Hydroa Vacciniforme: When and How to Suspect It.

Hydroa vacciniforme (HV) is a rare pediatric photosensitive dermatosis characterized by sun-induced recurrent lesions on exposed areas and subsequent atrophic scarring, with its pathogenesis remaining unclear. This report aims to improve clinicians' recognition of pediatric HV and provide practical references for its clinical diagnosis and management by detailing a typical case in a young child. We describe a 3-year-old male patient with recurrent erythema, blisters, erosion, crusts, and atrophic scars on the face, nasal dorsum, and auricles for over one year. His skin lesions worsened signifi

Topical Retinoids in Acne Vulgaris and Acne Scars-From Monotherapy to Combining Regimens.

Topical retinoids are the cornerstone of the treatment of multiple dermatological conditions. Long established in acne therapy, they exert effects on keratinization, inflammation, fibroblast activity, and collagen remodeling, suggesting a potential role in both the prevention and treatment of acne scars. This narrative review summarizes current evidence on the use of topical retinoids in acne vulgaris and acne scarring, focusing on different retinoid molecules, formulation technologies, and combination strategies. A review of published clinical and experimental studies evaluating tretinoin, ad

Adherence, Persistence, and Safety of Risdiplam in Spinal Muscular Atrophy: A Population-Based Cohort Study.

INTRODUCTION: Spinal muscular atrophy (SMA) is a rare neuromuscular disorder caused by biallelic SMN1 variants, partially modulated by SMN2 copy number. Risdiplam, an oral SMN2 splicing modifier, has demonstrated efficacy in SMA. However, long-term adherence and persistence are key to sustaining benefit. We evaluated real-world adherence, persistence, and safety of risdiplam in a population-based cohort. METHODS: This was a retrospective observational study including all genetically confirmed SMA type 1-3 patients treated with risdiplam in Spain between January 2020 and October 2025. Adherenc

Late-Onset En Coup de Sabre: A Rare Presentation of Linear Scleroderma in an Elderly Woman.

En coup de sabre (ECDS) is a rare form of linear scleroderma that typically presents in childhood and may lead to progressive craniofacial tissue atrophy. Adult-onset disease is uncommon and may delay recognition. Early identification is important to prevent permanent deformity and potential neurologic complications. A 60-year-old woman with no significant past medical history presented with a nine-month history of progressive hyperpigmentation, skin tightening, and indentation involving the nasal bridge and left frontoparietal region. Over time, the discoloration extended toward the right for

Cationic bakuchiol-loaded lipid nanoparticles enhance skin permeation and modulate neuro-immune inflammation and itch pathways in atopic dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disorder characterized by recurrent pruritus and epidermal barrier dysfunction. The long-term use of topical corticosteroids is limited by local and systemic adverse effects, including skin atrophy. Here, we developed a cationic lipid nanoparticle (LNP) platform for the topical delivery of bakuchiol (BKC), a hydrophobic plant-derived bioactive with potent anti-inflammatory but limited aqueous solubility and skin permeation. The formulated BKC-LNPs showed high encapsulation efficiency and stable nanoscale physicochemical properties. By promo

Lesions Reported as Plaque-Like CD34-Positive Dermal Fibroma Reveal Two Reproducible Clinicopathologic Patterns: Two Cases and Literature Review.

Plaque-like CD34-positive dermal fibroma (PDF), which historically overlaps with medallion-like dermal dendrocyte hamartoma (MDDH), is a rare benign CD34-positive spindle-cell lesion with highly variable clinicopathologic descriptions. We analyzed two adult-onset cases and reviewed the published literature to determine whether reported PDF/MDDH lesions cluster into reproducible clinicopathologic patterns. Our literature review comprises 25 cases. Comparative analyses revealed marked differences in age at onset, lesion size, anatomic distribution, depth of involvement, papillary dermis sparing,

Cutaneous alpha-synuclein deposition informs autonomic function in individuals with early-stage multiple system atrophy.

PURPOSE: Multiple system atrophy is a progressive neurodegenerative disorder characterized by autonomic failure, parkinsonism, and cerebellar ataxia. Phosphorylated alpha-synuclein in skin nerves has emerged as a promising biomarker, but longitudinal studies remain limited. This study evaluated changes in cutaneous alpha-synuclein deposition over time, its association with autonomic dysfunction, and its concordance with seed amplification assay findings. METHODS: Seventeen participants with probable multiple system atrophy were followed up for 12 months with clinical assessments, skin biopsie

A Case of Extragenital Lichen Sclerosus: Beyond the Anogenital Region.

Lichen sclerosus et atrophicus is a chronic inflammatory dermatosis that predominantly affects the genital region. Its exclusively extragenital presentation is rare. We describe the case of a 78-year-old male patient with a history of colon cancer treated with chemotherapy and radiotherapy, who presented with lesions clinically, dermoscopically, and histopathologically consistent with extragenital lichen sclerosus. The patient received treatment with high-potency topical corticosteroids (0.05% clobetasol propionate) and calcineurin inhibitors (0.1% tacrolimus), achieving significant clinical i

Acanthosis nigricans as a diagnostic clue for familial partial lipodystrophy type 2: a case report with review of literature on Japanese cases.

Familial partial lipodystrophy (FPLD) is a rare inherited disorder characterized by limb adipose tissue atrophy and metabolic abnormalities, including severe insulin resistance. However, diagnosis is often delayed because the characteristic physique can be difficult to recognize without a high index of suspicion. We present the case of a 19-year-old Japanese woman diagnosed with FPLD type 2 (FPLD2). She had a history of irregular menses and hirsutism since adolescence. The FPLD2 diagnosis was prompted by her concerns about axillary and inguinal pigmentation. Her mother's online search first su

Early Palliative Care in Degos Disease: A Case for Proactive Support in Rare, Life-Limiting Illnesses.

