The collagen fibers of the nodules and whorl-like figures in hypertrophic scars are "coated" with proteoglycans, mainly chondroitin-4-sulfate. The latter was shown to prevent collagenase from breaking down collagen. This suggests that the presence of great amounts of chondroitin-4-sulfate in hypertrophic scars may contribute to the overabundance of collagen deposition which is characteristic of this abnormal healing process.

This report deals with the morphological changes in the hypopharynx-esophageal region of two patients following laryngectomia. In these patients we found two sagittal infoldings of the lateral walls of the hypopharynx-esophageal region. Morphologic and biochemical analyses were performed to compare normal skin and mature scars to hypertrophic scar and keloid. Correlation of morphologic findings with biochemical profiles of the skin and scar samples proved feasible and enlightening. The authors used the technique of Z-palsty in the surgical treatment of hypertrophy of the superior labial fraenum. They consider that this operation is indicated when there is hypertrophy of the fraenum with a low insertion, associated with an inter-incisor diastema, and that the lateral incisors have appeared without causing the diastema to disappear. This technique eliminates the possibility of a hypertrophic scar, a possible source of failure.

By modifying the wound healing process, it is possible to deal effectively with most abnormal forms of scarring, through perhaps 15 per cent of these lesions cannot be managed to the satisfaction of surgeon and patient. In summary, the commonest late physical complications of the burn injury have been reviewed and an approach to their management described. An understanding of the pathophysiology of the conditions is invaluable in planning one's approach to these problems. The techniques to control and correct chronic wound and hypertrophic scar and contractural deformities have been described. We present only a brief summary of the techniques found most reliable in the management of a large group of burned children over many years.

We report a case of a traumatic neuroma of the nose. It was an unusual site for two distinct types of tissue response to skin and soft trauma that exceeded the reparative needs of the body. In the first instance, a small cutaneous nerve was sectioned, and its inability to establish continuity distally resulted in a neuroma. Hypertrophic scar tissue has been observed to soften and thin following Z plasty without removal of any of the scar tissue. Twenty-one patients with familial amyloidosis and polyneuropathy have been examined for the presence of skin lesions, localized to the lower legs and feet. There is abundant evidence in the literature that keloids and hypertrophic scars are different stages of the same process. The evidence, clinical and histogenetic, also shows that the hypertrophic scar is the earlier of the two stages. It therefore follows that all keloids do evolve through the hypertrophic scar phase. These conclusions have largely eased the confusion of nomenclature and made a clinicopathologic correlation of the available knowledge on keloid possible.

Low dose superficial x-irradiation to the incisional site after surgical excision of a keloid prevented keloid or hypertrophic scar formation in 31 of 35 patients evaluated (88%). The clinical manifestation of keloids, physiology of wound healing, pathophysiology of keloid formation, and the radiation technique utilized are presented. This study shows that the method is well tolerated, easily administered, and efficient in inhibiting recurrence of keloid or hypertrophic scar formation.
