyme disease: not always a rash with a central clearing].

During the past four decades the number of reported Lyme disease diagnoses in the Netherlands has increased to 27.000 a year, with a yearly incidence of Lyme disease between 111 (95% CI 106-115) to 131 (95% CI 126-136) per 100,000 person years. A large part of all Lyme disease diagnoses concern the skin; in the Netherlands, 77-89% erythema migrans, 2-3% borrelia lymfocytoom and 1-3% acrodermatitis chronica atrophicans. These skin manifestations have a variable clinical expression, reason why they can be difficult to diagnose. Early recognition and treatment is important to prevent the developm

Bridging the gap: Insights in the immunopathology of Lyme borreliosis.

Lyme borreliosis (LB), caused by Borrelia burgdorferi sensu lato (Bbsl) genospecies transmitted by Ixodes spp. ticks, is a significant public health concern in the Northern Hemisphere. This review highlights the complex interplay between Bbsl infection and host-immune responses, impacting clinical manifestations and long-term immunity. Early localized disease is characterized by erythema migrans (EM), driven by T-helper 1 (Th1) responses and proinflammatory cytokines. Dissemination to the heart and CNS can lead to Lyme carditis and neuroborreliosis respectively, orchestrated by immune cell inf

Evaluation of the Epitogen Lyme Detect IgG ELISA: a novel peptide multiplexing approach.

Lyme Borreliosis (LB), or Lyme disease, is a growing health concern caused by Borrelia burgdorferi sensu lato (Bbsl) bacteria transmitted through tick bites, and untreated cases can lead to severe health complications. Existing serology tests, while valuable, have low sensitivity in early infection stages where diagnosis is vital, interpretation variability, and false positives from cross-reactivity, while direct detection methods also suffer from low sensitivity, due to the inconsistent presence of Bbsl components in clinical samples. This study validated the diagnostic performance of the nov

Clinical characteristics and serological profiles of Lyme disease in children: a 15-year retrospective cohort study in Switzerland.

BACKGROUND: Lyme disease (LD) is caused by Borrelia burgdorferi and is the most common tickborne disease in the northern hemisphere. Although classical characteristics of LD are well-known, the diagnosis and treatment are often delayed. Laboratory diagnosis by serological testing is recommended for most LD manifestations. The objective of this study was to describe clinical characteristics and associated serological profiles in children with LD. METHODS: This retrospective cohort study included children aged 0-18 years, diagnosed with LD according to current guidelines at University Children'

High-risk groups for alpha-gal sensitization.

BACKGROUND: Tick bite-induced IgE-mediated reactions to the oligosaccharide galactose α-1,3-galactose (alpha-gal) are increasingly recognized. This study investigated alpha-gal sensitization in three groups with different tick bite exposure. MATERIALS AND METHODS: Specific IgE antibodies to alpha-gal and total IgE were investigated in 485 patients with Lyme borreliosis with different disease manifestations and compared to a control group of 200 randomly selected patients without increased exposure to tick bites. A group of 232 hunters and forest workers served as a model for multiple tick bit