Kohlmeier-Degos disease is one of the most rarely noted diseases in the medical literature. Due to its rarity, many individuals are likely to go undiagnosed or to be diagnosed in more advanced stages. In those individuals with the disease, the initial presentation consists of characteristic skin lesions (porcelain-white macules). These lesions can remain limited to the skin only, which indicates benign disease. Unfortunately, some patients will go on to develop systemic disease, which is life-threatening and can form lesions in the gastrointestinal tract, central nervous system, heart, and lun

Neuro-Ophthalmic Presentation of Steroid 5a-Reductase Type 3 Congenital Disorder of Glycosylation: A Case of Monozygotic Twins.

We report a very rare autosomal recessive metabolic disorder in monozygotic twin sisters caused by the steroid 5a-reductase type 3 (SRD5A3) gene defect, a subtype of congenital disorder of glycosylation (CDG). SRD5A3 activity is required for N-glycosylation of proteins. This step is important for the protein to gain its function. The condition is characterized by severe neurodevelopmental delay, cerebellar atrophy or hypoplasia, ocular abnormalities, and ichthyotic skin changes. We describe 20-month-old female monozygotic twins born to non-consanguineous South Asian parents. Notably, both twin

Generation and characterization of human induced pluripotent stem cells from neuropathologically confirmed multiple system atrophy patient-derived fibroblasts.

BACKGROUND: Multiple system atrophy (MSA) is an adult-onset, fatal neurodegenerative disease, classified as a synucleinopathy along with Parkinson's disease and dementia with Lewy bodies. The etiology of MSA is unknown and the treatment remains symptomatic with limited efficacy. To date most studies have been conducted on genetic mice models, engineered to overexpress α-synuclein (aSyn); and transmission mice models, treated with intracerebral injections of MSA brain extracts or fibrillar aSyn which can be assembled de novo or using MSA seeds. An in vitro human model is urgently needed to iden

Longitudinal Decline in Electrochemical Skin Conductance Reflects Disease Progression in Multiple System Atrophy.

BACKGROUND: Multiple system atrophy (MSA) is characterized by progressive autonomic/motor dysfunction, but robust biomarkers do not exist. Electrochemical skin conductance (ESC) provides a noninvasive measure of sudomotor function, but its longitudinal predictive value and prognostic relevance in MSA remain insufficiently studied. We aimed to comprehensively evaluate ESC in a large longitudinal cohort. METHODS: We analyzed 175 patients with MSA followed for a mean of 4.2 ± 2.1 years. Disease severity was assessed using UMSARS Parts 1 and 2, COMPASS31, SCOPA-AUT, and orthostatic blood pressure

Cutaneous α-Synuclein Pathology as a Differential Marker: A Histological and Statistical Comparison across Neurodegenerative Disease Groups.

There is an urgent need for early and accurate biomarkers of neurodegenerative disorders. Due to the high innervation and accessibility of the skin, a skin biopsy is a minimally invasive method of detecting phosphorylated α-synuclein (p-α-syn) and assessing intraepidermal nerve fiber density (IENFD). We analyzed biopsies taken from the back and the leg of patients with parkinsonian syndromes (Park.sy.), α-synucleinopathies, multiple system atrophies (MSA), tauopathies, and other neurological disorders, as well as from healthy controls. Double immunofluorescence was performed for p-α-syn (Ser12

Long-Standing Unilateral Verrucous Papules of the Hand Revealing Hypertrophic Lichen Planus: A Diagnostic Challenge.

Lichen planus (LP) is an inflammatory disorder that can involve the skin, nails, hair follicles, and mucous membranes and may present with diverse clinical morphologies that mimic other dermatologic conditions. Hypertrophic (verrucous) LP is a chronic variant characterized by hyperkeratotic lesions that most often affect the lower extremities, which makes isolated hand involvement diagnostically challenging. We report a 65-year-old man with skin-colored, dermally embedded verrucous papules confined to the right hand with extension to the wrist, evolving for more than 40 years. He denied prurit

Connecting Skin and Gut in Dermatitis Herpetiformis: Insights from Gluten Challenge Transcriptomics.

Dermatitis herpetiformis (DH) is a dietary gluten -induced autoimmune blistering skin disease. It is hypothesized to be initiated by a small intestinal mucosal immune response. Gluten-free diet (GFD) is the primary treatment for DHdermatitis herpetiformis and effectively resolves skin lesions. Previous studies show that after gluten reintroduction, peripheral cell responses are detectable already in six6 days, and most patients relapse clinically within months. The relapse presentation may differ between patients, with some experiencing both rash and small intestinal mucosal damage and some on

Transverse Histological Analysis of Scalp Seborrheic Dermatitis: A Novel Perspective.

BACKGROUND: Seborrheic dermatitis is a chronic inflammatory disease affecting seborrheic areas of the skin, including the scalp. It is often confused with scalp psoriasis due to similar clinical and histopathological features. OBJECTIVE: This study described microscopic changes in the pilosebaceous unit in seborrheic dermatitis (SDS) using cross-sectional histological sections in order to improve diagnostic accuracy in differentiating SDS from scalp psoriasis and early-stage alopecia. METHODS: This study included 35 men aged 18-65 diagnosed with SDS (both mild and severe cases). Scalp biopsi
